T1 Many studies are surfacing linking 12-hr shifts with increase in nursing care errors. How can
we salvage #ptsafety yet keep 12hrs #RNchat
rnchat Can someone retweet the topic to spread it around? Thanks! :) #RNchat
joshgriffin
RT @rnchat: T1 Many studies are surfacing linking 12-hr shifts with increase in nursing care
errors. How can we salvage #ptsafety yet keep 12hrs #RNchat
bthenextstep T1 I would love to see the analysis in regards to # of shifts worked in a row or per week. #rnchat
rnchat @dorameulman Welcome! #RNchat
3. bthenextstep
T1 I mean safety can be compromised in 4 hrs, 8, 12. Why we are so fixated on 12 all of a
sudden? #rnchat
nursingpins
T1 I think 12h shifts are here to stay -would take more nurses for 8 hr shifts-would cost more-
don't think it is a poss (8hr shifts #RNchat
rnchat
@bthenextstep T1 As you can imagine, the incidence of errors rises with each consecutive shift.
The # hrs sleep between shifts = 5.5 #RNchat
nursingpins
T1 I would like to see the study compared to working 8hrs - have seen nurses fall in the floor just
as often on 8 hr nights. #RNchat
Rapinedape
@PhilBaumann @EllenRichter I just thought I would say hello to all #nurses tonight, I can´t
attend @RNchat. Have a great #RNchat !
Wanderer_RN
T1, 8 hour shift are tough as you do 5 a week, it's difficult to come back daily like that. Still tired
after 8. #RNchat

rnchat @Rapinedape Glad you stuck your head in! We miss you! :) #RNchat
Trybarefoot One foot in #lrnchat - one foot in #RNchat. Bit of a balancing act tonite!
Wanderer_RN
@nursingpins We're having a hard enough time filling the 12 hours, not to mention the need if go
to 8 hours. #RNchat
rnchat
T1 I agree about desirability of 12 hour shifts in our life! I would hate to work 5 8-hr shifts at the
bedside again! #RNchat
bthenextstep T1 I think we all agree that 12hr is not only reason for errors or exhaustion. #rnchat
nursingpins
T1 I'm sure there are errors due to 12hr shifts - but nothing compared to working short staffed
shifts. #RNchat
bthenextstep grr. tweetgrid not cooperating #rnchat
rnchat
1 But unfortunately the data has been studied for the past 10 years & it does lean toward
greater safety issues with the 12-hr shift #RNchat
bthenextstep @nursingpins now now Vern stay on topic. *wink* #rnchat
4. rnchat @bthenextstep Share my TweetGrid with me! :) #RNchat
nursingpins LOL-ok #RNchat
Wanderer_RN
Yup. RT @nursingpins: T1 I'm sure there are errors due to 12hr shifts - but nothing compared to
working short staffed shifts. #RNchat
bthenextstep @rnchat safety is relevant to many variables, 12hr shift is only one! #rnchat
nursingpins
T1 12h shifts are part of nursing culture - lives have been built around them - will be hard to
change. #RNchat
rnchat
T2 Another good read on the issue "Patient Safety Risk? on ADVANCE for #Nurses"
http://j.mp/9qaYjA #RNchat
rnchat Oh geez, I meant T1 LOL #RNchat
bthenextstep
T1 so are we trying to imply that eliminating the 12 hr shift will decrease or eliminate the incident
of errors? really? #rnchat
rnchat
T1 Our own Twitter #nurse extraordinaire @DonnaCardilloRN wrote this blog "Are 12-hour
shifts safe?" http://j.mp/awouIq #RNchat
ClearMedical Is it possible that 12 hour shifts are favored by understaffed facilities? T1 #RNchat
Wanderer_RN
T1, it's easier to focus on length of shift adn fatigue vs other issues about patient safety. It's a
cop out. #RNchat
experien_CEZ
RT @RNchat: "Study Reveals Widespread Fatigue, Risk for Errors with 12-Hour Nursing
Shifts" http://j.mp/cwVpts #RNchat
rnchat
@bthenextstep T1 I think its a fact that came out of studies & we cant ignore it. Maybe learn
from it though & adjust in some ways? #RNchat
nursingpins
T1 - 12hrs do contribute to errors - but there are many other problems we should be looking at
instead of 12h shifts. #RNchat
bthenextstep @rnchat definitely further study. but yes, compelling to say the least. #rnchat
bthenextstep
@rnchat but then again, did we really need a study to tell us something all nurses already knew?
