section?
PLANNED DELIVERY ROUTE OF PRETERM BREECH SINGLETONS, AND NEONATAL
AND 2 YEAR OUTCOMES: A POPULATION BASED COHORT STUDY

Scenario
 A primiparous women presents at 28 weeks of gestation with spontaneous
rupture of membranes and contractions. She is 5 cm dilated. Bedside ultrasound
reveals a singleton fetus in frank breech position. You have stabilised her and
administer steroids and tocolytics. How do you counsel her about mode of
delivery?
Introduction
 preterm birth associated with high frequency of breech presentation (20% at 28
weeks)
 Mode of delivery at this gestational age remains controversial
 Risk of vaginal breech delivery – head entrapment
 Caesarean section in preterm associated with technical difficulties and morbidity
Why is choosing the optimal mode of delivery
is challenging?
1. No contributive RCT comparing vaginal and caesarean deliveries in preterm
breech
 Interrupted early because of recruitment difficulty
2. Most studies consider actual delivery route
 Subject to major indication bias instead of planned delivery route
3. Few studies address long term outcome
 Relevant when assessing perinatal practices within preterm population
Study design
 Prospective observational cohort study
o setting in France , 2011
o a planned analysis from EPIPAGE – 2 cohort
 This mode of study is a best compromise between quality and feasibility to assess
impact of planned delivery route on neonatal outcome with preterm breech birth
in the absence of RCT
Participants
o study population : all singleton breech fetus delivered in hospital from 26 + 0
to 34 + 6 weeks
o criteria : preterm labour or preterm prelabour rupture of membranes (PPROM)
who were alive at beginning of labour or at the decision to perform caesarean
section
Exclusion criteria
o pregnancies with hypertensive disorders, fetal growth restriction
o isolated placental abruption
o termination of pregnancy
o fetal death before admission to hospital or before labour
o multiple pregnancies
o homebirth
Comparison
 Planned vaginal delivery and planned caesarean section
 Planned vaginal delivery : vaginal delivery or caesarean section performed
during labour for abnormal fetal heart rate or failure to progress
 Planned caesarean section: when performed during labour for the indication
systematically due to gestational age and/or fetal position

Outcomes
 Primary outcome : survival – number of children discharged from hospital alive
relative to the number of foetuses alive at the beginning of labour
 Secondary outcome: survival to discharge without severe neonatal morbidity.
 Severe neonatal morbidity:
 Grade 3 or 4 IVH
 Cystic periventricular leukomalacia
 Stage 2 – 3 NEC
 retinopathy of prematurity
 Third outcome: survival without neurosensory impairment at 2 years of
corrected age
 Survival without CP, blindness or deafness
 A detailed neurological and sensory examination performed by referring physician

Results
 Primary outcome : neonates with PVD compared with PCD did not differ in
survival at discharge (93.0 vs 95.7%, P=0.14)
 Secondary outcome : survival at discharge without severe morbidity (90.4 vs
89.9%, P= 0.85)
 Third outcome : survival at 2 years without neurosensory impairment (86.6
vs 91.6%, P= 0.11)
Main findings
 In cases of preterm breech delivery at 26–34 weeks of gestation after PTL or
PPROM, as compared with PVD
 PCD was not associated with improved survival, survival at discharge without
severe morbidity, or survival at 2 years of corrected age without neurosensory
impairment
Strength
 The main strength of this study : design based on both planned delivery route
and propensity‐score analysis
 propensity‐score analysis allowed us to balance observed baseline covariates
across the two groups and therefore minimises the indication bias
 Propensity score reflects likelihood of planning a PCD rather than PVD
 analysing data from a prospective nationwide population‐based cohort, with good
representativeness of population and practices, as well as appropriate statistical
methods, provides robust and relevant alternative insights for daily obstetric
management

Interpretation
 greatest risks associated with PVD is head entrapment
 In this study, only one infant from the PVD group died because of head entrapment as
compared with none in the PCD group
 head entrapment is a rare complication and can also occur with PCD
 when discussing the route of delivery with parents, the high risks of maternal
morbidity associated with caesarean section performed at low gestational age
should be taken into consideration
 Reddy et al. reported high incidences of serious short‐term maternal complications after
early preterm delivery, up to 14.4% with caesarean delivery versus 3.5% with vaginal
delivery
Conclusion
 The study indicates that PCD is not associated with improved neonatal outcomes
for preterm breech singletons born at 26–34 weeks of gestation after PTL or
PPROM
 route of delivery should be discussed with women
 balancing neonatal outcomes with the higher risks of maternal morbidity associated
with caesarean section performed at low gestational age

Preterm breech, vaginal delivery or caesarean section?

