INTRODUCTION
The primary goals of endodontic treatment are straightforward: to
debride and disinfect the root canal space to the greatest possible
extent, and then seal the canals as effectively as possible.
The materials and techniques change somewhat over time, but
not the ultimate goals.
Successful endodontic treatment depends on the restorative treatment
that follows.
The connection between endodontic treatment and restorative
dentistry is well accepted, but the best restorative approaches for
endodontically treated teeth have always been somewhat
controversial.
Why is it important to properly restore the
endodontically treated tooth?
ā€¢ Exposure of gutta-percha to saliva in the pulp chamber results in
migration of bacteria to the apex in a matter of days. Endotoxin
reaches the apex even faster.
ā€¢ The importance of the coronal restoration in successful endodontic
treatment has been shown in several studies.
ā€¢ Delayed restoration has been show to result in lower success rates.
ā€¢ Good restorations + good endodontic treatments resulted in absence of
periapical inflammation in 91.4%
ā€¢ Poor restorations + poor endodontic treatment resulted in absence of
inflammation only in 18% teeth.

doubled the resistance to fracture compared with teeth restored
without a ferrule.

ā€¢ As early as 1728, Pierre Fauchard
described the use of ā€œtenons,ā€ which
were metal posts screwed into the
roots of teeth to retain bridges.
ā€¢ In the mid-1800s, wood replaced metal as the post
material, and the ā€œpivot crown,ā€ a wooden post
fitted to an artificial crown and to the canal of the
root, was popular among dentists. Often, these
wooden posts would absorb fluids and expand,
frequently causing root fractures

Historical perspectives
ā€¢
ā€¢
The Richmond crown was introduced in 1878 and incorporated a
threaded tube in the canal with screw retained crown.
Richmond crown was not post and core system but it is
customized, castable post and crown system as both are single
unit and casted together.
ā€¢ It was later modified to eliminate the
threaded tube and was redesigned as a
1-piece dowel and crown (Hampson EL
et al; 1958, and Demas NC et al; 1957),
which lost its popularity quickly because
they were not practical.
ā€¢ During the 1930s, the custom cast post-and-core was
developed to replace the one-piece post crowns. This
procedure required casting a post-and-core as a
separate component from the crown.2 This 2-step
technique improved marginal adaptation and allowed
for a variation in the path of insertion of the crown.

B) PRE MOLAR
ā€¢ Premolars are usually bulkier than anterior teeth, but
often are single-rooted teeth with relatively small pulp
chambers and are more likely than molars to be
subjected to lateral forces during mastication. For
these reasons, they require posts more often than
molars.
A post may be indicated if a premolar will function as
an abutment for an RPD or suffers from an attachment
loss
The canal anatomy of premolars is delicate and in
order to succeed, the post system chosen must require
minimal reshaping and enlargement of the canal
space.

ā€¢ Parallel metal posts are more retentive than tapered posts and induce less
stress into the root, because they have less wedging effect and are reported
to be less likely to cause root fractures than tapered posts.
ā€¢ In a retrospective study, Sorensen and Martinoff reported a higher success
rate with parallel metal posts than tapered posts.
ā€¢ Tapered posts, on the other hand, require less dentin removal because
most roots are tapered

Most active posts are threaded and intended to engage the walls of
the canal, whereas passive posts are retained primarily by the

ā€¢ Active posts have very limited indications, and are only
recommended when the need for retention is the overriding factor.
ā€¢ One factor that has reduced the use of metal posts is aesthetics. Metal
posts may be visible through translucent all-ceramic restorations, and
even with less translucent restorations may cause the marginal gingiva
to appear dark.
ā€¢ The concerns have led to the development of posts that are white or
translucent. Among the materials used for ā€˜ā€˜aestheticā€™ā€™ posts are
zirconium and other ceramic materials. These posts will work clinically,
but have several disadvantages.
How good are Zirconium posts?
ā€¢ Among rigid posts, zirconium is stiffer and more brittle than metal.
Zirconium posts were shown to cause significantly more root
fractures than fiber posts in vitro.
ā€¢ Zirconium posts cannot be etched, therefore it is not possible to bond
a composite core material to the post, making core retention a
problem.

ā€¢ Retrieval of zirconium and ceramic posts is very difficult if
endodontic retreatment is necessary or if the post fractures.
ā€¢ Some ceramic materials can be removed by grinding away the
remaining post material with a bur, but this is a tedious and risky
procedure.
ā€¢ It is impossible to grind away a zirconium post. In many cases,
excessive removal of dentin is necessary to remove a zirconium
post. For these reasons, ceramic and zirconium posts should be
avoided.