That's the disturbing part. #rnchat
5. rnchat
Just one more link for later reading T1 "Linking Medical Errors, Nurses’ 12-Hour
Shifts" http://j.mp/93EbKW #RNchat
rnchat
@bthenextstep T1 The studies tested decision-making capabilities & judgement at end of
12-hr shifts compared to 8-hrs #RNchat
nursingpins
T2 - I think 12hr shifts are here to stay - will have to look at how to improve what few errors are
caused by them. #RNchat
nursingpins T2 -I would still like to see the same study done with 8hr night shifts for comparison. #RNchat
DrDeanBurke
#rnchat, forgive this lurking doc, but all the nurses I know have built, their lives and second jobs
arnd 12 hr shifts....
rnchat
T1 Woo hoo! I'm making enemies with this topic! LOL hey I just read the data & interpret it.
I dont make the rules! I love 12 hrs! #RNchat
rnchat @DrDeanBurke T1 We welcome your additions & comments!! :) #RNchat
bthenextstep
I guess I'm stuck on 12hr shift, b/c from my pnt of view - the shift length has little correlation 2
exhaustion/decisions/ judgment #RNchat
Wanderer_RN
T1, love my 12-hours. I know I get tired, but I'm aware of it. Change to my practice to
comepnsate. #RNchat
rnchat
@ClearMedical T1 I think at this time in the evolution of nursing, most everyone favors 12-hr
shifts for convenience in scheduling! #RNchat
dorameulman
#RNchat T1 3X12 hrs are less strssfull than 5X8hrs less travil time. The extra days off make up
for the long days :))
bthenextstep T1 i don't know about you, but my head has been 'fuzzy' after just 4 hrs of a rough shift. #rnchat
nursingpins
T2 -It is to the point now - nurses could not do the min. that would be required in 8hrs - most
can't do it in 12hrs. #RNchat
bthenextstep
@nursingpins T1 to support my friend Vern. what will elimn of 12hr shift due to our already
stressed staffing? #rnchat
PamelaKline
RT @rnchat: Just one more link for later reading T1 "Linking Medical Errors, Nurses’ 12-

6. Wanderer_RN
T1, used to be more tired as I wasn't sleeping due to sleep apnea. That's fixed, now am clearer.
Wasn't work making me tired. #RNchat
bthenextstep
T1 isn't the key: the nurse recognizing, or fellow staff members recognizing the s/s of
fatigue/exhaust/etc #rnchat
rnchat @PamelaKline Welcome GF! :) #RNchat
Biebert
We need to not ignore data. Got to keep emotions out of it. We used to base everything off of
blood letting and leaches... T1 #RNchat
bthenextstep @DrDeanBurke thanks for joining us doc! #rnchat
rnchat
T1 Hopefully, as a result of these study findings, we'll get appropriate break periods during our 12
hour shifts? That will help! #RNchat
rnchat
@Biebert T1 Yes, thats my point. We cannot ignore data. Some good could come of this
information to improve safety in care delivery #RNchat
bthenextstep
T1 I am all for and 100% for EBP, but sometimes the research can be misleading or misguiding in
its message. #rnchat
NurseCircle
lol @Biebert: We need to not ignore data. Got to keep emotions out of it. We used to base
everything off blood letting and leaches #RNchat
CardioNP
Not worked 12 hr shifts in a while, can't recall errors, but after 3 12hrs in a row I was beat. Many
RNs doing 2 jobs -prob not safe #RNchat
Wanderer_RN
T1, about 2 years ago there was a lot of talk about naps to combat fatigue, but it went nowhere.,
no time for breaks much less naps. #RNchat
bthenextstep
@rnchat ooooh What's that? Whats a break? Is that like getting to use the bathroom or
something? ;) #rnchat
rnchat
OK moving along to topic 2 (T2) and thanks for your great input & comments on the first
topic, folks! :) #RNchat
DrDeanBurke
#rnchat glad to be here @bthenextstep the safety issue will trump convenience, if further
research confirms, imho-as it has in med. ed/res.
