BASIC SAFETY INSTRUCTIONS


The designated Production In-charge(s) shall ensure Safe Production of work intended to be
performed under the above company logo (irrespective of location) by means of all possible,
available, stipulated, specified, rules, regulations and through this basic SAFETY INSTRUCTIONS. This
document is a BASIC SAFETY INSTRUCTION and should be used to verify the status of safety
compliance.
This document consists of gist of SAFETY INSTRUCTIONS and cross reference is extended to guidelines,
rules and regulations of Dubai Municipality, EHSS, Nakheel, and Federal Law of United Arab Emirates.


                                       ACTIVITY BASED:

RIGGING AND SLINGING:

   1. Always Certified Rigger should undertake the job of Rigging and must wear Fluorescent
       jacket

   2. Tested Slings, proper slinging techniques and sound equipments to be used.

   3. Ensure no person under the suspended load.

   4. Load should never be touched with hand directly. Tag line should be used.

   5. Cylinders must be lifted in Vertical and in Cylinder Racks duly tested.

   6. No rigging to be done if the wind speed is more than 30Km/hr.

   7. Only certified Crane operator must operate the Crane.

LIFTING EQUIPMENT:

   8. Always ensure proper color code of the period for all lifting equipment.

   9. All lifting equipment must be visually verified prior, during and end of the work.

  10. Proper Eyes to be welded at right place so that weight to get distributed equally. Always to
     ensure the Eye thickness be more than the plate size.

  11. Web Slings must be verified with special focus.

TRANSPORTATION:

  12. All items intended to transport from either Stores and or from one site to another must be
     supported with Delivery Challan.

  13. At the loading point, the person who has supervised loading shall ensure that all items are
     secured properly, contains Delivery Challan and are in sound working conditions.

  14. Wherever possible, no projection is allowed out of vehicle without RED light and REG flag.

  15. Load should be placed in order of unloading and keeping in view centre of gravity. This shall
     be done with mutual concurrence with driver.

  16. During loading any items found defective visually shall not be loaded and the replacement
     to be loaded from Stores.

WORKING AT HEIGHTS:

  17. Any height above 1.8meters shall be considered at Working at Height and shall attract all
     statutory guidelines of the activity.

  18. Ensure that Full Body Safety Harness is used by persons working at height.

  19. No loose items shall be kept at Staging/heights.

  20. Certified Scaffolders alone shall erect Scaffolding. No one is allowed to ALTER even.

  21. If there is a change in condition, high wind speed, visual defect, loosened joints, unstable etc
     the same must be showed to the certified Scaffolder.

  22. All care shall be taken while boarding the vessel at Anchorage.

  23. Always avoid boarding of MEN and EQUIPMENT together.

HOT WORKS:

  24. Ensure good housekeeping

  25. Ensure no cables and hoses are tangled at workplace.

  26. Ensure no leakage in hoses. To repeat periodical check at every beginning of the shift prior
     started job.

  27. All grinders must have a dead man switch functioning well.

  28. Always to ensure the correct grinding wheel be used.

  29. All hot works must be followed strictly against Hot Work Permits. If hot work has to be
     performed in confined space, then a separate permit of Entry to Confined Space is to be
     obtained.

  30. Even after obtaining Permit of Hot work, Confined Space entry , the supervisor shall doubly
     verify for Gas Free work atmosphere. Any slightest doubt must be brought to the notice of
     Safety person on duty.

  31. No naked flame up to 5 meters for the Cylinders of oxygen and Acetylene.

  32. All Hot Work must be done only after RISK ASSESSMENT.

  33. All precautions to be taken with respect to heat transfer mode viz conduction, convection,
     direct, radiation etc.

  34. Hot work must not be started in absence of Fire Extinguisher and or Fire Fighting
     arrangement.

  35. Tool box Talk is compulsory.

  36. Hot Work Permit must be displayed at work site.

  37. Permits must be renewed> no work to be commenced during the permit issue process.

CONFINED SPACES:

  38. Ensure that no person enters in any confined space without Gas Free Certificate and or
     permit to Entry to Confined Space.