ā€¢ In 1990, Duret et al introduced a new non-metallic material for the
fabrication of posts based on the carbon-fiber reinforcement
principle.
ā€¢ High tensile strength
ā€¢ Modulous of elasticity similar to dentin.
ā€¢ This is due to that carbon fiber posts flex under load (unlike rigid
metal post) and as a result distribute stresses between the post and
the dentin
ā€¢ Besides that, during the preparation of the abutment when a carbon
fiber post was utilized, a dark powder was spread all around the
mouth of the patients.
Quartz (Glass) Fiber Post
ā€¢ In these systems the carbon fibers were replaced with quartz fibers or
glass fibers.
ā€¢ A major advantage of such posts is the possibility of using composite
restorative materials to rebuild missing coronal structure, which
offers interfacial integrity through the use of material of similar elastic
moduli
ā€¢ Advantages of glass fiber post over the conventional metallic post
include enhanced bond strength, formation of a cohesive and strong
foundation with cure composite resin and easy retrieval also.
ā€¢ The first composite reinforced fiber posts were made with
carbon fibers, which were arranged longitudinally and
embedded in an epoxy resin matrix.
ā€¢ The black carbon fibers were rapidly replaced by more
esthetic white and translucent glass and quartz fibers, which
are now the standard components in fiber posts. These posts
arecommonly used in aesthetically demanding areas.
ā€¢ The main advantage of fiber posts is the uniform distribution of forces
in the root, which results in fewer catastrophic failures than occur
with metal posts if an adequate ferrule is present.
ā€¢ A retrospective clinical study of carbon fiber posts and custom cast
posts reported root fractures in 9% of teeth restored with cast posts,
and no root fractures in teeth restored with fiber posts after 4 years

Are fibre posts, the best option???
ā€¢ Although fiber posts offer several advantages, they do have
limitations.
ā€¢ Posts and core foundations are subjected to repeated lateral forces in
clinical function.
ā€¢ Because non-rigid posts have a modulus of elasticity and flexural
strength close to that of dentin, they flex under occlusal load. When
there is an adequate ferrule, the cervical tooth structure itself resists
lateral flexion.
ā€¢ However, in structurally compromised teeth that lack cervical stiffness
from dentin walls and an adequate ferrule, a flexible post may result
in micro-movement of the core and coronal leakage, which in turn
may lead to caries or loss of the core and crown.
ā€¢ Fiber posts were shown to lose flexural strength if they are submitted
to cyclic loading or to thermocycling due to degradation of the matrix
in which the fibers are embedded.
ā€¢ The strength of fiber posts varied between brands, but was directly
related to post diameter and was reduced by thermocycling.

Root length
ā€¢ The length and shape of the remaining root determines the length of
the post (Holmes DC et al, 1996).
ā€¢ It has been demonstrated that the greater the post length, the better
the retention and stress distribution (Standlee JP et al; 1978 & 1972).
ā€¢ It may not always be possible to use a long post, especially when the
remaining root is short or curved. Mattison CD et al (1984) and Kvist T
et al (1989) suggest that is important to preserve 3 to 5 mm of apical
gutta percha to maintain the apical seal.
ā€¢ When the root length is short, the clinician must decide whether to
use a longer post or maintain the recommended apical seal and use a
parallel-sided threaded post.
ā€¢ The use of reinforced composite luting agents may compensate for the
reduced post length (Nissan J et al; 2001).
ā€¢ For molars with short roots the placement of more than one post will
provide additional retention for the core foundation restoration
(Hirshfeld Z et al; 1972)

