Forget Me Not Days is Bankers Life’s signature charitable giving event to
benefit the Alzheimer’s Association. Each year, agents and employees
hit the streets to raise money and awareness for Alzheimer’s disease.
In exchange for a contribution, donors receive packets of Forget-Me-
Not flower seeds to be planted in honor of the more than five million
people with Alzheimer’s.
Our 13th annual event will take place on June 5 6, 2015.
One hundred percent of the proceeds go directly to the
Alzheimer’s Association.

Register Your Office
Visit http://bnk.rs/16w7HoO to register your office for Forget Me Not Days.
Select a Forget Me Not Days Coordinator
Choose a coordinator from your office to be in charge of planning the
event. It’s also recommended that you appoint a committee to help the
coordinator in the planning efforts.
If you are coordinating this year’s event, consider the tips below:
»» Book collection locations early
»» Call your Alzheimer’s Association within a week of registering.
Click here to see which chapter you are matched with.
»» Do a little planning each week leading up to the event.
»» Recruit as many volunteers as possible.
»» Keep your event manageable. It’s best to stick to collecting in
a couple locations—complicated “extra events” can be time
consuming and expensive.
»» Create an event schedule for the day of the event (see page 10
for example).
»» Don’t try to tackle too much alone—delegate some tasks to others.
On-Demand Videos
This year, on-demand training videos will be released leading
up to Forget Me Not Days 2015.
Stay tuned to the WIRe, Perspectives, https://bspn.bankers.com/
forget-me-not-days, or the newsletters coordinators will start to receive
upon registration to view these videos as they become available.
Before the Event
8
Working with the Alzheimer’s Association
The Alzheimer’s Association has a network of over 80 chapters
nationwide. Click here to find out which chapter your office is
matched with.
Step 1: Call your local chapter as soon as possible.
Step 2: Share your event plan:
»» Collection Locations
»» Dates and Times
»» Expected number of volunteers
Step 3: Ask for the chapter’s feedback on your plan and, if possible,
if they can help recruit volunteers, speak to the branch, provide some
supplies—bracelets, pamphlets, etc, and/or attend the event.
Some large chapters may be able to offer their assistance promoting
the event or recruiting volunteers, but please keep in mind that many
chapters are small and only staffed by one or two part-time volunteers.
Chapters are extremely thankful for the funds you raise to support
their programs, but may not be as accessible as you would like. It’s
each branch’s responsibility to take ownership of its event and oversee
coordination and logistics.
Chosing a Collection Location
Picking a good location with heavy foot traffic is the best way to
increase your collection total.
Consider these suggestions:
»» Grocery/Department Stores
Setting up a table outside a busy a store guarantees a steady
stream of people who have their wallets on them!
General steps to reserve these locations:
1. Call the store manager to ask for permission to set up a
charity collection table in front of their store.
2. Explain that 100% of the donations collected benefit the
Alzheimer’s Association.
3. Mention the date, time, and number of volunteers you expect.
4. If the store asks you to submit a request in writing, click here to
download, personalize, and print the Retail Permit Template.
5. Some stores may request a copy of the Alzheimer’s Association
chapter’s 501(c) (3) letter proving that the chapter is a nonprofit
organization. You can request a copy from your local chapter.
6. A few weeks prior to your event, call to reconfirm your date
and time with the store.
Event Tip
Some stores will match your
collection amount at their location
with a “matching grant” contribution
of their own—instantly doubling
your collections!
Contacts
If you have trouble contacting
your chapter, please contact Erica
Smith at erica.smith@cnoinc.com
for assistance.
To learn more about the Alzheimer’s
Association, visit alz.org®.

»» Downtown Areas
Street corners or train stations are popular spots for branches
located in busy downtown areas.
General steps to reserve these locations:
1. Contact your local government office—mayor, city hall,
town council, or village manager—and explain the event
(charitable collections on public streets are often called
“canning” or “tag day” events).
2. Mention that 100% of the donations collected benefit the
Alzheimer’s Association.
3. Provide the date, time, and number of volunteers you expect.
4. Ask if you need to obtain a charitable solicitation permit. If
so, make a list of the requirements necessary to qualify for
the permit.
5. Click here to download, personalize, and print the City Permit
Template if your town requires a written request for permission.
Certificate of
Liability
If a store requires a copy of the
Bankers Life certificate of liability,
email the following information to
Paul Podgorny at paul.podgorny@
cnoinc.com. Requests take at least
72 business hours.
• Name of store/company
• Complete address
10
Creating an Event Schedule
Tips to keep in mind:
»» Divide your fundraising day(s) into 1- or 2-hour shifts.
»» Ask agents to sign up for one or more shifts.
»» Recruit other volunteers to fill in empty time slots, such as family members, friends, scout troops, etc.
»» Leave a set of supplies at the collection site for volunteers to share.
»» Appoint a coordinator to collect all of the donations at the end of the day and clean up the collection site.

