Eczema Prevalence in the United States
A substantial proportion of the US population has symptoms of eczema; 31.6 million with eczema, and at least 17.8 million with moderate to
severe eczema or atopic dermatitis. The prevalence of childhood eczema/atopic dermatitis in the US is 10.7% overall and as high as 18.1% in
individual states and 21% across various countries. Approximately one out of every three children with eczema/atopic dermatitis has moderate
to severe disease. A recent study found that the prevalence of eczema in adults could be as high as 10.2%, which suggests that most children
with eczema/atopic dermatitis continue to be affected even in adulthood. Three percent of US adults have moderate to severe eczema/atopic
dermatitis requiring systemic therapy. These numbers are much higher than for psoriasis, a disease that now has many good-targeted
treatments for moderate to severe patients. Yet, there are still very large unmet needs for the treatment of patients with eczema / atopic
dermatitis.
This discrepancy between the higher prevalence and lack of good treatments for moderate to severe eczema stems, in part, from an
incomplete understanding of the disease. For many years, there was an active debate whether the primary defect is in the skin barrier or an
immune abnormality.

THE VARIOUS LAYERS OF SKIN
▪ Epidermis - The upper or outer layer of the two main layers of cells that make up
the skin. The epidermis is mostly made up of flat, scale-like cells called squamous
cells. Under the squamous cells are round cells called basal cells. The deepest part
of the epidermis also contains melanocytes. These cells produce melanin, which
gives the skin its color.
▪ Stratum Grandulosum is one of the layers of the epidermis, situated just below the
stratum corneum except in the thick skin of the palms of the hands and the soles of
the feet, where it lies just under the stratum lucidum. The stratum granulosum
contains visible granules in the cytoplasm of its cells, which die, become keratinized,
move to the surface, and flake away.
▪ Stratum corneum - The stratum corneum refers to the outermost layer of the
epidermis, which is itself the outer layer of the skin. The full name of the stratum
corneum of the skin is the stratum corneum epidermidis. The other stratum
corneum, the stratum corneum unguis, is the horny compact outer layer of the nail.
In Latin "stratum" = "layer" and "corneum" = "horny" so stratum corneum = horny
layer.
▪ Atopic Dermatitis - A skin disease characterized by areas of severe itching, redness,
scaling, and loss of the surface of the skin. Atopic dermatitis is the most common of
the many types of eczema. Atopic dermatitis is frequently associated with other
allergic disorders, especially asthma and hay fever. A defect of the immune system
within the skin has been detected in patients who have atopic dermatitis, but the
reason for the defect is unknown.

PATHOGENESIS OF ATOPIC DERMATITIS
▪ Eczema - any superficial inflammatory process involving primarily the epidermis,
marked early by redness, itching, minute papules and vesicles, weeping, oozing, and
crusting, and later by scaling, lichenification, and often pigmentation. It’s a common
allergic reaction in children but it also occurs in adults, usually in a more severe
form. Childhood eczema often begins in infancy, the rash appearing on the face,
neck, and folds of elbows and knees. It may disappear by itself when an offending
food is removed from the diet, or it may become more extensive and in some
instances cover the entire surface of the body. Severe eczema can be complicated
by skin infections. Childhood eczema may persist for several years or return after
the child is older. Persons suffering from childhood eczema may develop another
allergic condition later, most often hay fever or asthma.

RESIDENT SKIN FLORA
The resident skin flora is the physiological skin flora, also
known as colonizing flora. It comprises bacteria such as
Staphylococcus epidermidis, propioni and corynebacteria,
which do not have a pathogenic effect on the skin. The
microorganisms have important functions: their metabolism
inhibits the growth of non-resident bacteria and fungi, and
maintain the balance between the colonizing microorganisms.
Around 80 per cent of the resident skin flora is on the surface
of the stratum corneum, the outermost layer of the epidermis.
Particularly high concentrations of microorganisms are found in
body regions rich in sebaceous glands, e.g. scalp, forehead,
axillae, front of the chest, and the area between the shoulder
blades. 20 per cent of the resident microorganisms are found in
the sebaceous glands (up to 0.3 mm deep). On non-intact skin,
such as wounds, the microorganisms of the resident skin flora
may trigger infections.