ā€¢ Tern and Hirshfeld (1973) proportionist approach suggest the post
width should not be greater than one third of the root width at its
narrowest dimension.
ā€¢ Preservationist (Halle EB et al; 1984) proposed that the post should
be surrounded by a minimum of 1mm of sound dentin.
ā€¢ Pilo and Tamse (2000) advocated minimal canal preparation and
maintaining as much residual dentin as possible (conservationist
approach).
ā€¢ The inļ¬‚uence of post width on its retention and fracture resistance
has also been studied.
ā€¢ It was shown that an increase in post width has no signiļ¬cant effect
on its retention.
ā€¢ The tooth restored with larger diameter posts is reported to provide
the least resistance to fracture with a decrease in the width of the
remaining den-tin.
Canal Configuration and Post Adaptability
ā€¢ Canal configuration aids in making a choice between a custom
designed post and a prefabricated post (Ash M jr. et al; 1993, and
Smith TC et al; 1997).
ā€¢ Often a dilemma arises in funnel-shaped canals, whether to use a
parallel sided post and fill the remaining post spaced with cement or
to use a tapered post that closely adapt to the canal wall. A third
option is to use large prefabricated parallel-sided posts.
ā€¢ It has been suggested that if a canal requires extensive preparation a
well adapted cast post and core restoration will be more retentive that
a prefabricated post that does not match the canal shape (Cohen BI et
al; 1996).
ā€¢ A well adapted tapered post provided increased resistance to fracture,
however on fracture it resulted in an extensive loss of tooth structure
(Sorenson JA et al; 1990 and Tjan AH et al; 1985).
ā€¢ Morgano and Milot (1993) stated that 44% of the cast posts were less
than one half to one quarter the length of the clinical crown and that
the failure rate reported was the result of compromised length rather
than post type.
ā€¢ In another investigation they reported custom cast posts success rate
to be more than 90% after 5 years in function.
Coronal Structure
ā€¢ Hoag and Dwyer (1982) determined that the amount of tooth
structure present is more important than the material from which the
post and core is fabricated (amalgam, resin, or cast gold).
ā€¢ The bulk of the tooth above the restorative margin should be at least
1.5mm to 2mm to achieve resistance form (Barkhordar RA et al;
1989).
ā€¢ The use of cast post and cores in restoring endodontically treated
teeth with moderate to severe coronal tooth loss has demonstrated a
success rate of 90.6% after 5 years of service (Bergman B et al; 1989).
ā€¢ When ample coronal dentin remains non-metal posts such as a
carbon fiber posts were deemed successful (Sidoli GE et al; 1997,
Stockton LW et al; 1999.
STRESS
ā€¢ Post and coreā€“restored endodontically treated teeth are subjected to
various types of stresses: compression, tensile, and shear.
ā€¢ Of these stresses, shear stress is most detrimental to the restored
tooth.
ā€¢ Holmes et al have demonstrated that the variation in post dimension
greatly inļ¬‚uences shear stresses.
ā€¢ An increase in the post length with diameter kept to a minimum will
help to reduce shear stresses and preserve tooth structure.
ā€¢ Thus, the vulnerability of the endodontically treated tooth to fracture
is decreased
MATERIAL COMPATIBILITY
ā€¢ Corrosion of the post and fracture of the root has been reported in
the dental literature.
ā€¢ Ideally, post and cores are made of the same alloy. Dissimilar alloys
may create galvanic action, which can lead to corrosion of the less
noble alloy
ā€¢ Corrosion of the post may be initiated because of the access of an
electrolyte to the post surface, through cementum and dentin,
through microleakage around the coronal restoration, through the
accessory canals, which may be opened during post space prepa-
ration, or through undiagnosed root fracture.
ā€¢ These corrosion products cause a change in volume that has been
postulated to cause the root fracture
ā€¢ Of the various alloys used for posts, titanium alloys are the most
corrosion resistant.
ā€¢ Alloys containing brass have lower strength and lower corrosion
resistance and, hence, are less desirable.
ā€¢ Noble metal alloys are corrosion resistant, but their cost is higher.
ā€¢ With the availability of nonmetallic post materials, the corrosion
factor is eliminated
Retention of fibre posts?? Mechanical or
Adhesive??
ā€¢ It has been shown that the retention of fiber posts relies mainly on
mechanical (frictional) retention rather than bonding, similar to metal
posts.
ā€¢ Studies have confirmed the presence of gaps in the interface between
the luting composite resin of the fiber post and the root canal wall,
and that the bond strengths between fiber posts and dentin are low,
about 5 to 6 MPa.

