Criança, Infantil e juvenil moderadamente obesa, porém normais sem estas condições, tendem a apresentar uma idade óssea levemente avançada e maturação psicológica avançada com aumento da estatura linear durante a infância e o início da puberdade poderá na maioria dos casos evoluírem para uma baixa estatura linear definitiva na fase adulta. Assim sendo, a baixa estatura linear em uma criança com sobrepeso deve ser considerado o resultado de uma causa orgânica até que se tenha prova ao contrário.
SÍNDROMES DE BAIXA ESTATURA; UM COMPROMETIMENTO SÉRIO DE CRESCIMENTO EM CRIANÇA, INFANTIL E JUVENIL.

As síndromes de Laurence–Moon, de Biedl-Bardet ou de Prader-Willi
podem combinar a
baixa estatura
(assim como outras
causas endócrinas
tais como o
hipotireoidismo,
excesso de
glicocorticóide,
pseudo-hipoparatireoidismo
com osteodistrofia hereditária de Albright [OMIM #
103580] e DGH). Criança, Infantil e juvenil moderadamente obesa,
porém normais sem estas condições, tendem a apresentar uma idade

Síndrome de Bardet-Biedl
Os sintomas mais frequentes desta síndrome são: obesidade,
retinopatias (doença degenerativa não inflamatória da retina) como a
distrofia retiniana, polidactilia, problemas renais, hipogonadismo e
retardo mental. Outros problemas também relacionados à SBB, porém
2. menos frequentes, são os seguintes: estrabismo, catarata, astigmatismo,
anosmia (diferentes graus, com possível perda total, de sensibilidade
olfativa), problemas auditivos, sindactilia (qualquer grau de fusão ou
aderência – por meio de membranas – dos dedos das mãos ou pés,
envolvendo as partes moles ou até estruturas ósseas), braquidactilia
(encurtamento dos dedos), cardiomiopatia dilatada (causada pela
hipertrofia do septo interventricular e do ventrículo esquerdo), “situs
inversus” (inversão de órgãos internos),
dificuldade de interações sociais (muitas vezes, associada à forma
branda de autismo), deficiência nas sensações térmica e mecânica,
diabete tipo II, hipertensão, hipercolesterolemia e baixa estatura linear.
SÍNDROME DE LAURENCE-MOON (SLM) é uma doença rara, autossômica
recessiva [1], doença genética associada com retinite pigmentosa, dígitos
extras, paraplegia espástica, hipogonadismo, retardo mental e baixa
estatura longitudinal.
SÍNDROME DE PRADER-WILLI é um distúrbio genético no qual sete genes
do cromossomo 15 estão faltando ou não são expressos (deleção no
braço longo do cromossomo 15) no cromossomo paterno. Foi descrita
pela primeira vez, em 1956 por Andrea Prader, Heinrich Willi, Alexis
3. Labhart, Andrew Ziegler e Guido Fanconi. A incidência da síndrome é
entre 1 em 12.000 e 1 em 15.000 nascimentos. A distinção do
cromossomo por origem parental é devido ao imprinting, e a síndrome
possui uma síndrome-irmã, a síndrome de Angelman que afeta os genes
"imprintados" maternalmente na região. A síndrome de Prader-Willi é
caracterizada por polifagia, pequena estatura e dificuldades de
aprendizado. Tradicionalmente, a síndrome era diagnosticada
clinicamente. Atualmente, a síndrome é diagnosticada através de
exames genéticos, que são recomendados para recém-nascidos que
apresentem hipotonia.
HIPOTIREOIDISMO:
OS HORMÔNIOS TIREOIDIANOS são necessários para o crescimento e
desenvolvimento normal desde a vida fetal. Sua produção insuficiente
ou sua ação inadequada em nível
celular ou molecular levam ao
hipotireoidismo. Esses hormônios são
necessários para o desenvolvimento
do cérebro na vida fetal e pós-natal.
