  1. 1. Helping Hands is a needs-based care program that Berkeley Patients Group (BPG) has proudly offered as a service to our community since our founding in 1999. Helping Hands provides free medical cannabis to very low-income Berkeley residents. For more information and to download an application packet please visit: www.myBPG.com/services/Helping-Hands An individual is encouraged to apply to the Helping Hands program if he or she is a very low-income Berkeley resident who is qualified under California law to use cannabis for medical purposes. Beginning in July 2015, new enrollees to the Helping Hands program are eligible to receive seven grams of flowers per week. All medicine is laboratory tested for safety and potency If you require high-CBD medicine, please be sure to note this on your application in the Member Information section. How To Apply: Be a patient-member of BPG. Fill out the Helping Hands application form completely. Attach supporting documents required by application, including proof of residence and your tax return or other financial documentation for the year 2014. Submit the completed application and all attachments to BPG in person or by mail by close of busi- ness on June 20, 2015. Incomplete or late applications will not be con- sidered. If mailed, the application must be postmarked no later than June 20, 2015 and addressed to: BPG ATTN: Helping Hands 2366 San Pablo Avenue Berkeley, CA 94702 Applications are evaluated by a committee in a blind selection process. Applicants not selected for enrollment may reapply during the next application period.   2. 2. Yes. Eligible applicants not enrolled will be placed on the program waitlist. Members will be enrolled from the waitlist as spots become available. It is important that you notify BPG if your financial situation improves and you no longer need Helping Hands’ assistance. Failure to inform BPG in a timely manner could result in removal from the program and/or exclusion from consideration for future enrollment. Yes. If you miss four (4) consecutive package pick-ups, you will be dropped from the program. Yes. You may only pick up one package per week. Any packages that are not picked up are forfeited. Yes. All of the information you provide will be kept strictly confidential and solely for the purposes of determining eligibility for the Helping Hands program. No. Applicants enrolled in Janurary 2015 need not re-apply until the fall enrollment period. Yes. BPG is instituting a legacy program for non-Berkeley residents. If you have been continuously enrolled in Helping Hands since before 2014, then you may re-apply regardless of Berkeley residency. Is there a waitlist? What if I am accepted into the Helping Hands program, but then my situation improves? Once I am in the Helping Hands program, can I be dropped? Do I have to pick up my medicine every week? Will my personal and medical information be protected? If I enrolled earlier this year, do I need to re-apply? I’ve been a Helping Hands patient for years, but I don’t live in Berkeley.