MATERIALS FOR CORE BUILD UP
GOLD
ā€¢ Cast gold offers high strength. It doesn't absorb water and has a
coefficient of thermal expansion (CTE) very close to that of dentin.
CTE for gold is 14 x 10-6; the CTE of dentin is 10.6 x 10-6.
ā€¢ Its resistance to leakage is derived from the luting agent.
ā€¢ Cast gold build-ups require a post for retention and a substantial
degree of coronal destruction to be used. It is expensive and time
consuming.
ā€¢ Where applicable, this is the build-up material of choice. Bullard et al,
1998 showed that a coefficient of thermal expansion close to that of
the tooth reduces leakage, which is an important factor in the success
of ETT
AMALGAM
ā€¢ Amalgam offers strength.
ā€¢ Its CTE is almost double that of dentin (about 22 x 10-6, versus 10.6 x
10-6, and it is relatively stable in the presence of water.
ā€¢ It offers a high level resistance to leakage once it has been in place for
a period of time due to the sealing effects of its corrosion products.
ā€¢ Initial leakage has been shown to be significantly lower with
dispersed phase alloys than with spherical alloys. Bonding of
amalgams is an option, and has the potential to strengthen the tooth
and reduce leakage.
ā€¢ Santos & Meiers, 1994 found no significant strengthening of teeth
with amalgam bond after thermocycling.
ā€¢ Bonilla & White, found short-term increases in strength that
disappeared after 500-day storage or load cycling. As apparently the
bond of amalgam to dentin will ultimately degrade, there is concern
about increased leakage after bond failure.
ā€¢ The bonded surface of the amalgam may be more corrosion resistant
than an un-bonded amalgam, leading to the risk of increased leakage
on a long-term basis.
ā€¢ Given the short-term nature of the bond, the additional time and cost
to bond, and unanswered questions as to the effects on long-term
leakage, amalgam bonding is contraindicated for build-ups.
ā€¢ On posterior teeth with enough pulp chamber depth to obviate the
need for a post (2 to 4 mm), amalgam is the material of choice.
ā€¢ Where a post is required to retain the build-up, amalgam is cheaper
and faster than a cast gold core and often less destructive of tooth
structure.
COMPOSITE RESIN
ā€¢ The coefficient of thermal expansion for most modern self-
polymerizing composite build-up materials is significantly higher than
that of tooth; examples include Ticore by EDS at 34 x 10-6, and Bis-
Core by Bisco at 25 x 10-6.
ā€¢ In addition, composites show significant polymerization shrinkage.
Sakaguchi et al, 1991, (26 ) showed 0.2% post gel contraction. This
shrinkage results in stresses on the bonding systems that may
contribute to long-term bond failure
ā€¢ Also, composite is not dimensionally stable in a wet environment. As it absorbs
water, the core expands and as the composite dries out, the core shrinks.
ā€¢ The major advantage of composite is its ability to be bonded to tooth structure
and then serve as a substrate to which a ceramic crown can be bonded. Its
resistance to leakage is almost totally dependent on the bonding agent, and the
ability of dentin bonding agents to prevent leakage over the long term is
unproven.
ā€¢ Burrow et al, 1996 showed a degradation of dentin bond strength in
vitro over three years almost to the level of an unbounded
restoration.
ā€¢ If this is typical clinically, then the long-term ability of dentin
adhesives to reduce leakage cannot be relied on, which means
composite build-ups must rely on mechanical retention as do
amalgam build-ups.
ā€¢ Its resistance to leakage is almost totally dependent on the luting
agent, and the ability of dentin bonding agents to prevent leakage
over the long term is unproven.
GIC
ā€¢ Glass ionomer materials offer a low level of leakage(18) relatively
weak dentin bond and a low level of mechanical strength (including
silver-reinforced).
ā€¢ They easy of manipulation and offer the appeal of fluoride release to
reduce decay potential, but there is minimal evidence that this has
any clinical significance.
ā€¢ Kovarik et al, fatigue-tested crowns with amalgam, composite and
glass ionomer cores, found that amalgam was significantly stronger
than composite and that glass ionomer had inadequate strength as a
core build-up.
ā€¢ Because of its weak mechanical properties, glass ionomer has little to
offer as a build-up material and should be reserved for limited
applications such as blocking out minor undercuts

What should be the ideal post length?
ā€¢ The retention of fiber posts is directly proportional to the insertion
length.
ā€¢ Braga and colleagues evaluated the force required to remove glass
fiber and metallic cast posts with different lengths.
ā€¢ Irrespective the post type, posts with 10-mm length had higher
retention values than posts with 6-mm length.

ā€¢ Several ā€˜ā€˜rulesā€™ā€™ have been suggested for passively fitting posts ā€“
1. The post length below the alveolar crest should be at least equal to
the length above the alveolar crest.
2. Sorensen and Martinoff reported 97% success if post length at least
equaled the crown height.
3. The post should end halfway between the crestal bone and the root
apex.
4. A post should extend at least apical to the crest of the alveolar bone

ā€¢ Another factor that influences post length is the length of the
remaining apical root canal filling.
ā€¢ Several studies have investigated apical seal following post space
preparation and have reported that 3 to 5 mm of gutta-percha is the
minimum recommended, and longer is better.

Is there benefit to placing a post after endodontic
treatment of a tooth with a crown?
ā€¢ There is growing evidence that the insertion of a fiber post can also
increase fracture resistance of teeth with crowns.
ā€¢ An in vitro study has shown that placement of fiber posts can improve
fracture resistance in maxillary premolars under full-coverage crowns.

ā€¢ The use of fiber posts in endodontically treated maxillary incisors with
different types of full-coverage crowns increased their resistance to
fracture and improved the prognosis in case of fracture.