Neonatologistas e pediatras deparam-se
com problemas do
desenvolvimento da criança, muitos
dos quais já começam na vida intra-uterina.
Atualmente, com a triagem neonatal,
neonatologistas e pediatras podem
evitar danos irreversíveis com tratamento precoce. Também devem
estar atentos para disfunções como as do hipotireoidismo subclínico e
tireoidite de Hashimoto, que podem provocar danos não só no
crescimento, mas também no desenvolvimento neurológico e
psicológico destas crianças e adolescentes. É de extrema importância
investigar a baixa estatura linear devido ao impacto psicológico e social
4. que ela acarreta, e pelo fato de a diminuição da velocidade de
crescimento na criança poder ser um dos primeiros sinais de diversas
doenças. O primeiro objetivo do médico é confirmar se realmente há
baixa estatura linear e se há necessidade de investigar sua causa. Para
isso, dispomos de tabelas e gráficos de peso, altura e velocidade de
crescimento, com os valores normais para cada idade e sexo. Se estiver
abaixo do mínimo considerado normal para a idade e o sexo a altura
linear da criança deve ser investigada. Também deve ser avaliada a
velocidade de crescimento quando muito abaixo do esperado para a
idade e sexo, mesmo que a altura linear ainda esteja normal em relação
ao gráfico, como também quando a altura linear da criança está dentro
do normal no gráfico, mas abaixo da esperada pela altura dos pais
(familial).
A HIPERPLASIA CONGÊNITA DAS SUPRARRENAIS (HCSR): representa um
grupo de doenças genéticas que comprometem a síntese de cortisol
devida à deficiência em uma das enzimas responsáveis pela
esteroidogênese suprarrenal. Na deficiência da 21-hidroxilase (D21OH),
responsável por mais de 90% dos casos de HCSR, a secreção androgênica
suprarrenal está aumentada. A forma clássica de D21OH é tratada com
glicocorticóide, repondo-se o mineralocorticóide quando necessário. O
uso continuado de corticosteróides pode comprometer o crescimento
através de diferentes mecanismos. Por outro lado, a excessiva secreção
adrenocortical dos esteróides sexuais pode levar à redução do tempo de
crescimento, tanto por aceleração da idade óssea quanto pela
possibilidade de indução de puberdade precoce central. A combinação
dos efeitos da corticoterapia e do excesso de esteróides sexuais sobre o
crescimento faz com que as crianças com HCSR estejam sob-risco de
baixa estatura linear.
5. Daí a importância da cuidadosa avaliação e monitorização do
crescimento e da evolução puberal desses pacientes. Igualmente
importante é o emprego de glicocorticóides com menor capacidade em
suprimir o crescimento. Desse modo, acreditamos que a melhor opção
terapêutica para crianças com HCSR é representada pela hidrocortisona
ou acetato de cortisona, empregadas na dose fisiológica e
administrando-se a maior dose pela manhã. Portanto uma das
substâncias mais importantes no bloqueio do GH são os derivados dos
corticosteróides, que fatalmente podem levar à baixa estatura
longitudinal ou linear.
HIPOPARATIREOIDISMO E PSEUDOHIPOPARATIREOIDISMO é uma
doença causada pela falta do hormônio da paratireóide (PTH)
geralmente detectado através da hipocalcemia, seu 1º sintoma visível.
Também é um dos causadores da baixa estatura longitudinal ou linear.
As manifestações clínicas da hipocalcemia aguda incluem espasmos
musculares, tetania, parestesias e convulsões. O eletrocardiograma pode
mostrar alterações da repolarização ventricular e aumento do intervalo
QT. O hipoparatireoidismo crônico pode causar catarata, calcificação em
núcleos da base no sistema nervoso central, com consequentes sintomas
extrapiramidais e retardo mental.
