10. Cascade Effect / Domino Effect
• A cascade effect is an inevitable and sometimes
unforeseen chain of events due to an act affecting
a system.
• In biology, the term cascade refers to a process
that, once started, proceeds stepwise to its full,
seemingly inevitable, conclusion.
• A domino effect or chain reaction is the cumulative
effect produced when one event sets off a chain of
similar events.
• It typically refers to a linked sequence of events
where the time between successive events is
relatively small.

14. Controlling the spread of fire
•The larger a fire, the greater its
destructive potential.
•The control of fire movement, or fire
spread, is discussed in four categories:
1. within the room of origin;
2. to other rooms on the same level;
3. to other storey of the same building;
4. to other buildings.

24. NATECH: Natural Hazard Triggering
a Technological Disaster
• There is growing evidence that natural disasters can
trigger technological disasters, and that these joint
events (also known as NATECHs) may pose
tremendous risks to regions which are unprepared
for such events.
• However, there is scarce information available on
the inter actions between natural disasters and
simultaneous technological accidents.

29. A Black Swan is an event with the following three attributes.
1. First, it is an outlier, as it lies outside the realm of regular expectations,
because nothing in the past can convincingly point to its possibility.
Rarity -The event is a surprise (to the observer).
2. Second, it carries an extreme 'impact'.
Extreme “impact” - the event has a major effect.
3. Third, in spite of its outlier status, human nature makes us concoct
explanations for its occurrence after the fact, making it explainable and
predictable.
Retrospective (though not prospective) predictability
- After the first recorded instance of the event, it is rationalized by
hindsight, as if it could have been expected; that is, the relevant data were
available but unaccounted for in risk mitigation programs. The same is
true for the personal perception by individuals.

66. Rischio, Rischio, Rischio (CPI)
tipico dell'attivita. L'individuazione delle aree a rischio specifico:
a. riportata nella regole tecniche verticali;
b. in assenza, e’ effettuata dal progettista secondo criteri
generali.

84. Soluzioni progettuali
• Soluzione conforme: soluzione di immediata
applicazione, che garantisce il raggiungimento del
livello di prestazione (Soluzione progettuale
prescrittiva che non richiede ulteriori valutazioni)
• Soluzione alternativa: il progettista è tenuto a
dimostrare il raggiungimento del livello di
prestazione (Soluzione progettuale prestazionale
che richiede ulteriori valutazioni).

90. Mechanical Analysis
• The mechanical analysis shall be performed for
the same duration as used in the temperature
analysis.
• Verification of fire resistance should be in:
• in the strength domain: Rfi,d,t ≥ Efi,requ,t
(resistance at time t ≥ load effects at time t);
• in the time domain: tfi,d ≥ tfi,requ
(design value of time fire resistance ≥ time required)
• In the temperature domain: Td ≤ Tcr
(design value of the material temperature ≤ critical
material temperature);

126. spiegazióne s. f.
[der. di spiegare; cfr. lat. explicationis]
plausibile.

127. Forensic Engineering: definitions
• Forensic Engineering is the application of
engineering methods in determination and
interpretation of causes of damage to, or failure
of, equipment, machines, or structures.
• Forensic Engineering is the application of the art
and science of engineering in matters which are
in, or may possibly relate to, the jurisprudence
system, inclusive of alternative dispute
resolution.

137. evidenza: rilievo materiale o documentale a carattere probatorio;
deduzione deriva dalla singola evidenza;
altre evidenze;
la compatibilità totale compara le singole compatibilità.
137
franco.bontempi@uniroma1.it
- se l’evidenza è compatibile con la deduzione di massima (phase 1);
- se la deduzione riscontrata è accettabile e conseguenziale alle cause (phase 2);
- se le cause hanno carattere di compatibilità con quanto già affermato (phase 3);

139. Cause e errori nella Fire Investigation (1)
Condizioni latenti (latent condition)
Derivano dalle decisioni assunte dal
management di qualsiasi livello: in ogni
momento si trovano endemiche
nell’organizzazione e possono insorgere da
decisioni sbagliate non riconosciute.
Fattori contribuenti (contributory factors)
Sono quei fattori che influenzano la
performance della struttura le cui azioni
hanno effetto sulla funzionalità,
determinando un problema strutturale.
(After Reason)
139
franco.bontempi@uniroma1.it
rischio incendio.
Errori attivi (active failure)
Sono azioni non sicure effettuate da coloro
che sono nelle interfacce estreme del
sistema organizzativo. Questi atti non sicuri
sono influenzati dai fattori contribuenti,
come l’inadeguato addestramento
(manutenzione impianti antincendio) che
producono effetti negativi sulle
performance individuali.
Errori latenti (latent failure)
(After Reason)
Cause e errori nella Fire Investigation (2)
140
franco.bontempi@uniroma1.it
141. Errore umano (human error)
sicurezza. In generale è l’azione od
omissione che determina insuccesso nel
compimento di un’azione pianificata come
disegnata, ovvero l’inidoneità di quanto
pianificato al raggiungimento dello scopo.
Violazioni
Si determinano per allontanamento dalle
regole di pratica o di procedura. Si
differenziano dagli errori rule-based in

In linea
generale essi posso essere classificati nel modo
seguente:
segni a colonna e a clessidra (column or hourglass);
segni a V o a cono (cone) e a cono rovescio (inverted
cone) e a U e a doppia U;
segni di protezione (signs of protection);
segni da liquidi infiammabili (signs of fiammable
liquids);
segni da afflusso di O2 su pareti (floor jet);
segni da combustione pulita (clean burn);
calore (line of smoke or heat). 157

165. Effetti dell’incendio sulle strutture:
teso a mostrare in maniera semplice e sintetica
quali sono le potenzialità delle moderne tecnologie
e come possono aiutare il tecnico nelle sue
decisioni.
