• Faith based nonprofit organization that operates mainly as a food and clothing pantry offering a
case management model that determines the root causes of food insecurity and crisis while
encouraging each client to motivate them to become self-sufficient.
• Along with food and clothing, CHRC offers a variety of services including English 101, financial
literacy/management, crafts class, crocheting, scrapbooking, couponing, female heard of household
support group, parenting classes, grief class, Zumba, financial budgeting, and cooking well for
diabetes. This is being addressed by offering a variety of classes that earn clients additional points
for use in the food pantry.
• Advanced Interns (Interns who are in their second field placement.) are allowed to conduct
counseling sessions under the Supervision of the Field Director.
3. Client Description
Client is an elderly Hispanic male 68 years old who dropped out of highschool in the 10th
grade. Client is happily divorced and currently lives alone. Client has a very good
relationship with his children, very proud of them, 3 out of 4 are Educators. Client is taking
medication for depression, anxiety, high blood pressure, and cholesterol. Client was
formally diagnosed with depression and anxiety.
4. Presenting Problem
Client was referred by his mentor who addressed his needs for food and
clothing to receive counseling services. Client shared that he has a fear of
being around crowds of people, participating in groups, and speaking in
front of a group of people. A requirement for clients to remain in the
program include engaging in the program by following the
recommendations of his psychiatrist and attending classes such as the
mandatory financial management class. Client does well on his medication
however when client forgets to take his medications client is unable to
keep his appointments.

• continue receiving counseling services,
• take medications as prescribed,
• attend the mandatory financial management class
• Follow the advice of psychiatrist and participate in group therapy

Client was informed that I am a social work intern under the supervision
of my field instructor and that I am not a professional counselor.
Client was also informed my placement would be completed soon
therefore I made arrangement to collaborate with another social work intern

Client was encouraged to keep a stress diary.
Will work with client on rating his symptoms
mild, moderate, or severe and develop an action plan
for those symptoms so that they won't progress to the next stage.
Will also teach client different skills and techniques to
manage his anxiety symptoms at each stage (mild, moderate, and severe)
starting with one skill/ intervention at a time
so that client can increase the tools in his toolbox.

Diagnostic capabilities- Client shared that he does not feel depressed,
That he enjoys singing and spending time with his daughters however
People assume that he is depressed based on his social withdrawal and
isolation. This intern feels that client’s diagnosis should be Social Anxiety
Disorder with Panic Attacks
Collaboration with medical and mental health providers-If involved in client’s
treatment and working with psychiatrist, this intern would recommend an
anxiety medication with a longer half life.