bthenextstep @rnchat T2: bring it on! :) #rnchat
rnchat
@bthenextstep LOL yes I know....a decent "break" is a prehistoric concept these days,
right? #RNchat
7. biofilic_ZinCo Install accessible greenroofs which have been shown to reduce medical errors! #RNchat
dorameulman
#RNchat T1 In critical care I'm in the zone sometimes 12 hrs aren't even enough. I get into it.
I'm not done in 8 hrs. A little crazy :)
bthenextstep @DrDeanBurke and your absolutely right about that. #rnchat
nursingpins
T1 12hr shifts are responsible for errors-will there be fewer with 8hr. shifts - prob not-8hr shifts
come with their own problems. #RNchat
rnchat
T2 How do we improve "Hand-Off" reporting betw healthcare professionals to reduce
errors in communication that impact on #ptsafety? #RNchat
PamelaKline @rnchat thanks! Glad to b here! #RNchat
bthenextstep @dorameulman oh yes. I know that feeling. #rnchat
Srini2000 Srini, developer of medical instrumentation. Hello everyone. Sorry for joining in late #rnchat
rnchat Again, will one or 2 of you RT the T2 topic so it gets around? :) #RNchat
bthenextstep T2 I've always been a big supporter of a standardized report form for all. #rnchat
rnchat @Srini2000 Welcome! :) #RNchat
NurseCircle
One of the nurses on our facebook discussion says she has to work two jobs anyway, so 5 8hr
shifts wouldn't help. T1 #RNchat
bthenextstep
T1 I however believe nurse-nurse is the only way to give report. Not a fan of recorded report at
all. #rnchat
rnchat
Me too! RT @bthenextstep: T2 I've always been a big supporter of a standardized report form for
all. #rnchat #RNchat

RT @rnchat: T2 How do we improve "Hand-Off" reporting betw healthcare
professionals to reduce errors in communication that impact on #ptsafety? #RNchat
CardioNP
OMG, so tired o Our institution uses SBAR, but seems I never get report when they send a pt to
nuc med for stress test. #RNchat
rnchat
T2 Joint Commission literature states up to 80% errors due to faulty & haphazard
communication about patients between caregivers #RNchat
DrDeanBurke
RT @rnchat: T2 How do we improve "Hand-Off" reporting betw healthcare

bthenextstep I think recorded reports are a big culprit of the hand off errors. #rnchat

Hand-Off Communication" http://j.mp/cJ1JSZ #RNchat
nursingpins
T2 - haphazard is right - also I have found that bed-side reports are much better - can ask
questions about what you see. #RNchat
CardioNP
Preferred to do bedside report, visualization of the pt was important. Reading a Kardex meant a
lot less #RNchat
bthenextstep
T2 some hospitals actually want you to do nurse-nurse report in front of pt/family. Not sure how i
feel about that? #rnchat
DrDeanBurke
#rnchat don't think recording can give the nuances of a face to face communication. but need
focus, no distractions-difficult in any enviro.