  39. Since Entry to Confined Space and Gas Free Certificate is highly CONDITIONAL in nature, one
     must be careful in its expiry and or renewal conditions and expiry time, too

  40. Remarks on the Permit and Certificates must be closely adhered to.

  41. Effective Ventilation and Lighting must be ensured continuously during the job.

  42. Hotwork to be done atleast 0.50meter away from the adjacent non-gas free tanks.

  43. Explosion proof lighting must be used to avoid Explosion due to spark, flammable gases.

  44. Minimum of 2 persons shall enter into Confined Space at any given time.

  45. OXYGEN LEVEL is a key element in Confined Space. It must be in between 19~21%. Lower
     than limit turns work place Toxic and excess turns it as Explosive.

  46. It is recommended to have a portable calibrated Oxygen / Gas detector and be carried by a
     person in the team, who is first to enter and last to exit of the work place.

  47. Confined space will be sounded for fluid before being entered.

  48. Standby person shall be deployed at the entrance of confined space. He will keep a tally of
     numbers /names of persons inside the confined space.

  49. Oxygen / Acetylene hoses will be removed from confined space during extended breaks and
     air re-tested for gas before re-entry.

HOUSE KEEPING:

  50. Prior, during and at the end of the shift – the basic HOUSE KEEPING shall CONTINUOUSLY be
     followed.

   51. This includes but not limited to well planning of job, designate location of Fire Extinguishers,
       Displays, Cylinders location, categorization of temporary storage areas, barricading, signage,
       cable and hose management, handling of men, machines, tools and equipment, clean floor
       etc..,

  ANY EMPLOYEE WHO FAIL TO FOLLOW THE PRESCRIBED SAFETY INSTRUCTIONS MIGHT
     ATTRACT THE DISCIPLINARY ACTION AND NOT LIMITED TO REMOVAL FROM SITE.

                          ADDITIONAL GUIDELINES INCLUDES:


1. Safety signs placed around the site in strategic locations to be followed like “NO SMOKING” (in
   hazardous areas), “Hard Hat Area” “Danger-Construction Site – Do not Enter” “Use Safety
   Harness”,“Entry for Authorised Personnel only” “Wear Safety goggles”

2. Possession or use of any alcohol, mind-altering drugs, or other controlled substance can be a
   ground for immediate dismissal. Gambling, fighting, horseplay, or any objectionable items and
   acts at the work place or in areas under company control will be a cause for dismissal.

3. Hard hats will be worn at all times. Painted hard hats are not allowed nor are alteration of the
   hat or suspension permitted. Metal hard hats are not permitted.

4. Proper eye protection must be worn when exposed to flying objects, dust, hot splashing metals,
   harmful rays, or other eye hazards.

5. Proper ear protection must be worn. Use when required.

6. Proper respiratory equipment will be worn when spray painting, burning and when exposed to
   dust, toxic hazards, or when working in confined areas.

7. All accidents, injuries, hazardous practices or condition shall be reported to the supervisor
   immediately for corrective action and medical attention.

8. In the event of on-job injury requiring medical attention, employee will be sent to First Aid
   Station. Employee is to report for work, after rest and physically fit to do the job.

9. Wear safety footwear that is in good condition and suitable for your job.

10. Wearing short pants or removal of upper shirts/trousers shall not be allowed at the site.

11. Wear clothing that is suitable for both the weather and your work. Torn or loose clothing and
   shirttails worn outside trousers are prohibited. You are required to work fully clothed.

12. Use gloves or other suitable skin protection when handling rough materials, chemicals and hot
   or cold objects. Replaced when worn or damaged.

13. Special safety equipment is provided for your protection. Use when required and keep in good
   working condition. Report immediately any loss or damage so replacement can be made.

14. Good housekeeping and fire prevention regulation will be maintained at all times. Scrap must
   be removed from work areas each day.

15. Do not ride fork lift equipment, equipment loads and hooks. Remain seated with arms and legs
   inside the body of any moving vehicle.

16. Defective power or hand tools will not be used.

17. All safe practices shall be followed in hoisting or suspending loads.

18. Explosives, flammable materials, electrical equipment, heating devices, compressed air and
   pressure vessels shall be handled according to safety rules and regulations.