Staphylococcus aureus is a gram-positive, round-shaped bacterium that is a
member of the Firmicutes, and is frequently found in the nose, respiratory tract,
and on the skin. It is often positive for catalase and nitrate reduction and is a
facultative anaerobe that can grow without the need for oxygen. Although S.
aureus is not always pathogenic, it is a common cause of skin infections such as
a skin abscess, respiratory infections such as sinusitis, and food poisoning.
Pathogenic strains often promote infections by producing virulence factors such
as potent protein toxins, and the expression of cell-surface proteins that bind
and inactivate antibodies. The emergence of antibiotic-resistant strains of S.
aureus such as methicillin-resistant S. aureus (MRSA) is a worldwide problem in
clinical medicine. Despite much research and development there is no approved
vaccine for S. aureus.
An estimated 20% of the human population are long-term carriers of S. aureus
which can be found as part of the normal skin flora, in the nostrils, and as a
normal inhabitant of the lower reproductive tract of women. S. aureus can
cause a range of illnesses, from minor skin infections, such as pimples, impetigo,
boils, cellulitis, folliculitis, carbuncles, scalded skin syndrome, and abscesses, to
life-threatening diseases such as pneumonia, meningitis, osteomyelitis,
endocarditis, toxic shock syndrome, bacteremia, and sepsis. While S. aureus
usually acts as a commensal bacterium, asymptomatically colonizing about 30%
of the human population, it can sometimes cause disease. S. aureus is extremely
prevalent in persons with atopic dermatitis. S. aureus infections can spread
through contact with pus from an infected wound, skin-to-skin contact with an
infected person.

How an Alkaline Diet Works
Here’s some background on acid/alkalinity in the human
diet, plus key points about how alkaline diets can be
beneficial:
•Researchers believe that when it comes to the total acid load
of the human diet, “there have been considerable changes from
hunter gather civilizations to the present.” Following the
agricultural revolution and then mass industrialization of our
food supply over the last 200 years, the food we eat has
significantly less potassium, magnesium and chloride, along
with more sodium, compared to diets of the past.
•Normally, the kidneys maintain our electrolyte levels (those of
calcium, magnesium, potassium and sodium). When we’re
exposed to overly acidic substances, these electrolytes are
used to combat acidity.
•According to the Journal of Environmental Health review
mentioned earlier, the ratio of potassium to sodium in most
people’s diets has changed dramatically. Potassium used to
outnumber sodium by 10:1, however now the ratio has dropped
to 1:3. People eating a “Standard American Diet” now
consume three times as much sodium as potassium on
average!
•Many children and adults today consume a high-sodium
diet that’s very low in not only magnesium and potassium, but
also antioxidants, fiber and essential vitamins. On top of that,
the typical Western diet is high in refined fats, simple sugars,
sodium and chloride.
•All of these changes to the human diet have resulted in
increased “metabolic acidosis.” In other words, the pH levels of
many people’s bodies are no longer optimal. On top of this,
many are suffering from low nutrient intake and problems such
as potassium and magnesium deficiency.
•This accelerates the aging process, causes gradual loss of
organ functions, and degenerates tissue and bone mass. High
degrees of acidity force our bodies to rob minerals from the
bones, cells, organs and tissues.
Alkaline Diet Benefits
1. Protects Bone Density and Muscle Mass
Your intake of minerals plays an important role in the
development and maintenance of bone structures. (5) Research
shows that the more alkalizing fruits and vegetables someone
eats, the better protection that person might have from
experiencing decreased bone strength and muscle wasting as
they age, known as sarcopenia.
An alkaline diet can help balance ratios of minerals that are
important for building bones and maintaining lean muscle mass,
including calcium, magnesium and phosphate. Alkaline diets
also help improve production of growth hormones and vitamin D
absorption, which further protects bones in addition to mitigating
many other chronic diseases.

