2. HYPERTANTION
 Hypertension is the term used to describe high blood pressure. Hypertension is repeatedly elevated
blood pressure exceeding 140 over 90 mmHg.

3. Hypertension Nursing Diagnosis And Care plan
 NURSING DIAGNOSIS
Acute pain related to Increased cerebral vascular pressure as evidenced by patient verbalization.
EXPECTED OUTCOME
 Patient will report relief of pain with in 2-3 hours.
 Patient will follow prescribed pharmacological regimen.
 Patient will demonstrate use of relaxation skills and diversional activities.

4. NURSING INTERVENTION
 Encourage and maintain bed rest during the acute phase.
 Provide or recommend nonpharmacological measures to relieve headache such as cold compression.
 Administer medications as indicated:
Analgesics; Reduce or control pain

5. NURSING DIAGNOSIS
Decreased Activity Tolerance related to Imbalance between oxygen supply and demand as evidenced by
dyspnea.
EXPECTED OUTCOME
 Patient will participate in necessary/desired activities.
 Patient will use identified techniques to enhance activity tolerance.
 Patient will report a measurable increase in activity tolerance.
 Patient will demonstrate a decrease in physiological signs of intolerance.

6. NURSING INTERVENTION
 Instruct patient in energy-conserving techniques .
 Encourage progressive activity and self-care when tolerated.
 Encourage to the patient to have gradual activity progression to prevent increase in cardiac
overload.
 Monitor vital signs before, during and after activities.

7.  POTENTIAL NURSING DIAGNOSIS
Risk for decreased cardiac output related to inadequate oxygenated blood pumped by
the heart to meet metabolic demands.
EXPECTED OUTCOME
 stable in the cardiac rate and rhythm
 Maintain blood pressure within an acceptable range.
 Engage in interventions to help decrease cardiac load and blood pressure

8. NURSING INTERVENTION
 Assess vital signs, focusing on blood pressure and pulses and record.
 Check blood pressure readings on arms and thighs and record.
 Advise the patient to limit intake of food high in sodium and cholesterol.
 Encourage patient to be vigilant in the intake of his maintenance medications.
 Give prescribed predication (diuretic furosemide (Lasix); ethacrynic acid (Edecrin);
bumetanide (Bumex), torsemide (Demadex).
Alpha, beta, or centrally acting adrenergic antagonists: doxazosin
(Cardura); propranolol (Inderal); acebutolol (Sectral); metoprolol (Lopressor)