6. A OSTEODISTROFIA HEREDITÁRIA DE ALBRIGHT é caracterizada por
calcificações cutâneas, obesidade, baixa estatura, braquidactilia
associada ao pseudo-hipoparatireoidismo do tipo IA entre outras
alterações hormonais como hipotireoidismo e hipogonadismo.
O diagnóstico é baseado no quadro clínico associado aos achados de
hipocalcemia e níveis elevados de hormônio da paratireóide. Os autores
relatam caso em que a avaliação dermatológica foi de grande
contribuição para o diagnóstico.
A DEFICIÊNCIA DE HORMÔNIO DO CRESCIMENTO (DGH) é uma
substância produzida por uma glândula no cérebro denominada hipófise
(também chamada de pituitária). Este hormônio promove o crescimento
durante a infância e juventude e continua tendo um papel importante
no metabolismo durante a vida adulta. O GH é secretado durante toda a
vida de uma pessoa, e sua atuação mais conhecida está na promoção do
crescimento. Além da altura, entretanto, o GH afeta muitos sistemas no
organismo, incluindo metabolismo de gorduras e proteínas e
metabolismo ósseo. O GH, por esta razão, é prescrito não somente para
crianças, mas também para adultos com DGH.
7. THE LOW HEIGHT SYNDROMES: A SERIOUS COMMITMENT TO GROWTH
IN CHILDREN, CHILD AND YOUTH.

Laurence-Moon Syndrome, Biedl-Bardet Syndrome or Prader- Willi
Syndrome can match with linear short stature (as well as other
endocrine causes such as hypothyroidism, glucocorticoid excess, pseudo-hypoparathyroidism
with Albright hereditary osteodystrophy [OMIM #
103580] and disability GH). Children, Child and youth moderately obese,
but normal with these
conditions tend to have a
slightly advanced age
and bone age advanced
psychological maturation
with increased height
during childhood and
early puberty in most
cases can evolve a low
linear final height in
adult phase. Thus, short linear stature in children with overweight
should be considered the result of an organic cause until the contrary is
proved.
BARDET-BIEDL SYNDROME
8. The most common symptoms of this syndrome are: obesity, retinopathy
(non-inflammatory degenerative disease of the retina) as retinal
dystrophy, polydactyl, kidney problems, mental retardation and
hypogonadism.
Other problems also related to SBS, but less frequent, are the following:
strabismus, cataracts, astigmatism, anosmia (different degrees, with
possible total loss of olfactory sensitivity), hearing problems, syndactyly
(fusion or any degree of adhesion - through membranes - of the fingers
or toes, involving soft tissue or even bone structures), brachydactyly
(shortening of the toes), dilated cardiomyopathy (caused by hypertrophy
of the interventricular septum and the left ventricle), situs inversus
(reversal of internal organs) , difficulty with social interactions (often
associated with a mild form of autism), deficiency in thermal and
mechanical sensations, type II diabetes, hypertension and
hypercholesterolemia, short stature.
LAURENCE-MOON SYNDROME (LMS) is a rare autosomal recessive
disease [1] associated with retinitis pigmentosa genetic disease, the
extra digits, spastic paraplegia, hypogonadism and mental retardation,
short stature longitudinal.
PRADER-WILLI SYNDROME is a genetic disorder in which seven genes on
chromosome 15 are missing or unexpressed (deletion in the long arm of
chromosome 15) on the paternal chromosome. It was first described in
1956 by Andrea Prader, Heinrich Willi, Alexis Labhart, Andrew Ziegler,
and Guido Fanconi. The incidence of the syndrome is between 1 in
12.000 and 1 in 15.000 births. The distinction by parental origin of the
chromosome is due to imprinting, and the syndrome has a sister
syndrome, Angelman syndrome that affects the "imprinted" genes
maternally in the region.
9. Prader-Willi syndrome is characterized by polyphagia, linear short
stature and difficulties in learning. Traditionally, the syndrome was
diagnosed clinically. Currently, the syndrome is diagnosed through
genetic testing, which are recommended for newborns presenting
hypotonia.
