• 15,906 square kilometres

• Hull, Leeds, Sheffield and York

• Rural areas
Airedale NHS
     Foundation Trust

• Positioned in the north west of the
  Yorkshire and Humber region
• Population of 220,000
• Small towns – Keighley, Ilkley, Skipton,
  Colne
• Serves patients in the Craven and
  Wharfedale areas of the Yorkshire Dales
Long Term Conditions
 Almost one in three people are living with a long term
    condition
   That 30%+ of the population consume around 70% of the
    NHS budget
     Over 50% of GP appointments
     Almost 65% of outpatient appointments
     Almost 75% of bed days
   Three out of every five people aged over 60 are likely to
    suffer from a long term condition
Population Trends
 The region’s population as a whole is projected to increase by
  455,000 between 2011 and 2031
 The region’s population will become more ethnically diverse, the
  projections suggest that by 2031, 13% of the population with be
  from ethnic minorities as opposed to the 10% seen in 2011
 The population will also age over the period, moving from 29 people
  of retirement age for every 100 of working age in 2011 to 43 people
  of retirement age for every 100 of working age by 2031
 This rise in the elderly population equates to a population aged over
  65 in the region of 980,000 in 2011 rising to 1,440,000 by 2031
Modelling suggests that between
2010 and 2020
 CHD will rise from 6.0% to 6.6% of the population, equivalent to
  49,000 people
 COPD will rise from 3.7% to 3.9% of the population, equivalent to
  24,000 people
 Hypertension will rise from 30.8% to 32.3% of the population,
  equivalent to 186,000 people
 Stroke will rise from 2.7% to 2.8% of the population, equivalent to
  21,000 people
 CVD will rise from 6.9% to 7.6% of the population, equivalent to
  59,000 people
 Diabetes will rise from 7.5% to 8.5% of the population, equivalent to
  77,000 people
These factors represent a huge
quality and efficiency challenge for our
        region’s health services
Telehealth in Y&H in 2010
 Some health communities in Y&H were embarking on
  significant developments:
    Airedale - trailblazers in the use of telemedicine
    Barnsley - blended service model
    Hull - success with HF
    North Yorkshire and York - aiming to reach 2000
 Most other health communities were running small pilots
  involving very low patient numbers
A Strategy for the Region
 Chief Executive of NHS Yorkshire and the Humber, the
  Strategic Health Authority keen to progress Telehealth
  in the region
 Chief Execs of the region’s Trusts agreed an approach
 Utilised over £900k Regional Innovation Funds
  (devolved from Department of Health to support
  Innovation in the regions)
Yorkshire & Humber Telehealth Hub
 Partnership made up of :
    3 of the region’s health organisations (Airedale FT,
     University of Hull and NHS Barnsley)
    Yorkshire and Humber Health Innovation and Education
     Cluster (HIEC)
    NHS Yorkshire and the Humber
 Offering Telehealth services to the rest of the region
 Services included:
    Telecoaching
    Telemonitoring
    Telemedicine

 Following the reported success of the Whole System
  Demonstrator projects
 Government concordat announced in January 2012
 Improve the lives of 3 million people living with long term

 Accelerating the use of Telecare and Telemonitoring
  technologies
3millionlives Y&H Approach
 Commissioned the NHS Technology Adoption Centre to
  facilitate events and work with organisations to progress
  their Telehealth action plans
 Regional Innovation Fund bursaries offered to support
  deployment of High Impact Innovations including 3ml
 Delivery of 3ml aligned to CQUIN payments for Trusts
 Y&H have put 3 Partnerships forward to act as
  Pathfinders with the DH
Digital First (formerly named Digital by Default)
 A ‘call to action’
 NHS organisations encouraged to utilise existing digital
  communication technologies e.g. mobile phones,
  websites, social networking, clinical systems etc
 To reduce unnecessary face to face contacts and DNAs
 On the basis that so many of these technologies are so
  readily available, why would they not take the
  opportunity to improve services to patients in this way
Digital First Y&H Approach
 RIF Bursaries offered to CCGs and Trusts to support
  deployment of High Impact Innovations incl. Digital First
 Delivery of Digital First aligned to CQUIN payments for
  Trusts
 Involving Y&H organisations in discussions with DH on
  issues such as barriers to deployment, how to calculate
  potential savings, organisational readiness etc
 Support Y&H organisations to deploy digital technologies
 Share good practice and learning across Y&H
 Promote the success of Y&H organisations
Other Telemedicine Projects in
Y&H
  Regional Telemedicine Stroke project deployed
   equipment in all Trusts in Y&tH (Airedale and Martin
   Dawes are delivery partners in this)
  Y&H Burns Network is working on a pilot to see if the
   same approach and infrastructure can benefit burns
   patients

