Job Performance Evaluation
Directions: Rate your job performance in your current or past work experience, in volunteer or internship
work, or in another significant task-oriented activity. In the space to the left of each Performance or
Behavioral Category, place the rating code you think most appropriately represents your performance (O =
Outstanding, V = Very good, G = Good, A = Acceptable, U = Unacceptable). In the space below each item,
give one or two examples of your performance. Then highlight the items you rated acceptable or
unacceptable; make these your targets for improvement.