One of the anti-aging effects of an alkaline diet is that it
decreases inflammation and causes an increase in growth

Studies have found a connection between an alkaline diet and
reduced levels of chronic pain. Chronic acidosis has been found
to contribute to chronic back pain, headaches, muscle spasms,
menstrual symptoms, inflammation and joint pain.
One study conducted by the Society for Minerals and Trace
Elements in Germany found that when patients with chronic
back pain were given an alkaline supplement daily for four
weeks, 76 of 82 patients reported significant decreases in pain

An increase in magnesium is required for the function of

Antioxidant sources, like antioxidant foods, herbs, spices and teas, reduce the effects of free radicals, also called oxidative
damage/stress, which plays a major role in disease formation. The leading health problems facing us today — including
conditions like heart disease, cancer and dementia — have been linked to increased levels of oxidative damage and
inflammation. In simplest terms, oxidation is a chemical reaction that can produce free radicals, leading to other chemical

VITAMIN D SUPPLEMENTS
It’s well-documented that Vitamin D is
essential for the proper synthesis of
calcium, and it’s been shown to greatly
reduce fracture risk in two ways. First, it
helps with the formation of stronger bones;
second, Vitamin D actually helps improve
balance and prevent falls by enhancing
muscle contraction.
For rejuvenating your bones, it’s vitally
important that you avoid Vitamin D2, and
choose a bioavailable form of this important
vitamin to avoid deficiency.
If you live in an area where it’s difficult to
get sun exposure or if you can’t spend 20 to
30 minutes outdoors a few times a week,
then you should be taking at a minimum
600 IU of Vitamin D3 or cholecalciferol daily.
But to achieve and maintain desirable
levels, I recommend taking 2,000 IU once a
day. Don’t worry that you may have too
much Vitamin D if you also sunbathe.
Remember that there’s no risk of
overdosing from the sun.

There are several biological mechanisms that contribute to the
superior absorbability and efficacy of Vitamin D3. In the liver,
thanks to a particular hepatic enzyme, Vitamin D3 is more readily
metabolized into a bioactive form of D that is easily converted to
its hormone form in the kidneys. It takes much longer to make
this hepatic conversion with Vitamin D2.
In fact, D2 and D3 are metabolized so differently that they result
in “the production of unique biologically active metabolites.”
Clearly, these forms of Vitamin D are simply not the same. Hence
the study’s conclusion that ergocalciferol should not be used as a
supplement.
You may be wondering how Vitamin D2 supplements came to be
the mainstream protocol in the first place. We need to go back
almost 100 years to find the answer.
Back in the early 1920s, scientists discovered that exposing foods
to ultraviolet light produced Vitamin D2. Thus the manufacture of
this vitamin was born, and the process was sold to what would
become Big Pharma. Drug companies then produced and sold
this medicinal form of Vitamin D to the public and to doctors.
Less than 10 years later, around 1930, scientists noticed that
Vitamin D2 did not seem as effective in the treatment of rickets
in children (the primary use of the vitamin in those days) as the
Vitamin D in cod liver oil. Subsequent studies showed mixed
results, and not surprisingly, in 1949 the World Health
Organization declared that differences between the two forms
are “minimal”. But recent research clearly shows that they could
not be more wrong!