LIGHT-TRANSMITTING FIBER POSTS
ā€¢ One study evaluated the influence of fiber-post translucency on the
degree of conversion of a dual-cure composite. Low degrees of
conversion were found for the medium and deep depths.
ā€¢ Another study showed that even without a post, the luminous
intensity inside the canal decreased to levels that are insufficient for
polymerization, especially in the apical third.

ā€¢ Another in vitro study measured light transmission through 4
different posts of a standard length of 10 mm.
ā€¢ All posts evaluated showed some light transmission capacity, but
with values lower than 40% of incident light. One post
demonstrated less than 1% light transmission.
ā€¢ Based on these findings, the use of light-cured resin cements for
post placement cannot be recommended. The benefits of light-
transmitting posts are unclear.

conditioning the dentin for post placement.

combined with ultrasonics are reported to be an effective

ā€¢ The primary disadvantages of this cement are solubility in oral fluids,
especially in the presence of acid,
ā€¢ and lack of true adhesion.
Polycarboxylate cements
ā€¢ Polycarboxylate cements have been shown to undergo plastic
deformation after cyclic loading which is a major disadvantage
although it does bond chemically to the tooth.
Glass ionomer cement
ā€¢ Glass ionomer cement releases fluoride but the ability of this leached
fluoride to provide long-term protection against dental caries in dentin has
never been proven.
ā€¢ A primary disadvantage of conventional glass ionomer cement is its setting
reaction. This cement does not reach its maximal strength for many days .
ā€¢ Therefore, any recontouring of the core with a dental handpiece on the
day of cementation of the post can potentially disturb the set of the
cement and weaken the immature cement film

RMGIC and post fracture??
ā€¢ Resin-modified glass ionomer cement contains hydrophilic resins that
slowly imbibe water, causing the cement film to gradually expand.
ā€¢ One in vitro study suggested that this expansion of the cement could
fracture allceramic crowns relatively soon after cementation .
J Dent 1999;27:175ā€“81.
ā€¢ However, a more recent study found no potential for fracture of all-
ceramic crowns cemented with resin-modified glass ionomer cement
after 60 weeks of storage in vitro in 100%humidity.
ā€¢ Nevertheless, this cement should be used with caution. If this cement
can expand and cause fracture of all-ceramic crowns, it could possibly
cause vertical fracture of the roots if used to cement posts

Problems and complications with posts
The most common complications reported in the literature for posts
are
ā€¢ dislodgment of the posts,
ā€¢ fracture of the roots, and
ā€¢ dental caries
ā€¢ Studies have shown that shorter posts are less retentive than longer
posts and are more likely to concentrate stresses in the root,
increasing the potential for post dislodgement or root fracture
J Endod 2001;27:46ā€“8.

Caries and the postā€¦
ā€¢ Although there are many factors that can predispose to dental caries,
a flexible post or flexible core can generate stresses within the
cement seal of the artificial crown, producing leakage at the margin of
the crown and eventual caries.
Antirotational properties in post
ā€¢ Posts are subjected clinically to torsional or rotational forces
produced by functional tooth cant acts, so various antirotational locks
ā€˜have been advocated.
ā€¢ These locks included a shallow parallel-sided slot across the root face,
an auxiliary pin,ā€ coronal flares, keyways or notches prepared in the
coronal surface of the post hole, and an extracoronal collar or
diaphragm.
Keyway
ā€¢ Keyway is prepared in the greatest bulk of dentin to a depth of 3 mm
using a tungsten carbide 170 bur.
Coronal flare
ā€¢ The coronal 3 mm of the post hole is prepared by leaving a minimum
of 1.5 mm of circumferential dentin in the mesiodistal plane and 2
mm in the faciolingual plane.
Auxiliary Pin
ā€¢ pin hole 3 mm in depth was prepared in the greatest bulk of dentin
using the Parapost pin jig and twist drill.
Cervical collar
ā€¢ 45-degree bevel was prepared at the periphery of the root face using
a tungsten carbide chamfer bur.
TAPERED
The preparation is flared from the base of the post hole coronally to
leave the same surface post hole dimensions as in flared and an
ovalshaped cross section.

  • 4. INTRODUCTION The primary goals of endodontic treatment are straightforward: to debride and disinfect the root canal space to the greatest possible extent, and then seal the canals as effectively as possible. The materials and techniques change somewhat over time, but not the ultimate goals.
  • 5. Successful endodontic treatment depends on the restorative treatment that follows. The connection between endodontic treatment and restorative dentistry is well accepted, but the best restorative approaches for endodontically treated teeth have always been somewhat controversial.
  • 6. Why is it important to properly restore the endodontically treated tooth? ā€¢ Exposure of gutta-percha to saliva in the pulp chamber results in migration of bacteria to the apex in a matter of days. Endotoxin reaches the apex even faster. ā€¢ The importance of the coronal restoration in successful endodontic treatment has been shown in several studies. ā€¢ Delayed restoration has been show to result in lower success rates.