Coordinating Volunteers
Volunteers are key to a successful fundraiser.
Here are some tips on how to build excitement in your branch and
involve the community:
»» Inspire your team. Invite a representative from your local
Alzheimer’s Association chapter to your office to speak about
Alzheimer’s disease and how their organization is the world’s
leading voluntary health organization in Alzheimer’s care,
support, and research.
»» Recruit outside your office. More volunteers mean more
collections. Consider inviting local organizations to help collect
alongside agents and their families.
Examples include:
–– Senior centers
–– Scouts troops
–– High school students—student senate, bands, sports teams, etc.
–– College students—clubs, fraternities, sororities, etc.
–– Civic organizations—Lions Club, Rotary, etc.
»» Recognize volunteers and businesses. Present a certificate
of appreciation to volunteers or businesses that support your
fundraiser. Simply print, fill in name, and distribute the
certificates below.
Click here to download the volunteer certificate sample

Me Not Days. NO EXCEPTIONS! Click here to download the waiver

Collection Total Submission Guidelines
This year, we are simplifying the process of submitting your collections by
having all branches mail their collections to the home office in the form
of the cashier’s check made payable to the Alzheimer’s Association.
All collections will still be given to your local Alzheimer’s Association
chapter. This new process is simply to create uniform procedures for
the submission and allow us to verify and credit you for your total
more efficiently.
Procedures and Submission Rules:
»» All Forget Me Not Days cash and coin collections must be
converted into a Cashier’s Check
»» Make payable to the Alzheimer’s Association
»» Write a memo on the front of the check stating:
–– Your BSO #
–– 2015 Forget Me Not Days
»» Collections may NOT for any reason be deposited in a personal or
business bank account. Converting collections to a Cashier’s Check
does NOT require the funds to be deposited in a bank account at all.
»» If you encounter a Cashier’s Check fee, be sure to mention
it’s for charity and you work for Bankers Life and ask them to
waive the fee. If they are unable to waive the fee at that point,
you may take the amount of the check out of the collections.
»» Send your Cashier’s checks along with any personal checks
collected to home office at the address below no later than
June 15, 2015.
CNO Financial Group
Attn: Erica Smith
11825 N. Pennsylvania Street
Carmel, IN 46032
»» Failure to submit your check by June 15, 2015 will make you
ineligible for the territory contest and any prizes UNLESS you make
other arrangements that have been approved by the home office.
–– If you have a fundraiser that falls after June 15, 2015,
contact Erica Smith at 317.817.2728 or erica.smith@cnoinc.com
immediately to see if other arrangements can be made.
After the Event
Remember—if it doesn’t
go directly to the
Alzheimer’s Association
to fund their programs,
it doesn’t count!
Please Note
Anything that can’t be converted
into a cashier’s check MAY NOT BE
INCLUDED IN COLLECTION TOTAL.
Which include, but are not limited to:
• Value of donated food
• Value of donated merchandise

Dear Community Leader,
An estimated five million people in the U.S. have Alzheimer’s disease*, a type of dementia that impairs memory,
thinking and behavior. Alzheimer’s is not a normal part of aging, and with time, it gets worse.
In fact, Alzheimer’s disease is the sixth-leading cause of death in the United States*—affecting our husbands,
wives, parents, grandparents, neighbors, friends and customers.
That’s why Bankers Life actively supports the Alzheimer’s Association and its mission to eliminate Alzheimer’s
through the advancement of research; to provide and enhance care and support; and to reduce the risk of
dementia through the promotion of brain health.
On June 5 and 6, 2015, Bankers Life will hold our 13th Annual Forget Me Not Days®
fundraiser benefiting
local chapters of the Alzheimer’s Association. Agents and employees in our offices across the nation will join forces
in a 48-hour fight against Alzheimer’s by promoting awareness and collecting donations.
As a community leader, you can help make a difference too! Please consider making a Forget Me Not
Days donation to the Alzheimer’s Association on behalf of your employees using the form below. One hundred
percent of the money collected in goes to the Alzheimer’s Association. The
majority of the funds will stay in the community to benefit the local Alzheimer’s Association chapter.
For more information on how you can get involved in Forget Me Not Days—whether it’s hanging a poster in your
window or putting a collection can at your register—please contact our event coordinator:
On behalf of Bankers Life agents and employees, thank you for helping us plant the seeds of hope for a future
without Alzheimer’s disease.

