Who We Are
When I was young, my grandfather was diagnosed with Parkinson’s
Disease. Despite this challenge, he led an active life and used to take
short walks around the block near his home in Green Bay, WI.

One day, Grandpa Charlie went out for a walk and never came home.
He was a missing person for a year until his remains were discovered.

Every time I think of how homecare can help one of my customers, I
think about my grandfather and how proper in-home care from a
good provider could have saved him from this tragedy.

Hello. My name is Michael Phelps and I’m a Networking Executive
with FirstLight HomeCare. I want to share a bit with out about what
our company does to help and to protect great clients like my
Grandpa Charlie. Enjoy this short presentation and learn something

 the supportive care that families need to
    help keep their loved one at home.
How services are provided

Although bundled visits may be provided,
  services are generally provided in hourly
increments, with a XX hour minimum, up to
               24 x 7 coverage
What services are offered
• Companion Care
  – Regular Visits, Conversation & company
  – Medication Reminders
  – Light Housekeeping
  – Laundry Services
  – Mail and Paperwork Organization
  – Family Respite Care
  – Meal Preparation
What services are offered
• Personal Care
  – Bathing & Hygiene
  – Walking and Mobility Assistance
  – Transfer and Posture Assistance
  – Continence and Toileting Care
  – Eating Assistance
  – Oral Hygiene
How services are delivered
• During the initial phone intake process
  – Basic information gathered regarding;
     • Address, phone, directions
     • Patient’s health, functional status
     • Living arrangements
  – Rate or cost range is usually quoted
  – In-home Visit/Assessment is scheduled
How services are delivered
• In-Home Visit
  – FirstLight HomeCare meets with the client,
    family caregiver and other involved parties
  – Completes a basic assessment of the client,
    environment, family support structure, etc.
  – Orients family on what to expect when
    receiving care
  – Verification of date for start of care
How services are delivered
• Office staff chooses and schedules the
  appropriate caregiver
• Staff goes through a mini orientation based
  on client’s particular needs and
  preferences.
• Caregiver is introduced to the Client.
How services are delivered
• Regular Billing Cycle
• Supervision by FirstLight HomeCare staff
  occurs on regular basis
  – Gives clients a chance to reassess their needs
  – Supervision of staff delivering the care
How services are delivered
• 7 Touch Points with Every Client
  – Initial Phone Call
  – In-Home Visit
  – Introduction of the Caregiver
  – Phone Call After first visit
  – Visit after the first week of services
  – Re-Visit after the first month of services
  – Ongoing Monthly/Quarterly visits
How Caregivers are Selected
• Phone screened
• Application process includes:
  –   Pre-Testing (personality assessment)
  –   Interviews
  –   Verifying References
  –   Criminal Background check
  –   Drug Testing
• Orientation process
• On-going Training
Why use FirstLight HomeCare
         over a private hire?
• Private Hire(PH) often not covered by Workers Comp
• PH screening process often incomplete (i.e.
  background check, drug tests, reference checks)
• No formal supervision of PH employee
• No oversight of care delivered
• Often no back up plan
More than just home care

• Transportation services can be provided
• Services can be used by patients having same day
  surgery, who may want the extra security of
  having someone with them for a period of time
• Bed side “sits” can be utilized within the hospital
• Assistance in Assisted/Independent Living Centers
