What is CDSMP Evidence based, peer-led programming designed to help people gain self-confidence in their ability to control their symptoms and health conditions and how their health problems affect their lives. 3. Statewide Goals for CDSMP Increase availability of CDSMP to older adults in Colorado Establish a statewide CDSMP distribution and delivery system Maintain quality through fidelity to the evidence based program Develop partnerships to build CDSMP implementation and sustainability
4. Timeline and Targets Grant period March 30, 2010 – March 30, 2012 Target population = people with chronic diseases > 60 years old Target = 566 participant completers (4 of 6 workshop sessions)
