• Responsible for coalescing a hard copy of paperwork for a permanent Lifesharing donor chart and for
processor records- a self-consent or family consent for tissue donation/the medical history questionnaire/tissue
referral summary, and onsite donor medical records.
• Responsible for uploading donor information pertaining to case into UNET during and at the end of the
case.