Whey protein can be bought as a concentrate (which contains small amounts of lactose – the milk sugar – and fat), whey isolate (a form which is
processed to remove the lactose and fat) or whey hydrolysate (where the protein chains are broken down into smaller units).
What do we know about whey protein and psoriasis?
Whey contains a number of smaller proteins that may influence psoriasis via their effects on the immune system and the growth of skin cells.
For example, lactoferrin (one of the small proteins found in whey) has anti-bacterial properties, and has been shown to exert anti-inflammatory
effects in humans and in animal studies. Whey protein extracts have also shown to slow the release of interleukin (IL)-2, a signaling molecule
which stimulates the overgrowth of skin cells in psoriasis. Some of the proteins in whey also exert positive effects via antioxidant activity. People
with psoriasis tend to have low levels of antioxidants together with high levels of oxidative stress, which may contribute to inflammation. Whey
is rich in cysteine, a small protein which is converted to a potent antioxidant known as glutathione. Studies show whey protein supplements can
enhance antioxidant defenses in a range of diseases 12 and potentially in psoriasis.
What does the science say?
Studies into the effects of whey protein in psoriasis have looked at whey protein extracts, whey protein isolate and topical application of the
proteins found in whey.
Whey protein extract
Several studies have shown the benefits of a whey protein extract (known as Dermylex) in the treatment of psoriasis. In one double blind
placebo-controlled study (considered the most rigorous type of clinical study there is), patients with mild-to moderate psoriasis were
randomized to receive either a daily dose of 800 milligrams of Dermylex or a placebo (dummy) supplement for a period of 56 days.
During the study period patients in the Dermylex group experienced a significant improvement in psoriasis severity, itching and self-reported
quality of life scores (which assess how much symptoms have affected daily life) when compared to the placebo group.
Similar to these findings, two earlier studies found that both 5 and 10 grams of Dermylex taken as a powder mixed into juice or food improved
psoriasis severity, although the larger doses were not found to be any more effective than the 800-milligram (0.8 gram) dose.
Whey protein isolate
In another small study from Canada, seven patients with moderate to severe psoriasis vulgaris consumed a daily dose of 20 grams of whey
protein isolate together with their usual treatments, which included topical steroids or UVB light therapy. After three months, the five patients
who had completed the study experienced a significant improvement in psoriasis severity, with no other changes in their treatment.

the skin and during washing, the crucial skin lipids dissolve and are washed out of the skin,

A skin care system researched and developed by the international dermal institute. A unique
anhydrous (waterless) moisturizer that melts gently into sensitized skin to help soothe irritation

Formulated with Dermalogica's exclusive anti-ozonate complex to help shield skin from reactive

PH BALANCED BODY WASH AND LOTION
The protective acid mantle or hydrolipid film which covers
skin’s surface is slightly acidic. The average pH of around
5.4-5.9 is important for the activation of protective enzymes,
which regulate bacterial growth and support desquamation,
or skin shedding. In sensitive skin, the protective mantle
loses its acidity, weakening skin’s natural boundary
functions. pH5 Citrate Buffer helps restore and stabilize
skin’s natural pH, encouraging enzyme activation and
protection against irritations.
Dexpanthenol, which derives from panthotenic acid (also
known as Vitamin B5), is an extremely effective moisturizer,
improving hydration of the outer most layer of the skin, to
help reducing transepidermal water loss and maintaining
skin softness and elasticity.
Glycerine, also called glycerol, is a humectant, which means
it helps attract water thereby trapping it in the skin. This
makes it a very effective moisturizer, as it both smoothes dry
skin and gives it long-lasting protection and care.

PANDEMIC Eczema Prevalence in the United States A substantial proportion of the US population has symptoms of eczema; 31.6 million with eczema, and at least 17.8 million with moderate to severe eczema or atopic dermatitis. The prevalence of childhood eczema/atopic dermatitis in the US is 10.7% overall and as high as 18.1% in individual states and 21% across various countries. Approximately one out of every three children with eczema/atopic dermatitis has moderate to severe disease. A recent study found that the prevalence of eczema in adults could be as high as 10.2%, which suggests that most children with eczema/atopic dermatitis continue to be affected even in adulthood. Three percent of US adults have moderate to severe eczema/atopic dermatitis requiring systemic therapy. These numbers are much higher than for psoriasis, a disease that now has many good-targeted treatments for moderate to severe patients. Yet, there are still very large unmet needs for the treatment of patients with eczema / atopic dermatitis. This discrepancy between the higher prevalence and lack of good treatments for moderate to severe eczema stems, in part, from an incomplete understanding of the disease.   • 8. ▪ Epidermis - The upper or outer layer of the two main layers of cells that make up the skin. The epidermis is mostly made up of flat, scale-like cells called squamous cells. Under the squamous cells are round cells called basal cells. The deepest part of the epidermis also contains melanocytes. These cells produce melanin, which gives the skin its color.

In Latin "stratum" = "layer" and "corneum" = "horny" so stratum corneum = horny layer.