Guidelines for Non-Canning Fundraisers for
Public Attendees (Golf Outings, Bowling etc.)
Forget Me Not Days helps raise awareness of Alzheimer’s disease and
increases agents’ visibility in the community, which is why “canning”
is the approved format for fundraising. It’s also a simple, low-cost and
effective way to collect donations.
However, in limited circumstances, branches may submit a proposal
for a non-canning fundraiser to be held in addition to the
canning activities.
Important: Only approved public fundraisers can be organized using
the Forget Me Not Days and Alzheimer’s Association names and logos.
In-branch fundraisers, such as office bake sales, Jeans Days, etc., don’t
need home office approval.
Getting a Non-Canning Public Fundraiser Approved
Charity fundraisers outside of the approved Forget Me Not Days
format raise a number of tax, contract, and liability issues that must be
addressed prior to the event in order to protect the branch, the BSM,
and the company.
Here’s the process:
1. Get BSM sign-off on the idea.
2. Receive approval from your local Alzheimer’s Association
chapter contact.
3. Receive home office approval:
–– Discuss event idea with the Forget Me Not Days staff.
Fundraisers must uphold the Bankers Life and Forget Me Not
Days brand.
–– Submit Event Proposal Form by March 30. Click here.
–– Submit Proof of Liability Insurance. If event will be held
at a venue, submit by March 30. The venue’s insurance must
meet CNO’s minimum standards.
–– Reminder: the approval you receive from home office for
your non-canning fundraiser is ONLY an approval of the event
concept as it pertains to fundraising.
4. Follow normal Contract Management procedures.
5. Approval may take up to 14 days. Bankers Life reserves the right to
decline fundraisers that do not meet the above criteria.
Contact
Contact Erica Smith at
erica.smith@cnoinc.com or

Deadlines
Forms must be submitted no

BANKERS LIFE’S FORGET ME NOT DAYS 2015 I 4
PRESIDENT’S MESSAGE
Bankers Life field—
The 13th annual Bankers Life’s Forget Me Not Days
campaign has been another record-breaking year for
us and our support of the Alzheimer’s Association.
With your dedication, you helped raise over $350,000
for Alzheimer’s care and research with signature
Bankers Life community events in more than 200
cities nationwide.
In addition, Bankers Life home office and our parent
company, CNO Financial Group, pledged $150,000
in corporate donations to support the Alzheimer’s
Association’s mission on both the local and national
levels, bringing our total 2015 financial support to
over $500,000!
Since we began Forget Me Not Days in 2003, you have helped raise more than $4.1 million for the
Alzheimer’s Association through a combination of fundraisers and corporate donations. I’m so thankful to all
who have helped over the years. This success would not be possible without the leadership and dedication of
Bankers Life managers and agents.
At Bankers Life, we see first-hand the profound impact of Alzheimer’s disease. Whether through our
customers or in our own families, it is a disease that has personally affected many of us.
Every 67 seconds, a person develops Alzheimer’s disease and there is no cure. The emotional and financial
impact of an Alzheimer’s diagnosis can be devastating for many families, which is why Bankers Life is
committed in supporting the Association’s search for a cure.
Each year when you participate in Forget Me Not Days, you are living our brand promise to serve our
customers and communities while raising awareness of this important cause. On behalf of the Bankers
Life senior team, thank you for your support during this year’s campaign. I’m looking forward to your
participation again in 2016!
Be well,
Scott Goldberg