  • 1. Preterm breech, vaginal delivery or caesarean section?   • 2. Scenario  A primiparous women presents at 28 weeks of gestation with spontaneous rupture of membranes and contractions. She is 5 cm dilated. Bedside ultrasound reveals a singleton fetus in frank breech position. You have stabilised her and administer steroids and tocolytics. How do you counsel her about mode of delivery?
  • 3. Introduction  preterm birth associated with high frequency of breech presentation (20% at 28 weeks)  Mode of delivery at this gestational age remains controversial  Risk of vaginal breech delivery – head entrapment  Caesarean section in preterm associated with technical difficulties and morbidity
  • 4. Why is choosing the optimal mode of delivery is challenging? 1. No contributive RCT comparing vaginal and caesarean deliveries in preterm breech  Interrupted early because of recruitment difficulty 2. Most studies consider actual delivery route  Subject to major indication bias instead of planned delivery route 3. Few studies address long term outcome  Relevant when assessing perinatal practices within preterm population
  • 5. Study design  Prospective observational cohort study o setting in France , 2011 o a planned analysis from EPIPAGE – 2 cohort  This mode of study is a best compromise between quality and feasibility to assess impact of planned delivery route on neonatal outcome with preterm breech birth in the absence of RCT
  • 6. Participants o study population : all singleton breech fetus delivered in hospital from 26 + 0 to 34 + 6 weeks o criteria : preterm labour or preterm prelabour rupture of membranes (PPROM) who were alive at beginning of labour or at the decision to perform caesarean section
  • 7. Exclusion criteria o pregnancies with hypertensive disorders, fetal growth restriction o isolated placental abruption o termination of pregnancy o fetal death before admission to hospital or before labour o multiple pregnancies o homebirth
  • 8. Comparison  Planned vaginal delivery and planned caesarean section  Planned vaginal delivery : vaginal delivery or caesarean section performed during labour for abnormal fetal heart rate or failure to progress  Planned caesarean section: when performed during labour for the indication systematically due to gestational age and/or fetal position

Outcomes  Primary outcome : survival – number of children discharged from hospital alive relative to the number of foetuses alive at the beginning of labour  Secondary outcome: survival to discharge without severe neonatal morbidity.  Severe neonatal morbidity:  Grade 3 or 4 IVH  Cystic periventricular leukomalacia  Stage 2 – 3 NEC  retinopathy of prematurity  Third outcome: survival without neurosensory impairment at 2 years of corrected age  Survival without CP, blindness or deafness  A detailed neurological and sensory examination performed by referring physician

  • 12. Results  Primary outcome : neonates with PVD compared with PCD did not differ in survival at discharge (93.0 vs 95.7%, P=0.14)  Secondary outcome : survival at discharge without severe morbidity (90.4 vs 89.9%, P= 0.85)  Third outcome : survival at 2 years without neurosensory impairment (86.6 vs 91.6%, P= 0.11)
  • 13. Main findings  In cases of preterm breech delivery at 26–34 weeks of gestation after PTL or PPROM, as compared with PVD  PCD was not associated with improved survival, survival at discharge without severe morbidity, or survival at 2 years of corrected age without neurosensory impairment
  • 14. Strength  The main strength of this study : design based on both planned delivery route and propensity‐score analysis  propensity‐score analysis allowed us to balance observed baseline covariates across the two groups and therefore minimises the indication bias  Propensity score reflects likelihood of planning a PCD rather than PVD  analysing data from a prospective nationwide population‐based cohort, with good representativeness of population and practices, as well as appropriate statistical methods, provides robust and relevant alternative insights for daily obstetric management

  • 16. Interpretation  greatest risks associated with PVD is head entrapment  In this study, only one infant from the PVD group died because of head entrapment as compared with none in the PCD group  head entrapment is a rare complication and can also occur with PCD  when discussing the route of delivery with parents, the high risks of maternal morbidity associated with caesarean section performed at low gestational age should be taken into consideration  Reddy et al. reported high incidences of serious short‐term maternal complications after early preterm delivery, up to 14.4% with caesarean delivery versus 3.5% with vaginal delivery
  • 17. Conclusion  The study indicates that PCD is not associated with improved neonatal outcomes for preterm breech singletons born at 26–34 weeks of gestation after PTL or PPROM  route of delivery should be discussed with women  balancing neonatal outcomes with the higher risks of maternal morbidity associated with caesarean section performed at low gestational age