  • 14. ā€¢ As early as 1728, Pierre Fauchard described the use of ā€œtenons,ā€ which were metal posts screwed into the roots of teeth to retain bridges. ā€¢ In the mid-1800s, wood replaced metal as the post material, and the ā€œpivot crown,ā€ a wooden post fitted to an artificial crown and to the canal of the root, was popular among dentists. Often, these wooden posts would absorb fluids and expand, frequently causing root fractures ā€¢ Historical perspectives
  • 15. ā€¢ ā€¢ The Richmond crown was introduced in 1878 and incorporated a threaded tube in the canal with screw retained crown. Richmond crown was not post and core system but it is customized, castable post and crown system as both are single unit and casted together.
  • 16. ā€¢ It was later modified to eliminate the threaded tube and was redesigned as a 1-piece dowel and crown (Hampson EL et al; 1958, and Demas NC et al; 1957), which lost its popularity quickly because they were not practical. ā€¢ During the 1930s, the custom cast post-and-core was developed to replace the one-piece post crowns. This procedure required casting a post-and-core as a separate component from the crown.2 This 2-step technique improved marginal adaptation and allowed for a variation in the path of insertion of the crown.

  • 24. ā€¢ Parallel metal posts are more retentive than tapered posts and induce less stress into the root, because they have less wedging effect and are reported to be less likely to cause root fractures than tapered posts. ā€¢ In a retrospective study, Sorensen and Martinoff reported a higher success rate with parallel metal posts than tapered posts. ā€¢ Tapered posts, on the other hand, require less dentin removal because most roots are tapered
  • 25. ā€¢ Prefabricated posts can be further divided in active or passive posts. Most active posts are threaded and intended to engage the walls of the canal, whereas passive posts are retained primarily by the frictional retention of the luting agent. ā€¢ Active posts are more retentive than passive posts, but introduce more stress into the root than passive posts. ā€¢ Active posts have very limited indications, and are only recommended when the need for retention is the overriding factor.
  • 26. ā€¢ One factor that has reduced the use of metal posts is aesthetics. Metal posts may be visible through translucent all-ceramic restorations, and even with less translucent restorations may cause the marginal gingiva to appear dark. ā€¢ The concerns have led to the development of posts that are white or translucent. Among the materials used for ā€˜ā€˜aestheticā€™ā€™ posts are zirconium and other ceramic materials. These posts will work clinically, but have several disadvantages.
  • 27. How good are Zirconium posts? ā€¢ Among rigid posts, zirconium is stiffer and more brittle than metal. Zirconium posts were shown to cause significantly more root fractures than fiber posts in vitro. ā€¢ Zirconium posts cannot be etched, therefore it is not possible to bond a composite core material to the post, making core retention a problem. Retreatment with Zirconium and Ceramic posts ā€¢ Retrieval of zirconium and ceramic posts is very difficult if endodontic retreatment is necessary or if the post fractures. ā€¢ Some ceramic materials can be removed by grinding away the remaining post material with a bur, but this is a tedious and risky procedure. ā€¢ It is impossible to grind away a zirconium post. In many cases, excessive removal of dentin is necessary to remove a zirconium post. For these reasons, ceramic and zirconium posts should be avoided.

