Understanding ICD-10
Implementation
at your office
WHAT IS ICD-10
ICD-10 is the 10th revision of the International
Statistical Classification of Diseases and Related
Health Problems (ICD), a medical classification list
by the World Health Organization (WHO).
Previously ICD-9 housed 13,000 codes, where as
the ICD-10 code set allow for approximately
68,000 codes.
WHEN IS THE ICD-10 COMPLIANCE
DEADLINE
The ICD-10 deadline to Comply is October
1st
, 2014.
CMS announced their final rule regarding
the ICD-10 compliance date on 08/24/12.
WHAT DOES COMPLIANCE MEAN
ICD-10 compliance means that a HIPAA-covered
entity uses ICD-10 codes for health care services
provided on or after October 1, 2014.
ICD-9 diagnosis codes cannot be used for
services provided on or after this date.
Everyone covered by HIPAA must be ICD-10
compliant starting on October 1, 2014.
WILL ICD-10 REPLACE CURRENT
PROCEDURAL TERMINOLOGY
CODING
No. It does not affect CPT coding for
outpatient procedures and physician services. ICD-
10-
PCS codes are for hospital inpatient procedures
only.
WHO IS AFFECTED BY THE
TRANSITION
TO ICD-10
Everyone covered by HIPAA!!
This includes health care providers and payers who
do not deal with Medicare claims. Organizations
that are not covered by HIPAA, but use ICD-9
codes should be aware that their coding may
become obsolete if they do not transition to ICD-10.
DO STATE MEDICAID PROGRAMS
NEED TO TRANSITION TO ICD-10
Yes. Everyone covered by HIPAA.
WHAT HAPPENS IF I DON’T SWITCH
TO ICD-10
Claims cannot be processed
Important to note, however, that claims for services
provided before October 1, 2014, must use ICD-9
diagnosis.
WILL I NEED TO USE BOTH ICD-9
AND ICD-10 CODES DURING THE
TRANSITION
Practice management systems must be able
to accommodate both ICD-9 and ICD-10
codes until all claims and other transactions
for services before October 1, 2014, have
been processed and completed.
IF I TRANSITION EARLY TO ICD-10,
WILL CMS BE ABLE TO PROCESS MY
CLAIMS
No. However, organizations will need to work
with their internal team and business trading partners
to
test their software systems from beginning to end. This
involves testing claims, eligibility verification, quality
reporting and other transactions and processes using
ICD-10 to make sure the new code set can be
processed
correctly.
CODES CHANGE EVERY YEAR, SO
WHY IS THE TRANSITION TO ICD-10
DIFFERENT
ICD-10 codes are completely different from ICD-9.
ICD-9 codes are mostly numeric and have 3 to 5 digits.
ICD-10 codes are alphanumeric and contain 3 to 7 characters.
ICD-10 is more robust and descriptive with “one-to-many” matches to ICD-9 in
some instances.
ICD-10 codes will be updated every year.
ICD-9 codes will not continue to be updated after October 1, 2014.

WHY IS THE SWITCH TO ICD-10
HAPPENING
ICD-9 is 30 years old.
ICD-9 has outdated and obsolete terms.
ICD-9 is inconsistent with current medical practices.
ICD-9 structure limits the number of new codes that can be
created, and many categories are full.

ICD-10 will accommodate newly developed diagnoses and
procedures, innovations in technology and treatment,
performance-based payment systems, and more accurate billing.
ICD-10 coding will make the billing process more streamlined
and efficient, and this will also allow for more precise methods of
detecting fraud.

PREPARE FOR THE TRANSITION TO
ICD-10
Steps To Take Now: Some activities might not be necessary
for small practices where one or two people are handling the transition
activities.
• Establish a transition team or ICD-10 project
coordinator.
• Develop a Plan.
Include a timeline that identifies tasks to be completed and
crucial milestones/relationships, task owners, resources
needed, and estimated start and end times.

Determine how ICD-10 will affect your organization. Start by
reviewing how and where you currently use ICD-9.
i.e. authorizations/pre-certifications, physician orders, medical records, superbills/encounter
forms, practice management and billing systems, and coding manuals.

Review how ICD-10 will affect clinical documentation
requirements and electronic health record (EHR) templates.
•
Communicate the plan, timeline, and new system changes and
processes.
Ensure that leadership and staff understand the extent of the effort the transition requires.