  • 17. ▪ Eczema - any superficial inflammatory process involving primarily the epidermis, marked early by redness, itching, minute papules and vesicles, weeping, oozing, and crusting, and later by scaling, lichenification, and often pigmentation. It’s a common allergic reaction in children but it also occurs in adults, usually in a more severe form. Childhood eczema often begins in infancy, the rash appearing on the face, neck, and folds of elbows and knees. It may disappear by itself when an offending food is removed from the diet, or it may become more extensive and in some instances cover the entire surface of the body. Severe eczema can be complicated by skin infections. Childhood eczema may persist for several years or return after the child is older. Persons suffering from childhood eczema may develop another allergic condition later, most often hay fever or asthma.
  • 23. RESIDENT SKIN FLORA The resident skin flora is the physiological skin flora, also known as colonizing flora. It comprises bacteria such as Staphylococcus epidermidis, propioni and corynebacteria, which do not have a pathogenic effect on the skin. The microorganisms have important functions: their metabolism inhibits the growth of non-resident bacteria and fungi, and maintain the balance between the colonizing microorganisms. Around 80 per cent of the resident skin flora is on the surface of the stratum corneum, the outermost layer of the epidermis. Particularly high concentrations of microorganisms are found in body regions rich in sebaceous glands, e.g. scalp, forehead, axillae, front of the chest, and the area between the shoulder blades. 20 per cent of the resident microorganisms are found in the sebaceous glands (up to 0.3 mm deep). Image Credit: arsusda.gov Staphylococcus aureus is a gram-positive, round-shaped bacterium that is a member of the Firmicutes, and is frequently found in the nose, respiratory tract, and on the skin. It is often positive for catalase and nitrate reduction and is a facultative anaerobe that can grow without the need for oxygen. Although S. aureus is not always pathogenic, it is a common cause of skin infections such as a skin abscess, respiratory infections such as sinusitis, and food poisoning. Pathogenic strains often promote infections by producing virulence factors such as potent protein toxins, and the expression of cell-surface proteins that bind and inactivate antibodies. The emergence of antibiotic-resistant strains of S. aureus such as methicillin-resistant S. aureus (MRSA) is a worldwide problem in clinical medicine. Despite much research and development there is no approved vaccine for S. aureus. An estimated 20% of the human population are long-term carriers of S. aureus which can be found as part of the normal skin flora, in the nostrils, and as a normal inhabitant of the lower reproductive tract of women. S. aureus can cause a range of illnesses, from minor skin infections, such as pimples, impetigo, boils, cellulitis, folliculitis, carbuncles, scalded skin syndrome, and abscesses, to life-threatening diseases such as pneumonia, meningitis, osteomyelitis, endocarditis, toxic shock syndrome, bacteremia, and sepsis. While S. aureus usually acts as a commensal bacterium, asymptomatically colonizing about 30% of the human population, it can sometimes cause disease. S. aureus is extremely prevalent in persons with atopic dermatitis. S. aureus infections can spread through contact with pus from an infected wound, skin-to-skin contact with an infected person.