19. Make sure ladders are in good working condition. Set firmly and secured in place.

20. Do not start or move machinery, operate valves or charge electric switches until you have
   determined it is safe to do so.

21. Never work under vehicles or equipment supported by jacks or hoists without protective
   blocking and a safety stand.

HSE MANAGEMENT SYSTEM Vessel/Job: Page 1 of 6 Implementation Date:01.01.2012 BASIC SAFETY INSTRUCTIONS BASIC SAFETY INSTRUCTIONS The designated Production In-charge(s) shall ensure Safe Production of work intended to be performed under the above company logo (irrespective of location) by means of all possible, available, stipulated, specified, rules, regulations and through this basic SAFETY INSTRUCTIONS. This document is a BASIC SAFETY INSTRUCTION and should be used to verify the status of safety compliance. This document consists of gist of SAFETY INSTRUCTIONS and cross reference is extended to guidelines, rules and regulations of Dubai Municipality, EHSS, Nakheel, and Federal Law of United Arab Emirates. ACTIVITY BASED: RIGGING AND SLINGING: 1. Always Certified Rigger should undertake the job of Rigging and must wear Fluorescent jacket 2. Tested Slings, proper slinging techniques and sound equipments to be used. 3. Ensure no person under the suspended load. 4. Load should never be touched with hand directly. Tag line should be used. 5. Cylinders must be lifted in Vertical and in Cylinder Racks duly tested. 6. No rigging to be done if the wind speed is more than 30Km/hr. 7. Only certified Crane operator must operate the Crane. LIFTING EQUIPMENT: 8. Always ensure proper color code of the period for all lifting equipment. 9. Always to ensure the Eye thickness be more than the plate size. 11. Web Slings must be verified with special focus. TRANSPORTATION: 12. All items intended to transport from either Stores and or from one site to another must be supported with Delivery Challan. 13. At the loading point, the person who has supervised loading shall ensure that all items are secured properly, contains Delivery Challan and are in sound working conditions. 14. Wherever possible, no projection is allowed out of vehicle without RED light and REG flag. 15. Load should be placed in order of unloading and keeping in view centre of gravity. This shall be done with mutual concurrence with driver. 16. During loading any items found defective visually shall not be loaded and the replacement to be loaded from Stores. WORKING AT HEIGHTS: 17. Any height above 1.8meters shall be considered at Working at Height and shall attract all statutory guidelines of the activity. 18. Ensure that Full Body Safety Harness is used by persons working at height. 19. No loose items shall be kept at Staging/heights. 20. Certified Scaffolders alone shall erect Scaffolding. No one is allowed to ALTER even. 21. If there is a change in condition, high wind speed, visual defect, loosened joints, unstable etc the same must be showed to the certified Scaffolder. 22.   • 3. HSE MANAGEMENT SYSTEM Vessel/Job: Page 3 of 6 Implementation Date:01.01.2012 BASIC SAFETY INSTRUCTIONS 23. Always avoid boarding of MEN and EQUIPMENT together. HOT WORKS: 24. Ensure good housekeeping 25. Ensure no cables and hoses are tangled at workplace. 26. Ensure no leakage in hoses. To repeat periodical check at every beginning of the shift prior started job. 27. All grinders must have a dead man switch functioning well. 28. Always to ensure the correct grinding wheel be used. 29. All hot works must be followed strictly against Hot Work Permits. If hot work has to be performed in confined space, then a separate permit of Entry to Confined Space is to be obtained. 30. Even after obtaining Permit of Hot work, Confined Space entry , the supervisor shall doubly verify for Gas Free work atmosphere. Any slightest doubt must be brought to the notice of Safety person on duty. 31. No naked flame up to 5 meters for the Cylinders of oxygen and Acetylene. 32. All Hot Work must be done only after RISK ASSESSMENT. 33. All precautions to be taken with respect to heat transfer mode viz conduction, convection, direct, radiation etc. 34. Hot work must not be started in absence of Fire Extinguisher and or Fire Fighting arrangement. 35. Tool box Talk is compulsory. 36. Hot Work Permit must be displayed at work site. 37. Ensure that no person enters in any confined space without Gas Free Certificate and or permit to Entry to Confined Space. 39. Since Entry to Confined Space and Gas Free Certificate is highly CONDITIONAL in nature, one must be careful in its expiry and or renewal conditions and expiry time, too 40. Remarks on the Permit and Certificates must be closely adhered to. 41. Effective Ventilation and Lighting must be ensured continuously during the job. 42. Hotwork to be done atleast 0.50meter away from the adjacent non-gas free tanks. 43. Explosion proof lighting must be used to avoid Explosion due to spark, flammable gases. 44. Minimum of 2 persons shall enter into Confined Space at any given time. 45. OXYGEN LEVEL is a key element in Confined Space. It must be in between 19~21%. Lower than limit turns work place Toxic and excess turns it as Explosive. 46. It is recommended to have a portable calibrated Oxygen / Gas detector and be carried by a person in the team, who is first to enter and last to exit of the work place. 47. Confined space will be sounded for fluid before being entered. 48. Standby person shall be deployed at the entrance of confined space. He will keep a tally of numbers /names of persons inside the confined space. 49. Oxygen / Acetylene hoses will be removed from confined space during extended breaks and air re-tested for gas before re-entry. HOUSE KEEPING: 50. Prior, during and at the end of the shift – the basic HOUSE KEEPING shall CONTINUOUSLY be followed. AUTHOR: Uma Mahesh Guraza, HSE Manager

