1. Ulcer associated with Lichen Planus

The erosive form of lichen planus is ulcerated in the central area of the lesion
•
The ulcer is covered by fibrinous plaque or pseudomembrane.
•
Keratotic striae present peripheral to the erosion
•
painful
Histopathology
•
hyperkeratosis
•
interface lymphocytic infiltration
•
basilar vacuolization with apoptosis
Ulcer associated with Pemphigus Vulgaris
• lesions of pemphigus presnt painful ulcer preceded bu bullae .
persistent and progressive
• the bullae rapidly rupture leaving red painful ulcerated base
• positive Niklosky’s sing
Histopathology
• loss of desmosomal attachment result in suprabasal bullae contain
free-floating cells called ( Tzanck cells )
2. Ulcer associated with Mucous
Membrane Pemphigoid
• multiple painful ulcers preceded by vesicles and bullae
• lesions may heal with scar
• positive Niklosky’s sing
• may affect mucous membrane of oral cavity, eyes and genitals
Histopathology
• subepithelial bullae
• no acantholysis
• lymphocytes present and increase with time
Ulcer associated with Bullous
Pemphigoid
• skin disease appear in trunk and extremities with infrequent oral
lesions
• ulcers, preceded by bullae, are located on the attached gingiva most
common site
Histopathology
• subepithelial bullae which the basement membrane is cleaved at the
level of lamina lucida.
