Sample MIChild Decision 09/16/2010 Conner Test           Application Number:  821078 Congratulations! Conner is approved for  MIChild .  MIChild  coverage starts on October 01, 2010 and is good for one year. We will send information about how to renew coverage at the end of that year. If you have Medicaid or become eligible for Medicaid,  MIChild  will end. We will send you a letter if this happens. The names and phone numbers for your  MIChild  providers are listed below.  Medical Plan: Blue Cross/Blue Shield of Michigan, Phone: 1-800-543-7765 Dental Plan: Delta Dental Plan of Michigan, Phone: 1-800-482-8915 Mental Health Plan: Clinton-Eaton-Ingham CMH, Phone: (517) 346-8200 Substance Abuse Plan: Mid-South Substance Abuse Commission, Phone: (517) 337-4406 You can change health or dental plans within 90 days of the start date. Your health plan card should come in the mail soon. You may start using services on the start date shown above. When you make an appointment, please tell your provider the name of your health or dental plan. Your approval is based on the information you gave about your family. You may be audited to check your income. If you are audited you will be asked to send check stubs or a letter from your employer. If you do not send the information for the audit your  MIChild  will end. Your payment coupons will be mailed to you. Your first payment is $20 for the first two months of coverage and is due as soon as you get your coupons. Each family must pay $10 by the 10th of each month. Remember to write your  Application Number  on your check or money order. If you do not receive your payment coupons within 2 weeks, please call us toll-free at 1-888-988-6300. If you need immediate services, take your Guarantee of Payment Letter and give it to your doctor as proof of MOMS coverage. The second Guarantee of Payment Letter is kept by the agency. It should be stated that the current system does not allow clients to report changes once they have submitted the application (via the system). The process for reporting changes will be covered in training, but for now, it is important to note that the goal is to have this function available online eventually/soon.