This includes but not limited to well planning of job, designate location of Fire Extinguishers, Displays, Cylinders location, categorization of temporary storage areas, barricading, signage, cable and hose management, handling of men, machines, tools and equipment, clean floor etc.., ANY EMPLOYEE WHO FAIL TO FOLLOW THE PRESCRIBED SAFETY INSTRUCTIONS MIGHT ATTRACT THE DISCIPLINARY ACTION AND NOT LIMITED TO REMOVAL FROM SITE. ADDITIONAL GUIDELINES INCLUDES: 1. Safety signs placed around the site in strategic locations to be followed like “NO SMOKING” (in hazardous areas), “Hard Hat Area” “Danger-Construction Site – Do not Enter” “Use Safety Harness”,“Entry for Authorised Personnel only” “Wear Safety goggles” 2. Possession or use of any alcohol, mind-altering drugs, or other controlled substance can be a ground for immediate dismissal. Gambling, fighting, horseplay, or any objectionable items and acts at the work place or in areas under company control will be a cause for dismissal. 3. Hard hats will be worn at all times. Painted hard hats are not allowed nor are alteration of the hat or suspension permitted. Metal hard hats are not permitted. 4. Proper eye protection must be worn when exposed to flying objects, dust, hot splashing metals, harmful rays, or other eye hazards. 5. Proper ear protection must be worn. Use when required. 6. Proper respiratory equipment will be worn when spray painting, burning and when exposed to dust, toxic hazards, or when working in confined areas. 7. All accidents, injuries, hazardous practices or condition shall be reported to the supervisor immediately for corrective action and medical attention. 8. In the event of on-job injury requiring medical attention, employee will be sent to First Aid Station. Employee is to report for work, after rest and physically fit to do the job. 9. Wear safety footwear that is in good condition and suitable for your job. 10. Wearing short pants or removal of upper shirts/trousers shall not be allowed at the site. Torn or loose clothing and shirttails worn outside trousers are prohibited. You are required to work fully clothed. 12. Use gloves or other suitable skin protection when handling rough materials, chemicals and hot or cold objects. Replaced when worn or damaged. 13. Special safety equipment is provided for your protection. Use when required and keep in good working condition. Report immediately any loss or damage so replacement can be made. 14. Good housekeeping and fire prevention regulation will be maintained at all times. Scrap must be removed from work areas each day. 15. Do not ride fork lift equipment, equipment loads and hooks. Remain seated with arms and legs inside the body of any moving vehicle. 16. Defective power or hand tools will not be used. 17. All safe practices shall be followed in hoisting or suspending loads. 18. Explosives, flammable materials, electrical equipment, heating devices, compressed air and pressure vessels shall be handled according to safety rules and regulations. 19. Make sure ladders are in good working condition. Set firmly and secured in place. 20. Do not start or move machinery, operate valves or charge electric switches until you have determined it is safe to do so. 21.