WORKING PROPERLY
• Plan to test systems early.
• Test claims, eligibility verification,
quality reporting, and processes from
beginning to end.
STEPS IN THE TESTING PHASE
Work with practice management and/or EHR and
coders/billers to develop processes and systems using ICD-
10 codes.
Determine when you will be ready to test, and work with
payers and any clearinghouses or billing services that you
use to schedule testing.
Develop a testing plan that outlines key dates and
milestones for when tests should be completed.
CMS implementation timelines and checklists are available
for small and medium practices.

Have a plan in place and make sure it documents
the steps being followed and the dates that
milestones will be achieved.
Include vendor tasks.
Establish an emergency fund to cover unexpected
costs and possible reimbursement delays.

Ask clearinghouses what their timelines are for
testing and implementation from beginning to end.
Review and evaluate trading partner agreements
and contracts.

TRANSITION
Products and services will be obsolete if steps
are not taken to prepare.
Please note, clearinghouse services do not convert ICD-9 to ICD-10 for
providers and payers.
CMS has resources to help vendors,
clearinghouses, and prepare for a smooth
transition.

CMS’s website.
WHY SHOULD I PREPARE NOW FOR
THE ICD-10 TRANSITION
It’s all about having a focus on your long-term goals
and modifying your day-to-day operations to support
the transition.
The transition from ICD-9 to ICD-10 will change how
you do business. You will need to devote staff time and
financial resources for transition activities.
The transition will go much more smoothly for
organizations that plan ahead and prepare now.
TRAINING PROVIDERS AND STAFF
WILL NEED
American Health Information Management Association
recommends training begin no more than six to nine months
before the October 1, 2014.
Coders in physician practices will need to learn ICD-10
diagnosis coding only, ICD-10 coding training may be
integrated into the CEUs that certified coders must take to
maintain their credentials.
Some high-level ICD-10 training will be required earlier to
learn and understand how the structure and granularity of
the ICD-10 codes will affect clinical documentation.

OPPORTUNITIES
CMS collaborated with Medscape Education on two
video lectures and an expert articles are available to
anyone who registers with Medscape at no cost.
Continuing medical education credits are available to
physicians who complete the activities and take a post-
test.

  • 1. CLIMBING THE ICD-10 LADDER Understanding ICD-10 Implementation at your office
  • 2. WHAT IS ICD-10 ICD-10 is the 10th revision of the International Statistical Classification of Diseases and Related Health Problems (ICD), a medical classification list by the World Health Organization (WHO). Previously ICD-9 housed 13,000 codes, where as the ICD-10 code set allow for approximately 68,000 codes.
  • 3. WHEN IS THE ICD-10 COMPLIANCE DEADLINE The ICD-10 deadline to Comply is October 1st , 2014. CMS announced their final rule regarding the ICD-10 compliance date on 08/24/12.
  • 4. WHAT DOES COMPLIANCE MEAN ICD-10 compliance means that a HIPAA-covered entity uses ICD-10 codes for health care services provided on or after October 1, 2014. ICD-9 diagnosis codes cannot be used for services provided on or after this date. Everyone covered by HIPAA must be ICD-10 compliant starting on October 1, 2014.
  • 5. WILL ICD-10 REPLACE CURRENT PROCEDURAL TERMINOLOGY CODING No. It does not affect CPT coding for outpatient procedures and physician services. ICD- 10- PCS codes are for hospital inpatient procedures only.
  • 6. WHO IS AFFECTED BY THE TRANSITION TO ICD-10 Everyone covered by HIPAA!! This includes health care providers and payers who do not deal with Medicare claims. Organizations that are not covered by HIPAA, but use ICD-9 codes should be aware that their coding may become obsolete if they do not transition to ICD-10.
  • 7. DO STATE MEDICAID PROGRAMS NEED TO TRANSITION TO ICD-10 Yes. Everyone covered by HIPAA.
  • 8. WHAT HAPPENS IF I DON’T SWITCH TO ICD-10 Claims cannot be processed Important to note, however, that claims for services provided before October 1, 2014, must use ICD-9 diagnosis.
  • 9. WILL I NEED TO USE BOTH ICD-9 AND ICD-10 CODES DURING THE TRANSITION Practice management systems must be able to accommodate both ICD-9 and ICD-10 codes until all claims and other transactions for services before October 1, 2014, have been processed and completed.
  • 10. IF I TRANSITION EARLY TO ICD-10, WILL CMS BE ABLE TO PROCESS MY CLAIMS No. However, organizations will need to work with their internal team and business trading partners to test their software systems from beginning to end. This involves testing claims, eligibility verification, quality reporting and other transactions and processes using ICD-10 to make sure the new code set can be processed correctly.
  • 11. CODES CHANGE EVERY YEAR, SO WHY IS THE TRANSITION TO ICD-10 DIFFERENT ICD-10 codes are completely different from ICD-9. ICD-9 codes are mostly numeric and have 3 to 5 digits. ICD-10 codes are alphanumeric and contain 3 to 7 characters. ICD-10 is more robust and descriptive with “one-to-many” matches to ICD-9 in some instances. ICD-10 codes will be updated every year.   • 12. WHY IS THE SWITCH TO ICD-10 HAPPENING ICD-9 is 30 years old. ICD-9 has outdated and obsolete terms. ICD-9 is inconsistent with current medical practices. ICD-9 structure limits the number of new codes that can be created, and many categories are full. ------------------------------------------------------------------------------------- ICD-10 will accommodate newly developed diagnoses and procedures, innovations in technology and treatment, performance-based payment systems, and more accurate billing. ICD-10 coding will make the billing process more streamlined and efficient, and this will also allow for more precise methods of detecting fraud.

