Hee Ken Wyn, 20 years old, Male.<br />Health Screening:<br />Par-Q: Answered no to all the questions.<br />Health history: Had a car accident last year, but only minor skin injuries.<br />Family health history: None.<br />Current problems: None.<br />Medication: None.<br />Others: None.<br />Exercise History and Attitude:<br />Current: Sedentary lifestyle, just started to workout in the gym for a month. Works out 2-3 time per week.<br />History: Never really exercise for the past 3-4 years after finishing high school.<br />Attitude: Wants to keep fit and reduce body fats in the mean while build some muscles. Wants to look better and stay healthier.<br />Preferences: Jogging, running and weight training.<br />Lifestyle:<br />Occupation: University student.<br />Diet: Consumes around 2500 calories per day. (tightness in hamstrings)<br />Cardio respiratory fitness test:<br />YMCA submaximal step test:<br />Client’s one minute post exercise HR =113 (Rating = 20%, Poor)<br />Muscle endurance testing:<br />Push up test: 10 reps (Rating = very poor)<br />Curl-up test: 40 reps (Rating = below average)<br />