HYPOTHYROIDISM:
THE THYROID HORMONES are required for normal growth and
development from fetal life. This insufficient production or inadequate
activity at the cellular or molecular level lead to hypothyroidism. These
hormones are necessary for brain development in fetal and postnatal
life. Neonatologists and pediatricians deal with issues of child
development, many of which start during intrauterine life. Currently,
with neonatal screening, neonatologists and pediatricians can prevent
irreversible damage to early treatment. It should also be alert for
disorders such as subclinical hypothyroidism and Hashimoto's
thyroiditis, which can cause damage not only to growth, but also in
neurological and psychological development of these children and
10. adolescents. It is extremely important to investigate the linear short
stature due to the psychological and social impact that it brings, and the
fact that the reduction in growth velocity in children can be one of the
first signs of several diseases. The first goal of the physician is to confirm
that there really short stature and no need to investigate the cause. For
this, we have tables and charts for weight, height and growth rate, with
normal values for each age and sex. If you are below the minimum
considered normal for age and sex to the child's height should be
investigated. The Growth rate much lower than expected for age should
also be evaluated, even if the height is still normal to the graph, as well
as the child's height is within normal on the chart, but below the
expected height for parents.
CONGENITAL adrenal hyperplasia (CAH) is a group of genetic disorders
that impair cortisol synthesis due to deficiency of the enzymes
responsible for adrenal steroidogenesis. In 21-hydroxylase deficiency
(21OHD), responsible for over 90% of cases of CAH, the adrenal
androgen secretion is increased.
The classical form of 21OHD is treated with glucocorticoid,
mineralocorticoid-replenishing it as needed. Your continued use of
corticosteroids may affect linear growth through different mechanisms.
11. On the other hand, the excessive secretion of adrenal sex steroids may
lead to a reduction in growth time, both by acceleration of bone age as
the possibility of induction of precocious puberty. The combined effects
of corticosteroids and excess sexual steroids on linear growth causes
children with CAH are under-risk of linear short stature. Hence, the
importance of the evaluation and the monitoring of linear growth and
pubertal development in these patients. Equally important is the use of
glucocorticoids with less ability to suppress linear growth. Thus, we
believe that the best therapeutic option for children with CAH is
represented by hydrocortisone or cortisone acetate, used in
physiological dose and administering the higher dose in the morning.
Therefore one of the most important substances in blocking GH is
derived from corticosteroids, which inevitably lead longitudinal or linear
growth low height.
HYPOPARATHYROIDISM AND PSEUDOHYPOPARATHYROIDISM is a
disease caused by a lack of parathyroid (PTH) hormone usually detected
by hypocalcemia, its first visible symptom. It is also one of the causes of
longitudinal or linear low height. The clinical manifestations of acute
hypocalcemia include muscle spasms, tetany, seizures, and paresthesias.
The electrocardiogram may show changes in ventricular repolarization
and increased QT interval. Chronic hypoparathyroidism can cause
cataracts, calcification of the basal ganglia in the central nervous system,
with consequent extrapyramidal symptoms and mental retardation.
OSTEOARTHRITIS OF ALBRIGHT HEREDITARY is characterized by
cutaneous calcification, obesity, short linear stature, brachydactyly
associated with pseudo-hypoparathyroidism type Ia among other
hormonal changes such as hypothyroidism and hypogonadism.
12. The diagnosis is based on clinical findings associated with hypocalcemia
and elevated levels of parathyroid hormone. The authors report the case
where the dermatologic evaluation was of great contribution to the
diagnosis.
DEFICIENCY OF GROWTH HORMONE-GHD is a substance produced by a
gland in the brain called the pituitary gland. This hormone promotes
linear growth during childhood and youth and continues to have an
important role in metabolism during adulthood. The GH is secreted
throughout a person's life, and his most famous role is in promoting
linear growth. Besides the point, however, the GH affects many systems
in the body, including metabolism of fats and proteins and bone
