Caso Clínico: TAC DE CRANEO SIMPLE Y CONTRASTADA: 7-Nov-03 Se observa evento vascular de apariencia isquémico en proyección tala- mocapsular derecha, que mide aproximadamente 41x39 mm con edema perilesional, que hace efecto de masa sobre el ventrículo lateral derecho y desplazando al tercero . El resto del parenquima es normal. Posterior a la aplicación de material de contraste, se define mejor el evento men- cionado. Desviación de la línea media a la izquierda.

10. Caso Clínico: EVOLUCION: - Durante su hospitalización no presenta fiebre. - Hemodinámica y metabólicamente estable. - Neurológicamente con mejoría de su estado de concien- cia. Sin cambio en relación a parálisis del III par de- recho y hemiplejía izquierda. - Se confirma Western-Blot positivo. - CD4: 120 células.

SIDA Y MANIFESTACIONES NEUROLOGICAS - La incidencia de Manifestaciones Neurológicas fluctúa entre 31 y 39%. - En series de autopsias de pacientes con SIDA, hasta en 80% hay evidencia de manifestaciones neurológicas. - Aproximadamente entre 10 y 20% de los pacientes tienen alteraciones neurológicas como manifestación inicial.

32. AIDS dementia complex. Axial T2WI in a 37-year-old HIV-positive man demonstrates marked atrophy (enlarged ventricles and sulci) as well as diffuse white matter hyperintensities (arrows) i.e., "dirty white matter," due to HIV encephalitis.
33. HIV encephalitis. This 39-year-old HIV-positive man has focal white matter lesions (arrows) that did not enhance following administration of gadolinium (not shown). This is typical of HIV encephalitis.
34. Toxoplasmosis. (A) An AIDS patient with recent onset of seizures demonstrates ring-enhancing lesion typical of toxoplasmosis (arrow) on this axial T1WI. Apparent dural enhancement (arrowhead) represents normal cortical vein. (B) Axial T2WI demonstrates a low-intensity ring (arrow) in same location as the enhancement seen in Figure 4A. The low intensity is due to T2 shortening from free radicals in the wall of the abscess.
35. Proton spectroscopic differentiation of toxoplasmosis and lymphoma in an AIDS patient. (A) Axial T2WI demonstrates voxel placement for MR spectroscopy (MRS) in this 37-year-old HIV-positive patient with new onset seizures. (B) MRS from voxel in 5A demonstrates elevated lipid/lactate at 1.0 to 1.6 ppm (arrow) but normal choline at 3.2 ppm (arrowhead). (Normal choline essentially excludes a tumor such as lymphoma.) (C) Axial T2WI demonstrates MRS voxel placement in a patient with a low-intensity mass (box). (D) MRS from the voxel in 5C demonstrates elevated choline at 3.2 ppm (arrowhead) with elevated lipid/lactate (arrow). The elevated choline is indicative of a neoplasm, such as lymphoma.
36. Primary CNS lymphoma. (A) Gadolinium-enhanced axial image demonstrates peripherally-enhancing central mass (arrow) in this 42-year-old AIDS patient. (B) Axial T2WI demonstrates low signal intensity in mass due to high nuclear:cytoplasmic ratio typical of lymphoma. Diffusion image (not shown) was slightly hyperintense.
37. Cryptococcus. (A) Gadolinium-enhanced coronal image demonstrates nonenhancing cystic masses (arrows) in the basal ganglia in this 40-year-old HIV-positive patient. These enlarged perivascular spaces surrounding the lenticulostriate arteries are typical of cryptococcal pseudocysts. (B) Gadolinium-enhanced axial image demonstrates nonenhancing ganglionic masses (large arrow) as well as ring-enhancing lesion of toxoplasmosis in the left occipital lobe (small arrow). Coexisiting disease states are typical of AIDS. (C) Axial T2WI through the basal ganglia demonstrates apparent enlargement of the perivascular spaces (arrows) due to cryptococcal pseudocysts as well as low intensity border of toxoplasmosis abscess in left occipital lobe.

39. PML. A proton-density weighted axial image demonstrates hyperintensity in the high parietal lobe in this patient with AIDS (arrow). This is a typical location for PML, however, gadolinium must be given to differentiate this lesion from toxoplasmosis or lymphoma, which both enhance. (B) This gadolinium-enhanced axial image does not demonstrate enhancement (arrow). This combination of findings is typical of PML, however, the only definitive means of diagnosis is biopsy.

There was no evidence of enhancement following administration of gadolinium (not shown). This is a typical location and typical (lack of) enhancement pattern of PML.
41. INSTITUTO DE NEUROLOGIA . Se estudiaron 149 pacientes con SIDA entre 1990 y 1998. . 133 de sexo masculino (89%) y 16 mujeres (10.7%) . Edad promedio de 33.8 años (9 a 75). . En 75 pacientes (50.3%) la enfermedad neurológica fue una condición definitoria de SIDA y su primera manifestación clínica. .