BENEFITS How an Alkaline Diet Works Here’s some background on acid/alkalinity in the human diet, plus key points about how alkaline diets can be beneficial: •Researchers believe that when it comes to the total acid load of the human diet, “there have been considerable changes from hunter gather civilizations to the present.” Following the agricultural revolution and then mass industrialization of our food supply over the last 200 years, the food we eat has significantly less potassium, magnesium and chloride, along with more sodium, compared to diets of the past. •Normally, the kidneys maintain our electrolyte levels (those of calcium, magnesium, potassium and sodium). When we’re exposed to overly acidic substances, these electrolytes are used to combat acidity. •According to the Journal of Environmental Health review mentioned earlier, the ratio of potassium to sodium in most people’s diets has changed dramatically.   • 75. VITAMIN D SUPPLEMENTS It’s well-documented that Vitamin D is essential for the proper synthesis of calcium, and it’s been shown to greatly reduce fracture risk in two ways. First, it helps with the formation of stronger bones; second, Vitamin D actually helps improve balance and prevent falls by enhancing muscle contraction. For rejuvenating your bones, it’s vitally important that you avoid Vitamin D2, and choose a bioavailable form of this important vitamin to avoid deficiency. If you live in an area where it’s difficult to get sun exposure or if you can’t spend 20 to 30 minutes outdoors a few times a week, then you should be taking at a minimum 600 IU of Vitamin D3 or cholecalciferol daily. But to achieve and maintain desirable levels, I recommend taking 2,000 IU once a day. Don’t worry that you may have too much Vitamin D if you also sunbathe. In the liver, thanks to a particular hepatic enzyme, Vitamin D3 is more readily metabolized into a bioactive form of D that is easily converted to its hormone form in the kidneys. It takes much longer to make this hepatic conversion with Vitamin D2. In fact, D2 and D3 are metabolized so differently that they result in “the production of unique biologically active metabolites.” Clearly, these forms of Vitamin D are simply not the same. Hence the study’s conclusion that ergocalciferol should not be used as a supplement. You may be wondering how Vitamin D2 supplements came to be the mainstream protocol in the first place. We need to go back almost 100 years to find the answer. Back in the early 1920s, scientists discovered that exposing foods to ultraviolet light produced Vitamin D2. Thus the manufacture of this vitamin was born, and the process was sold to what would become Big Pharma. Drug companies then produced and sold this medicinal form of Vitamin D to the public and to doctors. Less than 10 years later, around 1930, scientists noticed that Vitamin D2 did not seem as effective in the treatment of rickets in children (the primary use of the vitamin in those days) as the Vitamin D in cod liver oil. Subsequent studies showed mixed results, and not surprisingly, in 1949 the World Health Organization declared that differences between the two forms are “minimal”. But recent research clearly shows that they could not be more wrong!

WHEY PROTEIN Whey protein can be bought as a concentrate (which contains small amounts of lactose – the milk sugar – and fat), whey isolate (a form which is processed to remove the lactose and fat) or whey hydrolysate (where the protein chains are broken down into smaller units). What do we know about whey protein and psoriasis? Whey contains a number of smaller proteins that may influence psoriasis via their effects on the immune system and the growth of skin cells. For example, lactoferrin (one of the small proteins found in whey) has anti-bacterial properties, and has been shown to exert anti-inflammatory effects in humans and in animal studies. Whey protein extracts have also shown to slow the release of interleukin (IL)-2, a signaling molecule which stimulates the overgrowth of skin cells in psoriasis. Some of the proteins in whey also exert positive effects via antioxidant activity. People with psoriasis tend to have low levels of antioxidants together with high levels of oxidative stress, which may contribute to inflammation. Whey is rich in cysteine, a small protein which is converted to a potent antioxidant known as glutathione. Studies show whey protein supplements can enhance antioxidant defenses in a range of diseases 12 and potentially in psoriasis. What does the science say? Studies into the effects of whey protein in psoriasis have looked at whey protein extracts, whey protein isolate and topical application of the proteins found in whey. Whey protein extract Several studies have shown the benefits of a whey protein extract (known as Dermylex) in the treatment of psoriasis. In one double blind placebo-controlled study (considered the most rigorous type of clinical study there is), patients with mild-to moderate psoriasis were randomized to receive either a daily dose of 800 milligrams of Dermylex or a placebo (dummy) supplement for a period of 56 days. During the study period patients in the Dermylex group experienced a significant improvement in psoriasis severity, itching and self-reported quality of life scores (which assess how much symptoms have affected daily life) when compared to the placebo group. Similar to these findings, two earlier studies found that both 5 and 10 grams of Dermylex taken as a powder mixed into juice or food improved psoriasis severity, although the larger doses were not found to be any more effective than the 800-milligram (0.8 gram) dose. Whey protein isolate In another small study from Canada, seven patients with moderate to severe psoriasis vulgaris consumed a daily dose of 20 grams of whey protein isolate together with their usual treatments, which included topical steroids or UVB light therapy. After three months, the five patients who had completed the study experienced a significant improvement in psoriasis severity, with no other changes in their treatment.
