This document outlines an implant case planning format that includes: 1) Chief complaint and patient history focused on previous implant treatment 2) Oral examination including jaw relationship, alveolus dimensions, anatomical considerations, periodontal situation, and occlusion 3) Imaging including panoramic x-rays, periapical x-rays, cone beam CT, and consideration of virtual planning 4) Determining whether a fixed or removable restoration is needed