  • 36. ā€¢ Advantages of glass fiber post over the conventional metallic post include enhanced bond strength, formation of a cohesive and strong foundation with cure composite resin and easy retrieval also.
  • 37. ā€¢ The first composite reinforced fiber posts were made with carbon fibers, which were arranged longitudinally and embedded in an epoxy resin matrix. ā€¢ The black carbon fibers were rapidly replaced by more esthetic white and translucent glass and quartz fibers, which are now the standard components in fiber posts. These posts arecommonly used in aesthetically demanding areas.
  • 38. ā€¢ The main advantage of fiber posts is the uniform distribution of forces in the root, which results in fewer catastrophic failures than occur with metal posts if an adequate ferrule is present. ā€¢ A retrospective clinical study of carbon fiber posts and custom cast posts reported root fractures in 9% of teeth restored with cast posts, and no root fractures in teeth restored with fiber posts after 4 years Quintessence Int 2007; 38(9):733ā€“43. Are fibre posts, the best option??? ā€¢ Although fiber posts offer several advantages, they do have limitations. ā€¢ Posts and core foundations are subjected to repeated lateral forces in clinical function. ā€¢ Because non-rigid posts have a modulus of elasticity and flexural strength close to that of dentin, they flex under occlusal load. When there is an adequate ferrule, the cervical tooth structure itself resists lateral flexion.
  • 41. ā€¢ However, in structurally compromised teeth that lack cervical stiffness from dentin walls and an adequate ferrule, a flexible post may result in micro-movement of the core and coronal leakage, which in turn may lead to caries or loss of the core and crown.
  • 42. ā€¢ Fiber posts were shown to lose flexural strength if they are submitted to cyclic loading or to thermocycling due to degradation of the matrix in which the fibers are embedded. ā€¢ The strength of fiber posts varied between brands, but was directly related to post diameter and was reduced by thermocycling. Root length ā€¢ The length and shape of the remaining root determines the length of the post (Holmes DC et al, 1996). ā€¢ It has been demonstrated that the greater the post length, the better the retention and stress distribution (Standlee JP et al; 1978 & 1972). ā€¢ It may not always be possible to use a long post, especially when the remaining root is short or curved. Mattison CD et al (1984) and Kvist T et al (1989) suggest that is important to preserve 3 to 5 mm of apical gutta percha to maintain the apical seal.
  • 48. ā€¢ When the root length is short, the clinician must decide whether to use a longer post or maintain the recommended apical seal and use a parallel-sided threaded post. ā€¢ The use of reinforced composite luting agents may compensate for the reduced post length (Nissan J et al; 2001). ā€¢ For molars with short roots the placement of more than one post will provide additional retention for the core foundation restoration (Hirshfeld Z et al; 1972)

  • 52. ā€¢ Tern and Hirshfeld (1973) proportionist approach suggest the post width should not be greater than one third of the root width at its narrowest dimension. ā€¢ Preservationist (Halle EB et al; 1984) proposed that the post should be surrounded by a minimum of 1mm of sound dentin. ā€¢ Pilo and Tamse (2000) advocated minimal canal preparation and maintaining as much residual dentin as possible (conservationist approach).
  • 53. ā€¢ The inļ¬‚uence of post width on its retention and fracture resistance has also been studied. ā€¢ It was shown that an increase in post width has no signiļ¬cant effect on its retention. ā€¢ The tooth restored with larger diameter posts is reported to provide the least resistance to fracture with a decrease in the width of the remaining den-tin.
  • 54. Canal Configuration and Post Adaptability ā€¢ Canal configuration aids in making a choice between a custom designed post and a prefabricated post (Ash M jr. et al; 1993, and Smith TC et al; 1997). ā€¢ Often a dilemma arises in funnel-shaped canals, whether to use a parallel sided post and fill the remaining post spaced with cement or to use a tapered post that closely adapt to the canal wall. A third option is to use large prefabricated parallel-sided posts.
  • 55. ā€¢ It has been suggested that if a canal requires extensive preparation a well adapted cast post and core restoration will be more retentive that a prefabricated post that does not match the canal shape (Cohen BI et al; 1996). ā€¢ A well adapted tapered post provided increased resistance to fracture, however on fracture it resulted in an extensive loss of tooth structure (Sorenson JA et al; 1990 and Tjan AH et al; 1985).
  • 56. ā€¢ Morgano and Milot (1993) stated that 44% of the cast posts were less than one half to one quarter the length of the clinical crown and that the failure rate reported was the result of compromised length rather than post type. ā€¢ In another investigation they reported custom cast posts success rate to be more than 90% after 5 years in function.

Coronal Structure ā€¢ Hoag and Dwyer (1982) determined that the amount of tooth structure present is more important than the material from which the post and core is fabricated (amalgam, resin, or cast gold). ā€¢ The bulk of the tooth above the restorative margin should be at least 1.5mm to 2mm to achieve resistance form (Barkhordar RA et al; 1989).
  • 58. ā€¢ The use of cast post and cores in restoring endodontically treated teeth with moderate to severe coronal tooth loss has demonstrated a success rate of 90.6% after 5 years of service (Bergman B et al; 1989). ā€¢ When ample coronal dentin remains non-metal posts such as a carbon fiber posts were deemed successful (Sidoli GE et al; 1997, Stockton LW et al; 1999.