BANKERS LIFE FIELDHOUSE
Alzheimer’s Awareness Night at
Bankers Life Fieldhouse
Bankers Life’s partnership with the Indiana Pacers has—once again—helped to spread the word about
Alzheimer’s disease not only in Indiana, but to basketball fans across the country.
On April 12, Bankers Life held the fourth annual Alzheimer’s
Awareness Night at Bankers Life Fieldhouse. The first 5,000
fans that entered the Fieldhouse to watch the Indiana Pacers
take on the Oklahoma City Thunder received a packet of
Forget-Me-Not flower seeds to plant in honor of more than
5 million Americans living with Alzheimer’s disease. The
seed packet was attached to an information card featuring
Alzheimer’s statistics and resources.
Highlights of the evening included:
• LED assets and Game Time program ads dedicated to the
Alzheimer’s Association
• In-game promotion of the Alzheimer’s Association
upcoming events, help line, and text to donate
• Interviews Alzheimer’s Association representatives
featured on the Jumbotron during the game
• Table in the main concourse staffed by the Alzheimer’s
Association to provide information about Alzheimer’s
disease and the services they offer
• Purple cotton candy to promote Alzheimer’s Awareness.
Bankers Life donated $1 for everyone sold, and they sold
out in the first half.
If you listen to a Pacers game on the Pacers Radio Network,
you will hear actual Pacers players speaking about
Alzheimer’s disease and the Bankers Life’s Forget Me Not
Days program. This piece is strategically timed to raise
awareness around the event.

New Alzheimer’s
treatment research
revealed
More than 5 million Americans are living with Alzheimer’s
disease, and over 15 million are serving as their caregivers—
a tremendous physical, emotional and financial burden.
Bankers Life is working hard to be a part of the solution
to the Alzheimer’s crisis, by raising awareness and funds
through Bankers Life’s Forget Me Not Days to support
the care, support and research efforts of the Alzheimer’s
Association®
. With the dedication of groups like Bankers
Life, and a concerned Alzheimer’s community, progress is
being made toward methods of treatment, prevention and
ultimately, a cure.
During the Alzheimer’s International Conference®
2015
(AAIC®
), July 18-23 in Washington, D.C., promising
developments for new treatments were revealed by three
different labs. These treatments target misfolded proteins—
the root cause of Alzheimer’s and other diseases such as
Parkinson’s and Lewy Body Dementia.
When proteins misfold, they can start off a chain reaction of
binding to other proteins. This process continues until large
aggregates are formed. Two different misfolded proteins—
amyloid beta and tau—have been shown to be toxic to
brain cells. Their large aggregates—amyloid plaques and tau
tangles—are the hallmark brain lesions of Alzheimer’s disease.
Treatments that target more than one Alzheimer’s-related
protein may be especially useful in managing the disease.
“Alzheimer’s is very complex condition that has been
extremely hard to address with the ‘one target, one
treatment’ approach that’s been successful in other diseases,”
said Maria Carrillo, PhD, Alzheimer’s Association Chief
Science Officer. “Fortunately, we’re beginning to see
some very exciting early results new treatment approach
that targets common components of all the Alzheimer’s
proteins, which also are common to other diseases that
cause dementia. If these results can be shown in people, this
strategy could eventually have benefit not just in Alzheimer’s
but for other neurodegenerative diseases.”
Although the treatments have yet to go through clinical
testing, the results prove unique where past drugs have
failed and have a very promising outlook from the
research community.
Bankers Life is proud to be a part of efforts to support
innovative research through the Alzheimer’s Association. To read
more about the research unveiled at AAIC, visit alz.org/AAIC.

Forget Me Not Days
Territory Contest Debut
Bankers Life’s Forget Me Not Days 2015 launched with a
new kind of contest—the Territory Contest. All four Bankers
Life territories competed to get the highest percentage of
offices in their participating in this year’s event.
To count as toward the percentage of participating branches,
an office had to register and submit a total. It was a very
close race, but the Northeast Territory claimed the top spot
with over 80% of offices participating.
Bruce Jordan, Northeast Territory Vice President, attributes
the success of the territory contest to the branch agents,
managers, and administrators that worked so hard to raise
money for this great cause. “We won the territory contest,
but the real winners are the people that benefit from the
funds we raise that could one day lead to a cure. It makes
me feel good that my team and I are doing something to
help those with this awful disease,” said Bruce.
Bankers Life has been a longtime supporter of the
Alzheimer’s Association and Bankers Life’s Forget Me Not
Days is a part of who we are as an organization. “It’s a way
for us to show our customers we aren’t just interested in
making money, but that we care about the market we serve,”
adds Bruce.

