Crystal deposition
Calcium pyrophosphate dihydrate disease (CPPD disease), also
referred as pyrophosphate arthropathy and perhaps confusingly
as pseudogout, is common, especially in the elderly, and is
characterised by the deposition of calcium pyrophosphate in soft
tissues and cartilage.
CPPD is one of many causes of soft tissue calcification
(chondrocalcinosis). It is not synonymous with chondrocalcinosis and
not the only cause of soft tissue calcification.
Where crystal deposition causes acute clinical manifestation, the
term pseudogout should be used. Pyrophosphate arthropathy is a term
that describes arthropathy secondary to CPPD deposition. However, it
is often used indiscriminately to refer to chondrocalcinosis too.
Scapholunate advanced collapse (SLAC) refers to a pattern of wrist malalignment
that has been attributed to post-traumatic or spontaneous osteoarthritis of the wrist.

7. Spinal infections epidemiology
• spinal infections are considered uncommon, with an incidence of 1 case per
100,000-250,000 population per year. However, some reviews suggest that the
incidence of spinal infections is now increasing. This increase may be secondary
to increased use of vascular devices and other forms of instrumentation and to
increasing rates of IV drug abuse.
• Because of its rarity and vague initial signs and symptoms, diagnosis is often
delayed.
• Discitis and osteomyelitis peak in pediatric patients; the incidence of spinal
infections then decreases until middle age, when a second peak in incidence is
observed at approximately age 50 years.
• However, in less developed nations, infectious osteomyelitis is more common.

8. Pott’s disease
• Synonyms: Pott's syndrome, Pott's caries, Pott's curvature, angular kyphosis, kyphosis secondary
to tuberculosis, tuberculosis of the spine, tuberculous spondylitis and David's disease
• Pott's disease is named after Percival Pott (1714-1788) who was a surgeon in London. Pott's
disease is tuberculosis of the spinal column The usual sites to be involved are the lower thoracic
and upper lumbar vertebrae.
• The source of infection is usually outside the spine. It is most often spread from the lungs via the
blood.
• There is a combination of osteomyelitis and infective arthritis.
• Usually more than one vertebra is involved. The area most affected is the anterior part of the
vertebral body adjacent to the subchondral plate. Tuberculosis may spread from that area to
adjacent intervertebral discs.
• Caseation occurs, with vertebral narrowing and eventually vertebral collapse and spinal damage. A
dry soft tissue mass often forms and superinfection is rare.

Typical MR Sequence: Bone marrow edema, changes begin at endplate, extend into vertebral body;

Diagnosis: MR is most sensitive and specific
bone scan and labeled white cell scan: ↓ sensitivity compared to detection at other sites in body
Percutaneous biopsy: Pathogen identified in < 50%