STRESS ā€¢ Post and coreā€“restored endodontically treated teeth are subjected to various types of stresses: compression, tensile, and shear. ā€¢ Of these stresses, shear stress is most detrimental to the restored tooth. ā€¢ Holmes et al have demonstrated that the variation in post dimension greatly inļ¬‚uences shear stresses.
  • 60. ā€¢ An increase in the post length with diameter kept to a minimum will help to reduce shear stresses and preserve tooth structure. ā€¢ Thus, the vulnerability of the endodontically treated tooth to fracture is decreased
  • 61. MATERIAL COMPATIBILITY ā€¢ Corrosion of the post and fracture of the root has been reported in the dental literature. ā€¢ Ideally, post and cores are made of the same alloy. Dissimilar alloys may create galvanic action, which can lead to corrosion of the less noble alloy
  • 62. ā€¢ Corrosion of the post may be initiated because of the access of an electrolyte to the post surface, through cementum and dentin, through microleakage around the coronal restoration, through the accessory canals, which may be opened during post space prepa- ration, or through undiagnosed root fracture. ā€¢ These corrosion products cause a change in volume that has been postulated to cause the root fracture

  • 65. MATERIALS FOR CORE BUILD UP
  • 66. GOLD ā€¢ Cast gold offers high strength. It doesn't absorb water and has a coefficient of thermal expansion (CTE) very close to that of dentin. CTE for gold is 14 x 10-6; the CTE of dentin is 10.6 x 10-6. ā€¢ Its resistance to leakage is derived from the luting agent.
  • 67. ā€¢ Cast gold build-ups require a post for retention and a substantial degree of coronal destruction to be used. It is expensive and time consuming. ā€¢ Where applicable, this is the build-up material of choice. Bullard et al, 1998 showed that a coefficient of thermal expansion close to that of the tooth reduces leakage, which is an important factor in the success of ETT
  • 68. AMALGAM ā€¢ Amalgam offers strength. ā€¢ Its CTE is almost double that of dentin (about 22 x 10-6, versus 10.6 x 10-6, and it is relatively stable in the presence of water. ā€¢ It offers a high level resistance to leakage once it has been in place for a period of time due to the sealing effects of its corrosion products. ā€¢ Initial leakage has been shown to be significantly lower with dispersed phase alloys than with spherical alloys. Bonding of amalgams is an option, and has the potential to strengthen the tooth and reduce leakage.
  • 69. ā€¢ Santos & Meiers, 1994 found no significant strengthening of teeth with amalgam bond after thermocycling. ā€¢ Bonilla & White, found short-term increases in strength that disappeared after 500-day storage or load cycling. As apparently the bond of amalgam to dentin will ultimately degrade, there is concern about increased leakage after bond failure.
  • 70. ā€¢ The bonded surface of the amalgam may be more corrosion resistant than an un-bonded amalgam, leading to the risk of increased leakage on a long-term basis. ā€¢ Given the short-term nature of the bond, the additional time and cost to bond, and unanswered questions as to the effects on long-term leakage, amalgam bonding is contraindicated for build-ups. ā€¢ On posterior teeth with enough pulp chamber depth to obviate the need for a post (2 to 4 mm), amalgam is the material of choice.
  • 71. ā€¢ Where a post is required to retain the build-up, amalgam is cheaper and faster than a cast gold core and often less destructive of tooth structure.
  • 72. COMPOSITE RESIN ā€¢ The coefficient of thermal expansion for most modern self- polymerizing composite build-up materials is significantly higher than that of tooth; examples include Ticore by EDS at 34 x 10-6, and Bis- Core by Bisco at 25 x 10-6. ā€¢ In addition, composites show significant polymerization shrinkage. Sakaguchi et al, 1991, (26 ) showed 0.2% post gel contraction. This shrinkage results in stresses on the bonding systems that may contribute to long-term bond failure
  • 73. ā€¢ Also, composite is not dimensionally stable in a wet environment. As it absorbs water, the core expands and as the composite dries out, the core shrinks. ā€¢ The major advantage of composite is its ability to be bonded to tooth structure and then serve as a substrate to which a ceramic crown can be bonded. Its resistance to leakage is almost totally dependent on the bonding agent, and the ability of dentin bonding agents to prevent leakage over the long term is unproven.
  • 74. ā€¢ Burrow et al, 1996 showed a degradation of dentin bond strength in vitro over three years almost to the level of an unbounded restoration. ā€¢ If this is typical clinically, then the long-term ability of dentin adhesives to reduce leakage cannot be relied on, which means composite build-ups must rely on mechanical retention as do amalgam build-ups. ā€¢ Its resistance to leakage is almost totally dependent on the luting agent, and the ability of dentin bonding agents to prevent leakage over the long term is unproven.

GIC ā€¢ Glass ionomer materials offer a low level of leakage(18) relatively weak dentin bond and a low level of mechanical strength (including silver-reinforced). ā€¢ They easy of manipulation and offer the appeal of fluoride release to reduce decay potential, but there is minimal evidence that this has any clinical significance.

Low degrees of conversion were found for the medium and deep depths. ā€¢ Another study showed that even without a post, the luminous intensity inside the canal decreased to levels that are insufficient for polymerization, especially in the apical third.
