ā™¦ Colour of vomited blood depends on
1.Concentration of HCL acid
ā™¦ 2.Duration of contact with acid.

Short durationā€”red color
Long durationā€”dark red,brown
black or cofee ground appearance.
Melena
ā™¦ Black tarry stools.
ā™¦ Sticky,loose with characteristic odour.
ā™¦ HCL acts on Hb to produce haemetin giving

black colour.
ā™¦ Usaually follows hemetemesis.
ā™¦ Both suggest proximal source.
ā™¦ Bleeding from esophagus,stomach, small
gut and even ascending colon occasionally.
ā™¦ 60 ml of blood ā€“Single melena stool.
ā™¦ More than this may lead melena upto 7

days.
ā™¦ Occult blood in stools remains positive for
weeks with normal stool colour.
ā™¦ Black or dark gray stools may occur with
use of iron, bismith or licorice.
ā™¦ Occult blood in stoolā€”potentially serious.
Hematochezia
ā™¦ Passage of blood per rectum.
ā™¦ Bleeding distal to ligament of treitz.
ā™¦ Brisk proximal bleedingā€”rapid transit.
ā™¦ Anal or rectal lesions like haemorrhoids or

anal fissure.
ā™¦ Colonic lesions like growth ,IBD ,infections
and angiodysplasia.
Clinical manefestions
ā™¦ Extent of bleeding.
ā™¦ Rate of bleeding.
ā™¦ Comorbid factors.
Extent of bleeding.
ā™¦ Less than 500 ml of blood lossā€”rarely

associated with systemic signs.Exceptions
include elderly and anemic.
ā™¦ Orthostatic hypotentionā€”20% or greater
reduction in blood volume.
ā™¦ Concomitant symptoms include
lightheadedness, syncope, nausea, sweating
and thirst.
ā™¦ Blood loss upto 25-40%--Shock.

ā™¦ Vomiting center in dorsal part of lateral

reticular formation .
ā™¦ CTZ area postrema of floor of fourth
ventricle.
ā™¦ Vomiting center controls and integrates the
actual act of emesis.It receives inputs from
four different sources.

ā™¦ Vestibular system.
ā™¦ Higher centers.
ā™¦ Area postrema.
Causes based on input
ā™¦ Visceral afferent---Mechanical obstruction,

dysmotlity, peritonial irritation ,infection
,hepatobiliary or pancreatic and topical.
ā™¦ CNS disorders(vestibular & higher centers)
middle ear diseases, increased ICP,CNS
infections,psychogenic.
ā™¦ CTZā€”irritation from drugs and systemic
disorders(DKA,uremia,adrenocortial crisis.
Associated features.
ā™¦ Temporal relations like early morning

houres, relation with meals and psychogenic
ā™¦ Associated symptoms are important.
ā™¦ Vertigo and tinnitisā€”Meniers disease.
ā™¦ Long standing history with out significant
sequel point psychogenic.
ā™¦ Localizing symptoms like in abdomen or
CNS.
Character of vomitus.

Large amount of acid.
Absence of acid.
Feculent or putrid odor.

pralysis,cricopharyngeal
achlasia,scleroderma ,achalasia, esophageal
spasm and related motor disorders.

High cricoid cartilage-Difficulty with ejection of bolus.
Takes many swallows to clear.
May be associated with cough

After successful swallow food is held
up.
Tumors,achalasia,esophagitis.
Common causes
ā™¦ Painful mouth or throat:

Recurrent aphthous ulcers,glandular
fever, tonsillitis and quinsy.
ā™¦ Neurological involvement:
Bulbar or pseudobulbar palsy.
ā™¦ Neuromuscular weakness:
Myasthenia gravis,achalsia,
scleroderma.
ā™¦ Obstruction:Cacinoma esophagus,stomach
or extrinsic compression by bronchial Ca.

  • 4. Hemetemesis ā™¦ Vomiting of blood . ā™¦ Proximal to ligament of treitz. ā™¦ GI bleeding below duodenum rarely enters stomach. ā™¦ Colour of vomited blood depends on 1.Concentration of HCL acid
  • 5. ā™¦ 2.Duration of contact with acid. Short durationā€”red color Long durationā€”dark red,brown black or cofee ground appearance.
  • 6. Melena ā™¦ Black tarry stools. ā™¦ Sticky,loose with characteristic odour. ā™¦ HCL acts on Hb to produce haemetin giving black colour. ā™¦ Usaually follows hemetemesis. ā™¦ Both suggest proximal source. ā™¦ Bleeding from esophagus,stomach, small gut and even ascending colon occasionally.
  • 7. ā™¦ 60 ml of blood ā€“Single melena stool. ā™¦ More than this may lead melena upto 7 days. ā™¦ Occult blood in stools remains positive for weeks with normal stool colour. ā™¦ Black or dark gray stools may occur with use of iron, bismith or licorice. ā™¦ Occult blood in stoolā€”potentially serious.
  • 8. Hematochezia ā™¦ Passage of blood per rectum. ā™¦ Bleeding distal to ligament of treitz. ā™¦ Brisk proximal bleedingā€”rapid transit. ā™¦ Anal or rectal lesions like haemorrhoids or anal fissure. ā™¦ Colonic lesions like growth ,IBD ,infections and angiodysplasia.
  • 9. Clinical manefestions ā™¦ Extent of bleeding. ā™¦ Rate of bleeding. ā™¦ Comorbid factors.

