This document discusses how developing mHealth (mobile health) systems can help improve quality of healthcare, especially in low and middle income countries. It describes D-tree International's work developing decision support tools for frontline health workers using mobile phones. An example from Malawi helped community health workers better identify and treat childhood illnesses like pneumonia through a guided mobile app for assessments, referrals, stock management and reporting. Lessons learned stressed the importance of involving local stakeholders, adapting to existing systems, and ensuring technology supports - rather than drives - health programs.
3. Global Health Challenges
Shortage of almost 4.3 million doctors, midwives,
nurses and support workers worldwide, especially
in low and middle income countries.
Obstacles to Good Healthcare
- Limited number of healthcare providers
- Heavy client load
- Limited training
- High burden of disease
- Limited financial resources
- Paperwork
4. โข Provide decision support tools for use by
frontline health workers
โ Design, Test and Deploy systems
โ Partner with MOH and other health NGOs
โข Focus areas:
โ Maternal and Child Health
โ Chronic Disease
What D-tree does
7. The challenge
โข HSAs do not always follow guidelines
โ 56% asked about 3 danger signs
โ 37% assessed for 4 physical danger signs
โ 57% provided correct referrals and pre-referral
treatment
โ Major gap in identification of fast breathing
โข Stock management
โข Data flows for monthly reporting
8. CCM in Malawi
โข Phase 1 โ cIMCI tool on Nokia phones
โข Phase 2 โ integrated CCM on Android
โข Vaccinations
โข Commodity tracking/reporting
โข Monthly reports
โข Dashboard
โข Supervisory application
โข Phase 2a โ modification to allow for mRDTs
โข Phase 3 โ integration with CBMNH
16. Lessons learned
โข Fast pace of technological change
โข Change Management essential
โข Encourage data driven management
โข Field refinement is critical
โข Users must feel application supports them
โข Donโt let technology push the program
โ Sometimes a phone is just a phone โ and thatโs great!
17. Recommendations
โข Involvement with mHealth working
group in each country
โข Look for existing projects to leverage
โข Adapt each project around the in-country
m/eHealth ecosystem
โข Partner with MOH and consider the entire health
system
โข Plan for integration with HMIS from the start
โข Leverage mFinance, mAgriculture, other
e/mDevelopment initiatives
18. Whatโs next
โข Improved integration of facility and
community
โข Integration of across domains
โข Improved dashboard analytics
โข Continued usage of mobile money
โข Exploration of integration with point of care
diagnostics / unique identifiers
