1. Necrosis infectada mas deterioro clínico.
2. Necrosis infectada sin deterioro clínico en sitios donde no se pueda
realizar abordaje endoscópico o radiológico.
3. No evidencia de necrosis infectada pero mala evolución clínica
después de la cuarta semana.(Abscesos ocultos).
4. Isquemia intestinal o perforación de víscera hueca, Obstrucción
intestinal secundaria
5. Pancreatitis biliar mas colangitis en los lugares donde no se disponga
de CPRE

• 1. Paciente que tras varias semanas de tto conservador persistan
con dolor abdominal recurrente, nauseas, vómitos, letárgicos,
hiperamilasemia, (estos pacientes presentan grandes cantidades de
tejido necrótico retroperitoneal.
• 2. Paciente con ruptura postnecrotica del conducto pancreático
principal, catalogado como síndrome del conducto pancreático roto
(desconneted duct síndrome).
• 3.Oclusion intestinal o estenosis biliar como consecuencia de la
organización de la necrosis.
