EXPERIENCE
Case Manager December 2015 – Present
Children’s Community Services Bronx, New York
 Conducting intake interviews and assessments.
 Counseling clients with confronting issues adversely affecting their lives (i.e.: homelessness, mental
illness, addiction, abuse).
 Ensuring clients have appropriate medical insurance and are referred to medical provider.
 Assisting clients in finding housing and employment.
 Maintaining updated case files with necessary documentation.
 Networking with community partners.
 Conduct bi-weekly and weekly Independent Living Plan (ILP).
 Schedule weekly meetings with families in caseload. Create case plans and monitor these plans for
effectiveness.
 Keep records of meeting and actions taken by maintaining physical files as well as electronic files within
CARES.
 Evaluate new and existing client’s needs to determine their immediate needs, functional capabilities and
assistance needed to stabilize housing, healthcare, finances, etc.
 Establish and maintain working relationships with public agencies concerning public benefits,
employment services, supportive services, housing resources, etc.
 Attend and participate in all required agency meetings: staff meetings, house meetings, in-
service training and weekly case review.
 Conduct home inspections as required.

 Maintain a high case load.
 Responsible for coordinating intake interviews and assessments for new clients.
 Conduct a thorough needs assessment and develop a service plan to address identified, concrete
urgencies; using a short-term intervention model.

 Ensures all service plans/independent living plans are updated and reviewed with the client according to
program/regulatory mandates.
 Meets with client according to program/regulatory mandated frequencies in a face-to-face meetings
and/or services with/on behalf of clients.
 On-going assessment of participants' needs and required related services.
 Documents all referrals for and encounters with or on behalf of the client in program's case management
database.
 Attend New York City Housing Authority after care lease signings with clients
 Maintain files up to agency standards.
 Escort client/s to appointments as needed.
 Extensive knowledge of CARES, NYSHA and 2010E Housing Application for individuals with special
needs.
 Serves as a liaison with various agencies, including, but not limited to, medical/health care providers,
mental health providers, legal services, Human Resource Administration, housing subsidy providers,
Department of Homeless Services, and employment training providers in the interest of the clients.
 Advocates for and assists clients in the negotiation of the service delivery system, to ensure the receipt
of entitlements and permanent housing/housing stabilization.
 Responsible for conducting initial intake interviews and assessments (as determined by site protocols)
for all new clients and establishing a case file with all signed forms and entering/revising all initial case
management database information as indicated by site specific protocols.
 Provides information and referral services to clients as needed or as specified in the service
plan/independent living plan.
 Access internal and external financial assistance to support permanent housing. E.g. LINC IV, V & VI.
 Responsible for placing at least four (4) clients in permanent housing each month.
