En cambio, ninguno de los esquemas de
valoración del riesgo hemorrágico, incluido el

El tratamiento antitrombótico es la
anticoagulación.
Las Guías hacen una extensa revisión de los
estudios en este sentido que demuestran el más
que limitado papel de la antiagregación, no sólo
para los pacientes de bajo riesgo, donde no hay
estudios, sino también en los pacientes mayores
de 75 años o en los que a priori no se considera
candidatos a tratamiento anticoagulante con
warfarina.

46. Aparecen los Nuevos Anticoagulantes al mismo nivel de recomendación que la Warfarina
(Clase I) para pacientes con FA no valvular con un CHA2DS2VASc=2.
La decisión del tratamiento a elegir deberá individualizarse según las características del
paciente, pero igualmente haciéndole partícipe de la decisión.
Para pacientes con CHA2DS2VASc=0 es razonable no indicar anticoagulación (Clase IIa), y
para pacientes con CHA2DS2VASc=1 cualquier opción es posible (nada, anticoagular o
antiagregar) (Clase IIb).
Se presentan todos los estudios de los Nuevos Anticoagulantes y se reconoce sus
similitudes en diseño, pero las Guías no se inclinan hacia el uso de uno u otro en
diferentes escenarios atendiendo a sus diferencias. Fundamentándose en que no existen
estudios que los hayan comparado directamente, las Guías adoptan una posición
salomónica en cuanto a su uso.

Control de frecuencia ventricular usando Beta bloqueador o

recomendado para FA paroxística, persistente o permanente.

frecuencia ventricular en fase aguda en pacientes sin pre excitación. En

pocos agentes de control de ritmo que no
tienen efectos inotrópicos negativos .

Resumen del tratamiento de la frecuencia
En cuanto a la frecuencia objetivo, las Guías siguen manteniendo el límite en
una frecuencia cardíaca en reposo <80 lpm (Recomendación IIa).
Aunque el estudio RACEII objetivó que control mas laxo (< 110 lpm) tenía
resultados en eficacia similares, las limitaciones de este estudio sitúan este
objetivo como una Recomendación IIb.
El tratamiento de elección siguen siendo betabloqueantes y calcio
antagonistas no dihidropiridínicos (Recomendación Clase I).
Amiodarona ev es una opción en el escenario agudo (Recomendación Clase
IIa), pero no es una opción vía oral en el tratamiento crónico (IIb).

Es útil en FA paroxística sintomática,
recurrente o intolerante a por lo menos 1

67. Resumen control de ritmo
Los nuevos anticoagulantes son una opción en la anticoagulación
pericardioversión eléctrica (Clase IIa).
Flecainida y propafenona son la primera opción para la cardioversión
farmacológica (Recomendación Clase I); amiodarona “oral” (Recomendación Clase
IIa).
En cuanto al tratamiento a largo plazo, los fármacos antiarrítmicos pasan a ser
una Recomendación Clase I (en las Guías previas eran IIa); la amiodarona debe ser
considerada por su perfil de seguridad sólo cuando otros fármacos han fallado o
están contraindicados.
Ablación pasa a ser una Recomendación Clase I (antes IIa) en la FA paroxística (I) y
persistente sintomática(Iia), tras el fracaso de al menos un fármaco antiarrítmico,
pero en la FA paroxística sintomática también como primera línea antes que
fármacos.
En ningún caso sirve para justificar la retirada de la anticoagulación

68. En comparación a Guías Europeas En su algoritmo de elección fármaco
antiarrítmico, destaca dentro del término “Cardiopatía estructural”: Enfermedad
coronaria, IC y la HVI.
El problema HVI, al menos ligera, es frecuente en pacientes hipertensos según las
Guías Europeas, los espesores por encima de 11 mm excluyen el uso de flecainida
(sin que existan estudios que realmente justifiquen dicha recomendación).
Esta Guía excluye HVI en el algoritmo, pero dejando patente que la flecainida no se
recomienda en hipertrofia ventricular izquierda con espesores >15 mm, y que debe
ser usada en combinación con betabloqueantes o calcioantagonistas (por el riesgo
de flutter Ic)
