Definition – Intermediate Care
> “Umbrella” term
> Describes an approach involving a
  range of services working to
  common aims and principles.
> Supports recovery, mostly home
  based, at times of crisis.
Intermediate Care Demonstrators
 > December 2008 – bids invited
 > National priorities of dementia, 7 day
   access, staff training and
   development, pharmacy and
   reduced delayed discharges
The Art of the “Inbetween”
> Intentional design of something we
  could not easily describe
> All areas were working on this
> Evidence increasing but more
  required
The process
> Invitations to bid - Dec 2008
> Short turnaround eliminated many
> The enthusiasts prepared their bids
> Partnership sign off essential
> Selection procedure at JIT
The Sites selected – 5 + Orkney
> Ensured some overlap in topics to
  increase potential for learning
> All key priority areas were covered
> Support from the JIT given at start
  and throughout the project.
