1. Wellness and Recovery Action Plan (WRAP)
This is a document drawn up between your manager, on behalf of xx and you, as an
employee, to outline steps we can take to support you and your mental health at work.
This information will be held confidentially and regularly reviewed, in partnership with
you. You only need to tell us details about your health in relation to your role and the
workplace and as far as you feel comfortable. The WRAP is not legally binding but will
help us to agree, together, how to practically support you in your role and address any
health needs.
In your own words, what helps you stay mentally healthy at work? What can you
do to stay mentally healthy? What can your manager do to support this?
In your own words, how does stress / your mental health problem affect you?
How might your symptoms impact on your work?
Impact on work
• Inability to make decisions
• Inability to prioritise workload
• Physical symptoms e.g. headaches, sleeping problems
• Inability to focus
