8. BrightStar Clinical Pathways is home based and provides a combination
of non-medical and skilled medical care. This care follows a designated
Plan of Care created by an RN and we provide the resources the senior
requires to be at home, safe stable or improving in condition.
9. Clinical Pathways is also a time-limited program. This does not mean
that the senior will not need home care for a lengthy period but there is
a window after discharge where the risk of poor outcomes, including of
readmission is highest. The goal of a transitional care program is to
provide both care and education during this window to help ensure that
the patient is safe and continues on the road to recovery.
10. Clinical Pathways is evidence-based. It is grounded in research that has
demonstrated effectiveness. The research has been done in the US and
around the world. Most seniors want to age age at home and this
program will maximize their chances of doing this successfully.
11. BrightStar Clinical Pathways is a condition-specific care program. The
care and education the patient, family and caregiver need are very
specific to the condition and current health of the patient.
12. BrightStar Clinical Pathways is an in-home program, there is no travel
outside the home required for family or the client. As a matter of fact,
we can reduce required travel by doing shopping and picking up
prescriptions. We can also provide transportation to Doctor’s and other
appointments, including hair and nail appointments. This can greatly
reduce the stress on the senior living at home.
13. Finally, Clinical Pathways is RN
managed. The Registered Nurse
manages all care and works with the
rest of the patient’s medical team,
including the primary care physician
and specialists to promote the good
communication necessary for quality
care and positive outcomes.