Extent of bleeding. ā™¦ Less than 500 ml of blood lossā€”rarely associated with systemic signs.Exceptions include elderly and anemic. ā™¦ Orthostatic hypotentionā€”20% or greater reduction in blood volume. ā™¦ Concomitant symptoms include lightheadedness, syncope, nausea, sweating and thirst. ā™¦ Blood loss upto 25-40%--Shock.

  • 13. Peptic ulcer. ā™¦ Peptic ulcerā€”Break in gastric/duodenal mucosa may extend through muscularis. ā™¦ 5 times more common in duodenum ā™¦ 95% in bulb or pyloric channel. ā™¦ NSAIDS ,H pylori, and acid hypersecretion. ā™¦ History suggestive.

  • 15. Nausea and vomiting ā™¦ Nausea is a desire or feeling. ā™¦ Vomiting is forceful expulsion of gastric contents. ā™¦ Retching is laboured rythmic contraction of respiratory and abdominal musculature precede or accompany vomiting.
  • 16. Control of vomiting ā™¦ 2 distinct medullary centers. ā™¦ Vomiting center in dorsal part of lateral reticular formation . ā™¦ CTZ area postrema of floor of fourth ventricle. ā™¦ Vomiting center controls and integrates the actual act of emesis.It receives inputs from four different sources.
  • 17. Impulses reaching vomiting centre. ā™¦ Afferent vagal. ā™¦ Vestibular system. ā™¦ Higher centers. ā™¦ Area postrema.

Causes based on input ā™¦ Visceral afferent---Mechanical obstruction, dysmotlity, peritonial irritation ,infection ,hepatobiliary or pancreatic and topical. ā™¦ CNS disorders(vestibular & higher centers) middle ear diseases, increased ICP,CNS infections,psychogenic. ā™¦ CTZā€”irritation from drugs and systemic disorders(DKA,uremia,adrenocortial crisis.

Associated features. ā™¦ Temporal relations like early morning houres, relation with meals and psychogenic ā™¦ Associated symptoms are important. ā™¦ Vertigo and tinnitisā€”Meniers disease. ā™¦ Long standing history with out significant sequel point psychogenic. ā™¦ Localizing symptoms like in abdomen or CNS.
  • 20. Character of vomitus. Character of vomitus. Large amount of acid. Absence of acid. Feculent or putrid odor. Presence of blood. EFFECTS OF VOMITING
  • 21. Dyspepsia ā™¦ Upper abdominal or epigastric symptoms including pain ,discomfort ,fulness ,bloating early morning satiety,belching,heart burn, regurgitation and indigestion. ā™¦ PREVALENCE 25% of adult population 3% of OPD patients in west. Vast majority in our OPDs.

Functial or non ulcer dyspepsia ā™¦ Most common. ā™¦ 70 % ------ no organic cause. ā™¦ Young age. ā™¦ Vague symptoms. ā™¦ Anxiety and depression. ā™¦ History of use of psychotropic drugs. ā™¦ Presence of more specific symptoms like weight loss,dysphagia,hematemesis,malena and anemia should be sought.
  • 24. Underlying mechanisms. ā™¦ Increased visceral afferent sensitivity. ā™¦ Delayed gastric emptying. ā™¦ Impaired accomodation of food. ā™¦ Psychosocial stress.

Motor dysphagia ā™¦ Difficulty in initiating of swallowing. ā™¦ Abnormalities of peristalsis or deglutitive inhibition due to diseases of esophageal striated or smooth muscles. ā™¦ Important causes include pharyngeal pralysis,cricopharyngeal achlasia,scleroderma ,achalasia, esophageal spasm and related motor disorders.
  • 36. Symptoms associated with dysphagia ā™¦ PAIN Painlessā€”denervation---tumors. Pain------impaction---intact nerves. Odynophagia:inflammatory state. Peptic ulceration Candidiasis. Herpes simplex.

Takes many swallows to clear. May be associated with cough & aspiration. Tumor,stricture, pharyngeal pouch or reflux

Common causes ā™¦ Painful mouth or throat: Recurrent aphthous ulcers,glandular fever, tonsillitis and quinsy. ā™¦ Neurological involvement: Bulbar or pseudobulbar palsy. ā™¦ Neuromuscular weakness: Myasthenia gravis,achalsia, scleroderma. ā™¦ Obstruction:Cacinoma esophagus,stomach or extrinsic compression by bronchial Ca.
