6. How it Works INJURY REPORTING GUIDELINES Employees are required t o report job related injuries IMMEDIATELY to their supervisor. On the rare occasion when these members of management are not available, employees may call the AXIS-CARE nurse for patient assistance at 1-800-903-0840. In serious “life or limb” threatening circumstances (and only in these cases) immediate referral to emergency medical services should be made prior to calling the AXIS-CARE nurse by dialing 911 to access emergency care. Supervisors will advise AXIS-CARE medical managers of reported injuries immediately upon learning of such injury to any employee. All injuries are to be called in to the AXIS-CARE nurse with the exception of injuries requiring only minor first aid treatment (e.g. minor eye irritations, minor scrapes and cuts). All orthopedic injuries such as backs, knees, shoulders, and ankles are to be called in. The phone number is 1-800-903-0840. A nurse is available 24 hours a day, 7 days a week. The AXIS-CARE medical management staff will evaluate the reported injury for necessity of medical treatment, assist as needed in securing such treatment for the employee from appropriate physicians, engage in appropriate medical care providers treatment plans and advise supervisory and risk management of all return to work considerations. Concurrent with notification to the AXIS-CARE (or first thing the following business day) the nurse manager will notify the employer safety, risk management and claims administrative team of the injury and prepare the employer’s first report of injury. The AXIS-CARE managers will assist in all necessary documentation and will provide all medical information available to the claims administrator with an electronic copy to risk management and safety (per specified reporting plan). The AXIS-CARE medical manager will follow up with each injured employee to ensure compliance with treatment plans, effective return to work and necessary documentation for claims filing.