s_eller
T2 - TJC says hand-offs area of risk due 2 faulty communication - I believe pt/surrogate should
participate in transition of care #rnchat
Srini2000
Excuse my ignorance. I imagine there is a lot of spoken info that needs to be handed off.Not
everything can be noted. How to ? #rnchat
rnchat
@CardioNP T2 My facility uses SBAR too but it still doesnt help. Reports on pt conditions are
never complete #RNchat
9. nursingpins
T2 also have found many problems during bed-side reports - o2 off, iv's out, pt hanging out of
the bed etc. #RNchat
Wanderer_RN T2, bedside report is gret as you get "eyes-on" quick. #RNchat
bthenextstep T2 SBAR is only as good as the person utilizing it. #rnchat
CardioNP @rnchat I find SBAR totally ineffecitive (IMO). #RNchat
dorameulman
#RNchat We give report at the bedside. I just want the basic information 5 min per patient. I'll
figure the rest out myself. Please go home!
Wanderer_RN T2, unfortunately our reports to ancillary depts. are more lacking then nurse-nurse. #RNchat
s_eller
Pts and surrogates can fill in missing info & correct errors. Also gives real-time pain scores
& goals #rnchat
bthenextstep @dorameulman lol. hard to do in the ICU. 5 min report is impossible. :) #rnchat
Wanderer_RN RT @bthenextstep: T2 SBAR is only as good as the person utilizing it. #rnchat #RNchat
rnchat
T2 Even nurse-physician communication about patient condition varies from person to person. We
need standardized guides #RNchat
CardioNP including pt/fam/so would add so much time to report that you'd never get out on time #RNchat
nursingpins
T2 a standard report sheet is important too - (many do not use) with the basics - including brief
hx - labs etc. #RNchat
rnchat @s_eller T2 Excellent points about family & surrogates #RNchat
CardioNP
T2 @rnchat Agree standardized report forms would help, but doubt would solve all the issues
#RNchat
bthenextstep
T2 don't you think these 'studies' are trying to 'automatize' the human element of nursing
sometimes? #rnchat
Wanderer_RN
T2 as much as I hate our written reports due to the tedious nature, it is nice to have the
information. #RNchat
s_eller
@CardioNP agree that including pt/family can add time, but how much time is avoiding error
worth? Standardization of report may help #rnchat
10. Srini2000 Is it possible to have audio report in addition to written report? #rnchat
bthenextstep
T2 the very reason our reports 'catch' the unspoken/unnoticed is the same reason errors are
made. Hmm.. #rnchat
rnchat
T2 And its not only nurses who are lacking in hand-off reporting skills. Doctors, therapists, etc.
need to improve on hand-off too #RNchat
bthenextstep @Srini2000 yes, you can have both. but very time consuming. #rnchat
dorameulman
@bthenextstep #RNChat not imposable! Most nurses talk too much about nothing. Tell me the
basics I got this.... OK :))
ashtoreth_1221
T2 - Some units I've seen Charge Nurse gets all the report - then as a group they will listen to the
report as dictated by the CN. #RNChat
Srini2000 @bthenextstep Isn't audion quicker than the written one? #rnchat
DrDeanBurke
@rnchat #rnchat wide variability in the reporting-it is an art and should be taught, not everyone
can tell a story from beginning to end...
Wanderer_RN
T2 we have a written report sheet handed to the new nurse followed by a face-to-face. Maybe
much but it has helped. #RNchat
bthenextstep @dorameulman LOL I agree on the gossiping needing to stop. #rnchat
rnchat
@Srini2000 T2 Your suggestion could become very timely, writing a report AND verbally reporting
too. We would need an hour or more! #RNchat
ashtoreth_1221
@Srini2000 Some hospitals I've seen have audio reports - downside - time consuming to listen to
it =( #RNchat
s_eller
T2 - does anyone do SIDR - standardized inter-disciplinary rounds/report & if so, do you
include patients? #rnchat
bthenextstep
@Srini2000 audio good/quicker, but quality of report is lacking. hard to describe, but rn-rn report
much better. #rnchat
nursingpins
T2 - there is many ways to include family in the care but report is not one of them - report then
would change. #RNchat
11. rnchat
T2 I cant count the times I've driven home & then called the ICU to pass along info I forgot
to mention! #RNchat
bthenextstep
@rnchat currently we do audio report and then follow up afterward with face-face. very time
consuming. #rnchat
Srini2000
@ashtoreth_1221 Can imagine. Will need discipline and some training to be short and precise
#rnchat
CardioNP
RT @DrDeanBurke #rnchat reporting-it is an art and should be taught, not everyone can tell a
story from beginning to end...