*Left untreated, vertebral osteomyelitis can lead to permanent neurologic deficits, significant spinal deformity, or death. It can result
in severe compression of the neural structures due to formation of an epidural absces or due to a pathologic fracture resulting from
bone softening.
16. Pathway of spread
• Although the arterial route is the usual route of bacterial spread to a
vertebra, another proposed route of infection is the retrograde
seeding of venous blood via the Batson plexus. During periods of
increased intra-abdominal pressure, venous blood is shunted toward
the vertebral venous plexus. Some authors have proposed that the
venous system may be the route of bacterial spread from
genitourinary tract infections.
• Another possible means of infection is by the spread of contiguous
infection into the vertebrae and disk (eg, from a reytropharyngeal
abscess or a retroperitoneal abscess, facetarthritis, resulting in
osteomyelitis and diskitis.
17. Pathophysiology
• Approximately 95% of pyogenic spinal infections involve the vertebral body, and only 5% involve the posterior elements of
the spine. This disparity has been attributed in part to the voluminous blood supply to the vertebral body and its rich,
cellular marrow.
• Bacteria circulating through the blood may enter a vertebra or a disk space via its arterial blood supply or via the venous
system. In the typical case, bacteria enter the vertebral body through small metaphyseal arteries arising from larger
primary periosteal arteries that, in turn, branch from the spinal arteries. In adults, blockage of metaphyseal arteries by
septic thrombi may infarct relatively large amounts of bone. Subsequently, bacteria can readily colonize a large bony
sequestrum adjacent to the disk.
18. Difference between pathways of spread in children compared to adults
In children , vascular channels are present
across the growth plate , allowing primary
infection of the disc with subsequent secondary
infection of the vertebral body.
In the adult, after bacterial colonization of the
metaphyseal region, the avascular disk is secondarily
invaded by bacteria from the endplate region and
subsequently in the adjacent vertebral body.
Intermetaphyseal communicating arteries allow the
spread of septic thrombi from one metaphysis to the
other in a single vertebral body without involvement
of the midportion of the vertebra.
19. Etiology
Presumably, a distant focus of infection provides an infective nidus from which bacteria
spread by the bloodstream to the spinal column. The skin and the genitourinary tract are
common antecedent sites, but a review of the literature reveals multiple foci, such as septic
arthritis , sinusitis, subacute bacterial endocarditis, and respiratory, oral, or gastrointestinal
infection.
Approximately 30-70% of patients with vertebral osteomyelitis have no obvious prior
infection.
Risk factors for developing osteomyelitis include conditions that compromise the immune
system, such as the following:
Advanced age
IV drug abuse
Congenital immunodepression
Organ transplantation
Malnutrition
Cancer

25. Complicaties post chirurgie
• Etiology
• In a retrospective cohort study, Gupta et al assessed 260 patients with pyogenic vertebral
osteomyelitis, of whom 27% acquired the infection after an invasive spinal procedure, 40% had S
aureus as the cause of the infection, and 49% underwent spinal surgery as part of initial therapy.
• On multivariate analysis, the factors associated with greater likelihood of treatment failure were
(1) a longer duration of symptoms before diagnosis and (2) an infection caused by S aureus.

• Anamnese
• Sinds woensdagavond hevige pijn ter hoogte van de lagerug. Pijn is plots opgetreden, geen uitlokkende beweging.
Donderdag kwam de huisarts langs en heeft de diagnose acute lumbago gesteld. Patiënt komt naar de spoed
omdat de pijn ondraagbaar is. Dinsdag is hij met otitis media acuta rechts gediagnosticeerd nadat deze last
maandag begon (hiervoor oordruppels gekregen). Naast de rugpijn heeft hij ook algemene spier en gewrichtspijn
sinds donderdag. Koude rillingen in gisteren. Geen braken, geen diarree, geen mictie-problemen.

georganiseerd. Verwijdering in toto. Uitgebreide staalnames voor bacterio. Meticuleuze hemostase.

45. Controle consultatie
Deze 65-jarige man werd vandaag teruggezien op raadpleging. Meneer is gekend nadat hij 9 maand geleden werd
opgenomen in kader van een progressieve tetraparese op basis van een epiduraal abces hoogcervicaal waarvoor
brede laminectomie. Meneer revalideerde lange tijd op onze Sp dienst, en volgt ondertussen nog steeds driemaal per
week ambulante kine en ergo.
Meneer stapt ondertussen zelfstandig, meestal met een kruk aan 1 zijde, maar soms ook al zonder. Binnen in huis
heeft hij geen loophulpmiddel meer nodig.
De handen evolueren ook, maar wat trager. Heel fijne zaken opnemen gaat nog moeilijk, ritssluitingen beginnen te
lukken, knopen wisselend. Het geschrift begint wat leesbaarder te worden. Er zijn nog storende
sensibiliteitsstoornissen in de vingers en de handen. Er zijn milde temperatuursensatiestoornissen.
De nekpijnklachten zijn vrij behoorlijk onder controle met Durogesic
