Este documento presenta una introducción al concepto de Patient Blood Management. Aborda la legislación y recomendaciones internacionales sobre el uso de sangre, incluyendo la necesidad de reducir las transfusiones innecesarias. También discute los desafíos de la sostenibilidad de la donación debido al envejecimiento de los donantes y los riesgos asociados con las transfusiones.

Artículo 4.
Objetivos.
El Banco de Sangre y Tejidos de Navarra tiene como objetivos generales:
a) Velar por el cumplimiento del Real Decreto 1088/2005,…
d) Conseguir la autosuficiencia de sangre y hemoderivados en Navarra.
f) Promover la disminución de la exposición a sangre homóloga y
hemoderivados, actualizando y divulgando recomendaciones de uso e
indicaciones y potenciando otras alternativas a la transfusión, incluyendo todas
las modalidades de autotransfusión.
j) Promover programas de formación del personal del Banco de Sangre y Tejidos
de Navarra, de carácter continuo, que incluirán programas de buenas prácticas.

Artículo 5.
Funciones.
1. Como Centro Comunitario de Transfusión desempeñará las funciones
siguientes:
a) Coordinar las actuaciones de los Centros y Servicios en la Red de transfusión
y de células y tejidos humanos de Navarra.
n) Fomentar programas de autotransfusión, ahorro de sangre y esquemas
mínimos de transfusión en todos los centros hospitalarios de la Comunidad.
p) Implantar programas de formación del personal vinculado a la
hemoterapia

14. To promote improvements in the quality of the clinical
transfusion process, which is defined as:
Transfusion of the right unit of blood to the right
patient at the right time, and in the right condition
and according to appropriate guidelines.
A chain of integrated events that begins with a correct
decision that the patient needs blood and ends with an
assessment of the clinical outcome of the transfusion. Its
goal is to achieve optimal use of blood.

16. 12. Patients should be informed of the known risks and
benefits of blood transfusion and/or alternative therapies
and have the right to accept or refuse the procedure. Any
valid advance directive should be respected.

17. Los estudios indican que los productos sanguíneos se usan a menudo de forma
inapropiada, tanto en los países desarrollados como en los países en desarrollo.
Las transfusiones innecesarias y las prácticas peligrosas y los errores de transfusión
comprometerán gravemente la seguridad exponiendo a los enfermas al riesgo de
sufrir reacciones transfusionales graves e infecciones transmitidas.
El uso innecesario reduce gravemente la disponibilidad de productos sanguíneos
para los pacientes que los necesitan.

It is necessary to reduce the unnecessary transfusions. This can be achieved
through the appropriate clinical use of blood, avoiding the needs for
transfusion and use of alternatives to transfusion.
The commitment of the health authorities, health care providers and
clinicians are important in prevention, early diagnosis and treatment of
diseases/conditions that could lead to the need for blood transfusion.
Blood transfusion is an essential part of modern health care. Used correctly, it can save life
and improve health. However, as with any therapeutic intervention, it may result in acute or
delayed complications and carries the risk of transmission of infectious agents.

21. We define a sustainable blood system as one that:
(1) maintains or improves on current safety levels for blood and blood products,
(2) provides blood for the full range of clinical applications consistent with standard
practice, and
(3) delivers blood in a timely fashion such that patient health and preparedness are
not unduly compromised.

▪▪ Anaemia and/or bleeding is detrimental for patients
▪▪ Transfusions carry a much bigger hazard ratio than previously realized
▪▪ In hemodynamically stable patients, transfusion should not be the default

"Whenever clinically feasible, patients undergoing elective surgery with a high
risk of severe postoperative anaemia should have their haemoglobin level and
iron status tested, preferably at least 28 days before the surgical procedure.
For patients >60 yr old, vitamin B12 and folic acid should also be measured".

Recommendations
• Healthcare pathways should be structured to ensure anaemia screening and correction before
surgery (Grade 2B).
• Patients should be counselled about the relationship between anaemia, morbidity and mortality, and
should be given the opportunity to defer non-urgent surgery until anaemia is investigated and treated
(Grade 1C).
• To avoid causing unnecessary delay to patients, anaemia screening should take place when referral
for surgery is first made, in order to allow investigation and correction if appropriate (Grade 1C).
• Where surgery is urgent, whatever time is available before operation should still be used for
anaemia investigation and treatment initiation (Grade 1C).

104. 7. Se recomienda la detección de la anemia preoperatoria, puesto que está
asociada a un aumento de mortalidad perioperatoria.
Recomendación fuerte +. Nivel de evidencia alto.
8. Se recomienda la realización de una determinación de Hb en pacientes a los
que se va a realizar cirugía electiva, al menos 28 días antes de la cirugía, lo que
permite un tiempo suficiente para la estimulación de la eritropoyesis, si fuera
necesario.
Recomendación fuerte +. Nivel de evidencia moderado.
105. 9. Se sugiere que el nivel de Hb preoperatorio antes de la intervención quirúrgica

117. First Recommendation
The AABB recommends a restrictive RBC transfusion threshold
in which the transfusion is not indicated until the Hb level is
7g/dL for hospitalized adult patients who are hemodynamically
stable, including critically ill patients, rather than a liberal
threshold when the Hb level is 10g/dL (strong recommendation,
moderate quality evidence).
For patients undergoing orthopedic surgery or cardiac surgery
and those with preexisting cardiovascular disease, the AABB
recommends a restrictive RBC transfusion threshold (Hb level
of 8g/dL) (strong recommendation,moderate quality evidence).
The restrictive Hb transfusion threshold of 7 g/dL is likely comparable with 8 g/dL, but
RCT evidence is not available for all patient categories.