to $500,000
Annual Event Raises Over $350,000 to Support Care, Education and Research
CHICAGO, Sept. 1, 2015 /PRNewswire/ -- After a successful Bankers Life's Forget Me Not
Days fundraiser held throughout the country, support for the Alzheimer's Association from
Bankers Life and its parent company CNO Financial Group has grown to $500,000 for 2015.
Bankers Life is a national life and health insurance brand that focuses on the insurance needs
of Americans who are near or in retirement.
Forget Me Not Days is Bankers Life's annual street corner fundraising campaign to raise
awareness of and collect money for the Alzheimer's Association. Nationally, this year's Bankers
Life's Forget Me Not Days event collected just over $350,000, a four percent increase over last
year. In addition, Bankers Life and CNO Financial Group pledged $150,000 in corporate
donations to support the Alzheimer's Association's mission.
Bankers Life's Forget Me Not Days represents our commitment to supporting the Alzheimer's
Association and its efforts to end this devastating disease, said Scott Goldberg, president of
Bankers Life. We look forward to the event each year, hitting the streets to collect money and
educate our community about Alzheimer's and its impact on the growing Baby Boomer
population.
The event is called Forget Me Not Days because in exchange for their donations, individuals
receive packets of Forget-Me-Not flower seeds to take home and plant in honor of families
across America who are facing or have lost someone with Alzheimer's.
In its 13th
record-breaking year, more than 1,000 Bankers Life agents, associates and
Alzheimer's Association volunteers took to the streets in June to collect donations from
passersby. Volunteers collected in more than 200 cities across the country and over 200,000
packets of Forget-Me-Not flower seeds were exchanged for donations.
With more than 5 million Americans living with Alzheimer's and 15 million people providing
care, support for the cause is more critical than ever. The ongoing dedication of Bankers Life
and their employees to the Alzheimer's Association is always significant and greatly
appreciated, said Beth Kallmyer, vice president, constituent services, Alzheimer's Association.
The commitment made by Bankers Life volunteers in communities to raise awareness and
collect funds for critical Alzheimer's Association care, support and research programs is
meaningful and will help ensure that families have the support they need, while also advancing
research to change the course of this devastating disease.
One hundred percent of the money collected during Forget Me Not Days is donated to the
Alzheimer's Association, the largest nonprofit funder for Alzheimer's research in the nation.
Since 2003, Bankers Life's Forget Me Not Days has helped provide more than $4.1 million to
the Alzheimer's Association.

