Please complete this form by typing or writing your answers to those questions which are relevant
to your particular circumstances and then return the completed form by email, fax or post. If
your system supports it, you can also submit the form to us online. If you are completing this
form for a couple who wish us to prepare very similar Wills, only one form is usually needed as the
second spouse/partner's details can be included on page 2 and you can tell us to prepare a mirror
Will for him or her at Question 22.

person, this person would normally be named
as the sole executor of your Will (i.e. the

If the questions we asked about naming a sole beneficiary/executor weren’t relevant to you OR if
you completed them but wish to make provision in your Will for what would happen if the person
named as sole beneficiary/executor should die before you or at the same time as you (say in a joint
accident) then please answer the following additional questions. (We recommend that everyone
does so).

15. If you have young children and wish to
appoint someone to be a legal guardian for
them, please tell us the full name, relationship
to you and address of this person:
(Note. You can appoint more than one person
but if the people you appoint do not live
together there may be disagreements about
where the children concerned reside).
The next question asks about any specific legacies (fixed sums of money) or specific gifts (like
jewellery, furniture or even a house) which you would like to include in your Will as gifts passing
separately from the residue/main part of your estate. (Please note that if the intended beneficiary
of the gift is the same person who will inherit the residue of your estate then there is no need to
include the gift or legacy here. Please also note that in order to pass by your Will the item gifted
must be in your sole name).

the people you have named?
(Note. if not please tell us what the shares will

minors, please tell us what age they will receive
their share if you die before they have reached

and the executors, as trustees, will hold the
person’s share in the estate in trust until he or
she reaches the chosen age)
20. If any of these beneficiaries should die
before you or before they reach the specified
age, would you like any children of the
beneficiary to inherit his/her share?

divided between any surviving beneficiaries)
