14. 1. Create new form either from blank or a
template.
2.Starting from the top, you can add a title and
description.
3.In the box below, you can add questions.
4.To add more questions hit the + icon from the
toolbar on the right.
5.Other settings found in the floating toolbar
includes importing questions from other forms,
adding a subtitle and description, adding a photo,
adding video and creating a separate section in
your form.
6.To see how your form looks when others open it,
hit the Preview icon present at the top right next
to Settings.

15. 1.Hit the Customize Theme icon, right
next to the Preview icon, to open
theme options.
2.You can then choose a preloaded
image as the header or you can even
choose a personal photo as well.
3.Choose theme colour of the header
image

16. 1.Choose Short answer for giving a one line answer
2.Choose Paragraph for a multiple line or detailed
answer.
3.Choose Multiple choice, checkboxes or dropdown
for multiple options.
4.Select Linear for scaled answers.( from lower to
higher)
5. If you want more columns and rows in multiple
choice questions then select Multiple Choice grid or
Checkbox grid.
6. Answer in the form of uploading files viz.
photos, videos, documents etc. ( set the max. no. of
files as well as the max. file size limit)
7.Duplicate question by hitting Duplicate at the
bottom & remove a particular question by hitting
Delete.

Multiple choice grid question :
➢It allows to select one answer per row.
➢ It has multiple rows and columns
➢Used for online surveys.
Checkbox grid question:
➢It allows to select multiple answer
choices per row.
➢ They are used when one has
similar sub questions (rows) and
standard answer options
(columns).
Linear scale question:
➢It allows users to provide
a numeric response to a
statement or a
question.
➢ It starts from lowest to
the left & highest on the
right (either 0 or 1 and
ends on 5 to 10 with
labels for each end of
the scale).
➢Used for collecting rated
opinion.
24. 1.Go to the three dots icon in the top
right and click Add collaborators.
2.You can then add the emails of
people with whom you want to
collaborate with or you can copy the
link and share it via third party apps
such as WhatsApp Web or Facebook
Messenger.
3. Click Send to share your form via
email or link (shorten the URL) or
embed .

25. 1.Go to Settings > click Quizzes tab >
enable Make this a quiz.
2.Choose respondents to get the results
immediately or reveal it later manually.
3.Select - respondent can see Missed
questions, Correct answers and Point
values. Click Save to close.
4.Under every question assign the correct
answer and its points /marks.
[ Answer Key > tick the correct answer
> assign the points > add answer feedback
(optional) > click Save]
5.Correct answer will automatically be
rewarded with full points which can be
checked by going to the Responses tab .

26. 1.Click Responses tab.
2.Disable Accepting Responses
3.Click on Summary tab to look at
the performance of all the
respondents.
4.The question tab evaluates the
responses by selecting each question
one by one.
5.Finally, the Individual tab evaluates
the individual performance of each
respondent.
6.Click on plus icon for creating
spreadsheet for all the received
responses