bthenextstep @s_eller never heard of that one. interesting . #rnchat
ashtoreth_1221
T2 - maybe: a detailed written report (standardized form used) w/c would be read after a face-to-
face brief report #RNChat
bthenextstep
RT @rnchat: T2 I cant count the times Ive driven home & then called the ICU to pass along
info I forgot to mention! #rnchat
bthenextstep
T2 with all these variables: the standardized report is probably the most effective intervention.
#rnchat
Wanderer_RN
@rnchat @Srini2000 we write report quickly then give face2face. Most are out w/in 30 min. Have
to guide the verbal report though. #RNchat
ashtoreth_1221T2 - sorry if I just jumped right in guys. #RNChat
rnchat
@CardioNP Agree w/ the RT @DrDeanBurke Reporting is an art & should be taught, not
everyone can tell a story from beginning to end #RNchat
bthenextstep
T2 then there is the question: does each specialty area need/require their own 'standardized'
report sheet? #rnchat
PamelaKline
RT @rnchat: T2 Joint Commission lit states up to 80% errors d/t faulty & haphazard
communication about patients between caregivers #RNchat
nursingpins
T2 standardization of information is most important - otherwise some would be all over the board.
#RNchat
Wanderer_RN @ashtoreth_1221 Go for it! More ideas the better! #RNchat
bthenextstep
RT @Srini2000: @ashtoreth_1221 Can imagine. Will need discipline and some training to be short
and precise #rnchat
rnchat @ashtoreth_1221 Please jump whenever you like! We love the participation! :) #RNchat

floors, but they don't include patients - yet #rnchat
DrDeanBurke
RT @rnchat: T2 I cant count the times ive driven home & then called the ICU 2 pass along
info #RNchat LOL, how many tmes, I calld, ben cald!
bthenextstep
T2 for me: my report sheet ALWAYS helped me give my report. It doubled as a 'brain' for me
while at the bedside. #rnchat
ashtoreth_1221
T2 - d difficulty really is not the forms used or the way we report - it's the change that is difficult
- some r not open to changes #RNChat
rnchat
@CardioNP T2 Wow, now that is interesting. Online pt logs showing Dx, meds, tx plan etc for
hand off reporting #RNchat

#rnchat
rnchat
@ClearMedical T2 Some agency nurses have it down better than the regular staff! They know how
hard it is to communicate on new pts #RNchat
dorameulman
@bthenextstep T2 #RNChat I leave my report sheet with oncoming staff. They have my cell
number if they have questions. I never get called!
nursingpins
T2 there are so many PT nurses - the whole story has to be told over and over - as opposed to
continuity of FT staff. #RNchat
ashtoreth_1221@ClearMedical I can only imagine... =S #RNChat
Wanderer_RN
T2, it's really about being able to give the "Story" concisely and quickly. It's an art.
Paper just helps! #RNchat
DrDeanBurke
#rnchat forgive me, but LOL thinking of one of my southern nurse travelers, giving report in
NYC....
bthenextstep @nursingpins I'm a PRN employee now - so I'm living that nightmare right now. LOL #rnchat
rnchat
@dorameulman T2 Thats a good idea, to include a phone # to reach a caregiver after report. It
can save much aggravation at times #RNchat
ashtoreth_1221@Wanderer_RN that is definitely something I can agree on.... #RNChat
PamelaKline @nursingpins T2 couldn't agree more! #RNchat
bthenextstep
T2 I also think the 'art' of a good report is receiving it properly. Asking the app. questions,
inquiring, etc. #rnchat