WHAT SHOULD PROVIDERS DO TO PREPARE FOR THE TRANSITION TO ICD-10 Steps To Take Now: Some activities might not be necessary for small practices where one or two people are handling the transition activities. • Establish a transition team or ICD-10 project coordinator. • Develop a Plan. Include a timeline that identifies tasks to be completed and crucial milestones/relationships, task owners, resources needed, and estimated start and end times.   • 14. WHAT SHOULD PROVIDERS DO TO PREPARE FOR THE TRANSITION TO ICD-10 (Continued) • Determine how ICD-10 will affect your organization. Start by reviewing how and where you currently use ICD-9. i.e. authorizations/pre-certifications, physician orders, medical records, superbills/encounter forms, practice management and billing systems, and coding manuals. • Review how ICD-10 will affect clinical documentation requirements and electronic health record (EHR) templates. • Communicate the plan, timeline, and new system changes and processes. Ensure that leadership and staff understand the extent of the effort the transition requires.

WHAT SHOULD PROVIDERS DO TO PREPARE FOR THE TRANSITION TO ICD-10(Continued) • Secure a Budget. • Talk with your payers, billing, and IT staff, and Vendors. • Coordinate your transition plans. among your trading partners and evaluate contracts with payers and vendors for policy revisions, testing timelines, and costs related to the ICD-10 transition. • Create a testing plan.   • 16. WHAT YOU NEED TO DO TO MAKE SURE YOUR ICD-10 SYSTEMS ARE WORKING PROPERLY • Plan to test systems early. • Test claims, eligibility verification, quality reporting, and processes from beginning to end.
  • 17. STEPS IN THE TESTING PHASE Work with practice management and/or EHR and coders/billers to develop processes and systems using ICD- 10 codes. Determine when you will be ready to test, and work with payers and any clearinghouses or billing services that you use to schedule testing.   • 18. ENSURING A SMOOTH TRANSITION Have a plan in place and make sure it documents the steps being followed and the dates that milestones will be achieved. Include vendor tasks.   • 19. WHAT SHOULD PAYERS DO TO PREPARE FOR THE TRANSITION Ask clearinghouses what their timelines are for testing and implementation from beginning to end. Review and evaluate trading partner agreements and contracts.
  • 20. WHAT SHOULD SOFTWARE VENDORS AND CLEARINGHOUSES BE DOING TO PREPARE THEIR CUSTOMERS FOR THE TRANSITION Products and services will be obsolete if steps are not taken to prepare. Please note, clearinghouse services do not convert ICD-9 to ICD-10 for providers and payers. CMS has resources to help vendors, clearinghouses, and prepare for a smooth transition.

  • 22. WHY SHOULD I PREPARE NOW FOR THE ICD-10 TRANSITION It’s all about having a focus on your long-term goals and modifying your day-to-day operations to support the transition. The transition from ICD-9 to ICD-10 will change how you do business. You will need to devote staff time and financial resources for transition activities. The transition will go much more smoothly for organizations that plan ahead and prepare now.
  • 23. TRAINING PROVIDERS AND STAFF WILL NEED American Health Information Management Association recommends training begin no more than six to nine months before the October 1, 2014. Coders in physician practices will need to learn ICD-10 diagnosis coding only, ICD-10 coding training may be integrated into the CEUs that certified coders must take to maintain their credentials. Some high-level ICD-10 training will be required earlier to learn and understand how the structure and granularity of the ICD-10 codes will affect clinical documentation.