In exchange for a contribution, donors receive packets of Forget-Me- Not flower seeds to be planted in honor of the more than five million people with Alzheimer’s. • 12. 7 Register Your Office Visit http://bnk.rs/16w7HoO to register your office for Forget Me Not Days. Select a Forget Me Not Days Coordinator Choose a coordinator from your office to be in charge of planning the event. It’s also recommended that you appoint a committee to help the coordinator in the planning efforts. If you are coordinating this year’s event, consider the tips below: »» Book collection locations early »» Call your Alzheimer’s Association within a week of registering. Click here to see which chapter you are matched with. »» Do a little planning each week leading up to the event. »» Recruit as many volunteers as possible. »» Keep your event manageable. It’s best to stick to collecting in a couple locations—complicated “extra events” can be time consuming and expensive. »» Create an event schedule for the day of the event (see page 10 for example). »» Don’t try to tackle too much alone—delegate some tasks to others. On-Demand Videos This year, on-demand training videos will be released leading up to Forget Me Not Days 2015. Stay tuned to the WIRe, Perspectives, https://bspn.bankers.com/ forget-me-not-days, or the newsletters coordinators will start to receive upon registration to view these videos as they become available. • 13. 8 Working with the Alzheimer’s Association The Alzheimer’s Association has a network of over 80 chapters nationwide. Click here to find out which chapter your office is matched with. Step 1: Call your local chapter as soon as possible. Step 2: Share your event plan: »» Collection Locations »» Dates and Times »» Expected number of volunteers Step 3: Ask for the chapter’s feedback on your plan and, if possible, if they can help recruit volunteers, speak to the branch, provide some supplies—bracelets, pamphlets, etc, and/or attend the event. Some large chapters may be able to offer their assistance promoting the event or recruiting volunteers, but please keep in mind that many chapters are small and only staffed by one or two part-time volunteers. Chapters are extremely thankful for the funds you raise to support their programs, but may not be as accessible as you would like. It’s each branch’s responsibility to take ownership of its event and oversee coordination and logistics. Chosing a Collection Location Picking a good location with heavy foot traffic is the best way to increase your collection total. Consider these suggestions: »» Grocery/Department Stores Setting up a table outside a busy a store guarantees a steady stream of people who have their wallets on them! General steps to reserve these locations: 1. Call the store manager to ask for permission to set up a charity collection table in front of their store. 2. Explain that 100% of the donations collected benefit the Alzheimer’s Association. 3. Mention the date, time, and number of volunteers you expect. 4. If the store asks you to submit a request in writing, click here to download, personalize, and print the Retail Permit Template. 5. Some stores may request a copy of the Alzheimer’s Association chapter’s 501(c) (3) letter proving that the chapter is a nonprofit organization. You can request a copy from your local chapter. 6. A few weeks prior to your event, call to reconfirm your date and time with the store. Event Tip Some stores will match your collection amount at their location with a “matching grant” contribution of their own—instantly doubling your collections! Contacts If you have trouble contacting your chapter, please contact Erica Smith at erica.smith@cnoinc.com for assistance. To learn more about the Alzheimer’s Association, visit alz.org®.
  • 14. 9 »» Downtown Areas Street corners or train stations are popular spots for branches located in busy downtown areas. General steps to reserve these locations: 1. Contact your local government office—mayor, city hall, town council, or village manager—and explain the event (charitable collections on public streets are often called “canning” or “tag day” events). 2. Mention that 100% of the donations collected benefit the Alzheimer’s Association. 3. Provide the date, time, and number of volunteers you expect. 4. Ask if you need to obtain a charitable solicitation permit. If so, make a list of the requirements necessary to qualify for the permit. 5. Click here to download, personalize, and print the City Permit Template if your town requires a written request for permission. Certificate of Liability If a store requires a copy of the Bankers Life certificate of liability, email the following information to Paul Podgorny at paul.podgorny@ cnoinc.com. Requests take at least 72 business hours. • Name of store/company • Complete address
  • 15. 10 Creating an Event Schedule Tips to keep in mind: »» Divide your fundraising day(s) into 1- or 2-hour shifts. • 16. 11 Coordinating Volunteers Volunteers are key to a successful fundraiser. Here are some tips on how to build excitement in your branch and involve the community: »» Inspire your team. Invite a representative from your local Alzheimer’s Association chapter to your office to speak about Alzheimer’s disease and how their organization is the world’s leading voluntary health organization in Alzheimer’s care, support, and research. »» Recruit outside your office. More volunteers mean more collections. Consider inviting local organizations to help collect alongside agents and their families. Examples include: –– Senior centers –– Scouts troops –– High school students—student senate, bands, sports teams, etc. –– College students—clubs, fraternities, sororities, etc. –– Civic organizations—Lions Club, Rotary, etc. »» Recognize volunteers and businesses. Present a certificate of appreciation to volunteers or businesses that support your fundraiser. Simply print, fill in name, and distribute the certificates below. • 18. 13 Collection Total Submission Guidelines This year, we are simplifying the process of submitting your collections by having all branches mail their collections to the home office in the form of the cashier’s check made payable to the Alzheimer’s Association. All collections will still be given to your local Alzheimer’s Association chapter. This new process is simply to create uniform procedures for the submission and allow us to verify and credit you for your total more efficiently. Procedures and Submission Rules: »» All Forget Me Not Days cash and coin collections must be converted into a Cashier’s Check »» Make payable to the Alzheimer’s Association »» Write a memo on the front of the check stating: –– Your BSO # –– 2015 Forget Me Not Days »» Collections may NOT for any reason be deposited in a personal or business bank account. Converting collections to a Cashier’s Check does NOT require the funds to be deposited in a bank account at all. »» If you encounter a Cashier’s Check fee, be sure to mention it’s for charity and you work for Bankers Life and ask them to waive the fee. If they are unable to waive the fee at that point, you may take the amount of the check out of the collections. »» Send your Cashier’s checks along with any personal checks collected to home office at the address below no later than June 15, 2015. CNO Financial Group Attn: Erica Smith 11825 N. Pennsylvania Street Carmel, IN 46032 »» Failure to submit your check by June 15, 2015 will make you ineligible for the territory contest and any prizes UNLESS you make other arrangements that have been approved by the home office. –– If you have a fundraiser that falls after June 15, 2015, contact Erica Smith at 317.817.2728 or erica.smith@cnoinc.com immediately to see if other arrangements can be made. After the Event Remember—if it doesn’t go directly to the Alzheimer’s Association to fund their programs, it doesn’t count! Please Note Anything that can’t be converted into a cashier’s check MAY NOT BE INCLUDED IN COLLECTION TOTAL. Which include, but are not limited to: • Value of donated food • Value of donated merchandise (TVs, trips, gift cards, services, etc.) • Cash or check donations that have been promised, but not received • Grants that have been promised, but not received

