PARTS OF AN IMPLANT RESTORATION
Abutment
Crown
Titanium Implant
Fitted over the Abutment
for a natural appearance.
Fitted over the portion of
implant that protrudes
from the gum line
Surgically fused with the
jawbone.
5. CONCEPT
The use of smaller diameter abutment on a
larger diameter implant collar.
Shifts the perimeter of the implant-
abutment junction (IAJ) inward toward the
central axis of the implant.
Reduces crestal bone loss.
5
6. HISTORY
• Accidental finding!
• In 1980s, dental implant manufacturers introduced wider diameter implants (5-6 mm).
• When introduced matching-diameter prosthetic components were unavailable.
• So, they were restored with standard diameter prosthetic components (4.1 mm)
Long-term radiographic follow-up of "platform-
switched" implants showed
Less crestal bone as compared to “platform
matched” implants

7. HOW PLATFORM SWITCHING REDUCES CRESTAL BONE LOSS?
Shifts the inflammatory cell infiltrate inward & away from crestal bone
Decreases influence of micro gap on crestal bone
Maintenance of biologic width
Decreases stress level in peri implant bone

8. HOW PLATFORM SWITCHING REDUCES CRESTAL BONE LOSS?
Shifts the inflammatory cell infiltrate inward & away from crestal bone.
Inflammatory cell infiltrate
IAJ Platform switching
IAJ inward and away from outer edge of implant and adjacent bone
Decreasing the effect of inflammatory cells on crestal bone
8
9. MAINTENANCE OF BIOLOGIC WIDTH
Provides additional horizontal biologic width.
Decreases stress level in peri implant bone
Platform switching
Shifts the stress concentration area away from bone-implant interface
Reduce crestal bone resorption
9
12. LIMITATIONS
• Deformation of abutment / screw.
• Compromise the emergence profile
in esthetic areas.
• Under sizing of the components
during all phases of the treatment.
12
13. CONCLUSION
Crestal bone preservation
Platform switching limits crestal
resorption
It maintains the biological width
& helps improve esthetics
