1. Description of Services
Collection and Verification of Certificate of Insurance
Assurance Screening handles everything from document retrieval, storage and management of certificates of insurance,
including liability and worker’s compensation coverage. We verify if your business partners have the correct coverage amounts
that your business requires. Through our producing agency verification, you are notified if we find fraudulent documents and if
anything is changed, altered, or cancelled with your business partners policies.
Civil Financial Search/Bankruptcy, Lien, & Judgment Check
Assurance Screening helps determine the financial stability of your suppliers, vendors, and contractors by screening public
records for bankruptcy filings, tax liens, and judgments. This screening can be implemented and monitored at any interval
chosen by the client whether daily, weekly, etc.
OSHA Citation & Violation Search
Assurance Screening screens to identify any citations and/or violations listed with the United States Department of Labor
Occupational Safety and Health Administration (OSHA). Such can result from planned or unplanned company inspections or
from referral. Ensuring that the companies you do business with are safe and healthy places to work is essential in positive
company standing and public image.
EIN & W9 Collection & Records Search
Assurance Screening checks the validity of your business partners’ tax identification numbers. Assurance Screening then flags
the number if it matches another business’ EIN.
EMR Collection & Verification
Experience Modification Rate (EMR) has strong impact upon a business. It is a number used by insurance companies to gauge
both past cost of injuries and future chances of risk. Assurance Screening collects documentation from the subcontractor/trade
partner and then verifies the three year EMR histories with the workers’ compensation insurance company issuing the rating, or
through the producing agency of the workers’ compensation policy.
The Experian™ Business Profile Report
This report provides the information needed to make informed credit-granting decisions quickly and easily. Reports include
current payment information, payment trending, public record information including bankruptcy, lien and judgments, company
background information and financial information.
Professional License & Educational Verifications
Assurance Screening verifies that your business partners have up to date operating licenses for their field of expertise.
Assurance Screening can also verify that your business partners have the educational degrees stated in their hiring process.
Corporate Status Verification
Assurance Screening verifies the company’s good standing status with the state as an active corporation. We conduct state
business entity verifications.
Professional & Project Reference Checks
Assurance Screening provides a third-party solution for professional reference collection and past project reference checks.
Assurance Screening contacts up to three references by phone, and documents back the information received from the Client
approved screening questions. Five(5) call attempts are made. Up to ten (10) questions are asked. The references must be
provided directly to Assurance Screening by the vendor/subcontractor being screened.

2. Description of Services
Supplier Payment Checks
If applicable, Assurance Screening contacts up to three suppliers listed by fax, and documents back the information received
from the Client approved screening questions. Five (5) fax attempts are made. Up to four (4) questions are asked. NOTE: The
suppliers must be provided directly to Assurance Screening by the vendor/subcontractor being screened.
Additional Document Collection & Verification Services
With customizable document collection and management services, Assurance Screening can assist your organization in the
collection and verification of most documents you require from your subcontractor, vendors and suppliers. The Assurance
Screening team will request the documentation, and communicate with your business partners on what your company requires.

Program Implementation Schedule
Week 1
 Client composes a subcontractor/trade partner and location list with contact information and provides to Assurance
Screening. If applicable, each subcontractor/trade partner will be associated with the correlating location(s) or project(s).
 Current insurance requirements are provided by Client for review, if applicable.
 The Assurance Team provides to Client a tailored internal company memo regarding the program to be distributed to
Client staff.
 The Assurance Team provides to the Client a copy of the enrollment email for customization and approval.
 The Assurance Team finalizes the subcontractor/vendor list and returns to Client for final approval.
Week 2
 The Assurance Team sends the enrollment email to the subcontractors/trade partners introducing Assurance Screening,
and requests completion of the process. The screening process and the necessity to complete the registration in a timely
manner are explained in this communication piece.
 The Assurance Team continues to fill in any contact holes to those subcontractors/trade partners that have missing
information and send out enrollment emails as information is gathered.
Week 2+
 Subcontractors/Trade Partners click on the link in the email which takes them securely to our website where they provide
all requested information. Each are given a specified date to complete the screening process, determined by the Client.
 Upon enrollment, The Assurance Team screens the enrollees against the predetermined screening parameters and
verifies documents received by the subcontractors/trade partners. Each subcontractor/trade partner has the ability to
request a copy of their completed screening report.
Week 3
 An online training session is scheduled with Client. This instructional training will demonstrate to the Client users how to
navigate the Assurance website and view their reports. Client may log in at any time to check the status of any
subcontractor/trade partner.
Ongoing Processes
Reminder emails are sent automatically to subcontractors/trade partners every three (3) days requesting enrollment and completion of the
screening process.
Upon enrollment, reminder requests for documentation are initiated two times (2x) per week, for thirty (30) days. This is initiated in the form
of calls and emails to the subcontractor/trade partner point of contact until required documents are received.