  • 22. 17 Guidelines for Non-Canning Fundraisers for Public Attendees (Golf Outings, Bowling etc.) Forget Me Not Days helps raise awareness of Alzheimer’s disease and increases agents’ visibility in the community, which is why “canning” is the approved format for fundraising. It’s also a simple, low-cost and effective way to collect donations. However, in limited circumstances, branches may submit a proposal for a non-canning fundraiser to be held in addition to the canning activities. Important: Only approved public fundraisers can be organized using the Forget Me Not Days and Alzheimer’s Association names and logos. In-branch fundraisers, such as office bake sales, Jeans Days, etc., don’t need home office approval. Getting a Non-Canning Public Fundraiser Approved Charity fundraisers outside of the approved Forget Me Not Days format raise a number of tax, contract, and liability issues that must be addressed prior to the event in order to protect the branch, the BSM, and the company. Here’s the process: 1. Get BSM sign-off on the idea. 2. Receive approval from your local Alzheimer’s Association chapter contact. 3. Receive home office approval: –– Discuss event idea with the Forget Me Not Days staff. Fundraisers must uphold the Bankers Life and Forget Me Not Days brand. –– Submit Event Proposal Form by March 30. Click here. –– Submit Proof of Liability Insurance. If event will be held at a venue, submit by March 30. The venue’s insurance must meet CNO’s minimum standards. –– Reminder: the approval you receive from home office for your non-canning fundraiser is ONLY an approval of the event concept as it pertains to fundraising. 4. Follow normal Contract Management procedures. 5. Approval may take up to 14 days. Bankers Life reserves the right to decline fundraisers that do not meet the above criteria. Contact Contact Erica Smith at erica.smith@cnoinc.com or 317.817.2728 to begin the process and receive the forms/liability insurance standards. Deadlines Forms must be submitted no later than March 30, 2015. All non-canning fundraisers must be reviewed on an annual basis. ©2015 Bankers Life (02/15) 160038 151413

  • 29. BANKERS LIFE’S FORGET ME NOT DAYS 2015 I 4 PRESIDENT’S MESSAGE Bankers Life field— The 13th annual Bankers Life’s Forget Me Not Days campaign has been another record-breaking year for us and our support of the Alzheimer’s Association. With your dedication, you helped raise over $350,000 for Alzheimer’s care and research with signature Bankers Life community events in more than 200 cities nationwide. In addition, Bankers Life home office and our parent company, CNO Financial Group, pledged $150,000 in corporate donations to support the Alzheimer’s Association’s mission on both the local and national levels, bringing our total 2015 financial support to over $500,000! Since we began Forget Me Not Days in 2003, you have helped raise more than $4.1 million for the Alzheimer’s Association through a combination of fundraisers and corporate donations. I’m so thankful to all who have helped over the years. This success would not be possible without the leadership and dedication of Bankers Life managers and agents. At Bankers Life, we see first-hand the profound impact of Alzheimer’s disease. Whether through our customers or in our own families, it is a disease that has personally affected many of us. Every 67 seconds, a person develops Alzheimer’s disease and there is no cure. The emotional and financial impact of an Alzheimer’s diagnosis can be devastating for many families, which is why Bankers Life is committed in supporting the Association’s search for a cure. On behalf of the Bankers Life senior team, thank you for your support during this year’s campaign. I’m looking forward to your participation again in 2016! Be well, Scott Goldberg President, Bankers Life
  • 30. BANKERS LIFE’S FORGET ME NOT DAYS 2015 I 5 BANKERS LIFE FIELDHOUSE Alzheimer’s Awareness Night at Bankers Life Fieldhouse Bankers Life’s partnership with the Indiana Pacers has—once again—helped to spread the word about Alzheimer’s disease not only in Indiana, but to basketball fans across the country. On April 12, Bankers Life held the fourth annual Alzheimer’s Awareness Night at Bankers Life Fieldhouse. The first 5,000 fans that entered the Fieldhouse to watch the Indiana Pacers take on the Oklahoma City Thunder received a packet of Forget-Me-Not flower seeds to plant in honor of more than 5 million Americans living with Alzheimer’s disease. The seed packet was attached to an information card featuring Alzheimer’s statistics and resources. Highlights of the evening included: • LED assets and Game Time program ads dedicated to the Alzheimer’s Association • In-game promotion of the Alzheimer’s Association upcoming events, help line, and text to donate • Interviews Alzheimer’s Association representatives featured on the Jumbotron during the game • Table in the main concourse staffed by the Alzheimer’s Association to provide information about Alzheimer’s disease and the services they offer • Purple cotton candy to promote Alzheimer’s Awareness. Bankers Life donated $1 for everyone sold, and they sold out in the first half. If you listen to a Pacers game on the Pacers Radio Network, you will hear actual Pacers players speaking about Alzheimer’s disease and the Bankers Life’s Forget Me Not Days program. This piece is strategically timed to raise awareness around the event.