  • 5. Yorkshire and Humber Region • Population 5.2 million • 15,906 square kilometres • Hull, Leeds, Sheffield and York • Rural areas
  • 6. Airedale NHS Foundation Trust • Positioned in the north west of the Yorkshire and Humber region • Population of 220,000 • Small towns – Keighley, Ilkley, Skipton, Colne • Serves patients in the Craven and Wharfedale areas of the Yorkshire Dales
  • 7. Long Term Conditions  Almost one in three people are living with a long term condition  That 30%+ of the population consume around 70% of the NHS budget  Over 50% of GP appointments  Almost 65% of outpatient appointments  Almost 75% of bed days  Three out of every five people aged over 60 are likely to suffer from a long term condition
  • 8. Population Trends  The region’s population as a whole is projected to increase by 455,000 between 2011 and 2031  The region’s population will become more ethnically diverse, the projections suggest that by 2031, 13% of the population with be from ethnic minorities as opposed to the 10% seen in 2011  The population will also age over the period, moving from 29 people of retirement age for every 100 of working age in 2011 to 43 people of retirement age for every 100 of working age by 2031  This rise in the elderly population equates to a population aged over 65 in the region of 980,000 in 2011 rising to 1,440,000 by 2031
  • 9. Modelling suggests that between 2010 and 2020  CHD will rise from 6.0% to 6.6% of the population, equivalent to 49,000 people  COPD will rise from 3.7% to 3.9% of the population, equivalent to 24,000 people  Hypertension will rise from 30.8% to 32.3% of the population, equivalent to 186,000 people  Stroke will rise from 2.7% to 2.8% of the population, equivalent to 21,000 people  CVD will rise from 6.9% to 7.6% of the population, equivalent to 59,000 people  Diabetes will rise from 7.5% to 8.5% of the population, equivalent to 77,000 people
  • 10. These factors represent a huge quality and efficiency challenge for our region’s health services
  • 11. Telehealth in Y&H in 2010  Some health communities in Y&H were embarking on significant developments:  Airedale - trailblazers in the use of telemedicine  Barnsley - blended service model  Hull - success with HF  North Yorkshire and York - aiming to reach 2000  Most other health communities were running small pilots involving very low patient numbers
  • 12. A Strategy for the Region  Chief Executive of NHS Yorkshire and the Humber, the Strategic Health Authority keen to progress Telehealth in the region  Chief Execs of the region’s Trusts agreed an approach  Utilised over £900k Regional Innovation Funds (devolved from Department of Health to support Innovation in the regions)

Yorkshire & Humber Telehealth Hub  Partnership made up of :  3 of the region’s health organisations (Airedale FT, University of Hull and NHS Barnsley)  Yorkshire and Humber Health Innovation and Education Cluster (HIEC)  NHS Yorkshire and the Humber  Offering Telehealth services to the rest of the region  Services included:  Telecoaching  Telemonitoring  Telemedicine

  • 18. 3millionlives Campaign  Following the reported success of the Whole System Demonstrator projects  Government concordat announced in January 2012  Improve the lives of 3 million people living with long term conditions  Over 5 years  Accelerating the use of Telecare and Telemonitoring technologies
  • 19. 3millionlives Y&H Approach  Commissioned the NHS Technology Adoption Centre to facilitate events and work with organisations to progress their Telehealth action plans  Regional Innovation Fund bursaries offered to support deployment of High Impact Innovations including 3ml  Delivery of 3ml aligned to CQUIN payments for Trusts  Y&H have put 3 Partnerships forward to act as Pathfinders with the DH
  • 20. Digital First (formerly named Digital by Default)  A ‘call to action’  NHS organisations encouraged to utilise existing digital communication technologies e.g. mobile phones, websites, social networking, clinical systems etc  To reduce unnecessary face to face contacts and DNAs  On the basis that so many of these technologies are so readily available, why would they not take the opportunity to improve services to patients in this way
  • 21. Digital First Y&H Approach  RIF Bursaries offered to CCGs and Trusts to support deployment of High Impact Innovations incl. Digital First  Delivery of Digital First aligned to CQUIN payments for Trusts  Involving Y&H organisations in discussions with DH on issues such as barriers to deployment, how to calculate potential savings, organisational readiness etc  Support Y&H organisations to deploy digital technologies  Share good practice and learning across Y&H  Promote the success of Y&H organisations
  • 22. Other Telemedicine Projects in Y&H  Regional Telemedicine Stroke project deployed equipment in all Trusts in Y&tH (Airedale and Martin Dawes are delivery partners in this)  Y&H Burns Network is working on a pilot to see if the same approach and infrastructure can benefit burns patients