  • 32. BANKERS LIFE’S FORGET ME NOT DAYS 2015 I 7 New Alzheimer’s treatment research revealed More than 5 million Americans are living with Alzheimer’s disease, and over 15 million are serving as their caregivers— a tremendous physical, emotional and financial burden. Bankers Life is working hard to be a part of the solution to the Alzheimer’s crisis, by raising awareness and funds through Bankers Life’s Forget Me Not Days to support the care, support and research efforts of the Alzheimer’s Association® . With the dedication of groups like Bankers Life, and a concerned Alzheimer’s community, progress is being made toward methods of treatment, prevention and ultimately, a cure. During the Alzheimer’s International Conference® 2015 (AAIC® ), July 18-23 in Washington, D.C., promising developments for new treatments were revealed by three different labs. These treatments target misfolded proteins— the root cause of Alzheimer’s and other diseases such as Parkinson’s and Lewy Body Dementia. When proteins misfold, they can start off a chain reaction of binding to other proteins. This process continues until large aggregates are formed. Two different misfolded proteins— amyloid beta and tau—have been shown to be toxic to brain cells. Their large aggregates—amyloid plaques and tau tangles—are the hallmark brain lesions of Alzheimer’s disease. Treatments that target more than one Alzheimer’s-related protein may be especially useful in managing the disease. “Alzheimer’s is very complex condition that has been extremely hard to address with the ‘one target, one treatment’ approach that’s been successful in other diseases,” said Maria Carrillo, PhD, Alzheimer’s Association Chief Science Officer. “Fortunately, we’re beginning to see some very exciting early results new treatment approach that targets common components of all the Alzheimer’s proteins, which also are common to other diseases that cause dementia. If these results can be shown in people, this strategy could eventually have benefit not just in Alzheimer’s but for other neurodegenerative diseases.” Although the treatments have yet to go through clinical testing, the results prove unique where past drugs have failed and have a very promising outlook from the research community. Bankers Life is proud to be a part of efforts to support innovative research through the Alzheimer’s Association. To read more about the research unveiled at AAIC, visit alz.org/AAIC. Retrieved from http://alz.org/aaic/releases_2015/ Sun-930amET.asp Forget Me Not Days Territory Contest Debut Bankers Life’s Forget Me Not Days 2015 launched with a new kind of contest—the Territory Contest. All four Bankers Life territories competed to get the highest percentage of offices in their participating in this year’s event. To count as toward the percentage of participating branches, an office had to register and submit a total. It was a very close race, but the Northeast Territory claimed the top spot with over 80% of offices participating. Bruce Jordan, Northeast Territory Vice President, attributes the success of the territory contest to the branch agents, managers, and administrators that worked so hard to raise money for this great cause. “We won the territory contest, but the real winners are the people that benefit from the funds we raise that could one day lead to a cure. It makes me feel good that my team and I are doing something to help those with this awful disease,” said Bruce. Bankers Life has been a longtime supporter of the Alzheimer’s Association and Bankers Life’s Forget Me Not Days is a part of who we are as an organization. “It’s a way for us to show our customers we aren’t just interested in making money, but that we care about the market we serve,” adds Bruce.
