CASE PRESENTATION
o A 66-year-old woman, who was brought in by his son to ED. (7:00 a.m.)
oPatient was referred to GP in Tohid Hospital as a Simple flank pain and as the
pain was not responded to conventional out.pt therapy including : Lasix 40 mg (IM)
, Morphine Sulfate(IM), Apotel (IV), fluid therapy she was referred back to the
triage for admission (7:50 a.m.)
ONSET, DURATION , LOCATION , FREQUENCY
oShe complained of right flank pain starting over 4 hours ago.
osignificant pain (8/10), right flank and radiated to the back. no related to respiration or
movement. It was worsen at about1 hr. before arriving to ED.
oMinimal chest discomfort from 20 minutes before admission in acute ward of the ED
and the pain not responded to analgesic medications. so our GP decided to referee her for
admission in Acute area for visiting Emergency medicine Specialist.
oAssociated symptoms : Mild Sweating and Nausea. No hematuria ,
The patient was alert and she seemed neither ill nor toxic.
The pulmonary examination was NL.
Heart Examination was NL.
All the pulses was detected and the left upper limb’s pulse was slightly declined compared the
right side .
JVP not distended.
 Dialysis fistula was at left upper limb .
Abdomen: no distended , soft and slight tenderness over right side of Abdomen
 Neurological examination : NL.
PAST HISTORY
Previous disease :Hypertension , ESRD , History of occasional chest discomfort and heart
failure based on her family’s report.
DH: Unknown

oV/s: BP 220/110, PR: 110 , RR: 24 , SO2: 92 % , BS : 145.

O2 supplement if So2 less than 93-95 %
Pain control

Ct Angiography , was not available in Tohid hospital.
Specialist consultant : Vascular surgeon , cardiac surgeon
Analgesia :
fentanyl is choice in ESRD
Anti hypertensive medication :
Labetalol , 10 mg (*4) , infusion

BP 220/110, PR: 110 , RR: 24 , SO2: 92 % , VAS : 8/10 . BS : 145.
Just Before transferring the Pt to Kawsar Hospital :
BP : 140/80 , HR : 72, RR : 22 , So2 : 93% , VAS : 4/10

Management
A) THE FIVE PAIN PEARLS OF AORTIC DISSECTION
Pain Pearl #1: Ask the following 3 questions for all patients with torso pain:
Quality of pain (most commonly “sharp” but highest LR for “tearing”)
Pain intensity at onset
Radiation of pain (back and/or belly)
A 1998 study that reviewed a series of aortic dissection cases showed that for the 42% of
physicians who asked these 3 questions, the diagnosis was suspected in 91%. When
less than 3 questions were asked dissection was suspected in only 49%.
PAIN PEARL #2: THINK OF AORTIC DISSECTION AS THE
SUBARACHNOID HEMORRHAGE OF THE TORSO
Just as the patient who presents with headache and is suspected of suffering from a
subarachnoid hemorrhage, if the patient describes truly abrupt onset of severe torso pain
with maximal intensity at onset, think aortic dissection.
PAIN PEARL #3: SEVERE COLICKY CHEST
PAIN + OPIOIDS = HEIGHTENED SUSPICION
If you find yourself treating your chest pain patient with IV opioids to control their
severe colicky pain, think about the possibility of aortic dissection.
PAIN PEARL #4: MIGRATING PAIN HAS A +LR =
7.6
In addition to the old adage “pain above and below the diaphragm should heighten
your suspicion for aortic dissection”, severe pain that progresses and moves in the same
vector as the aorta, increases the likelihood of aortic dissection.
PAIN PEARL #5: THE PAIN CAN BE INTERMITTENT AS
DISSECTION OF THE AORTIC INTIMA STOPS AND
STARTS
The combination of severe migrating and intermittent pain should raise the suspicion for
aortic dissection.
Painless Aortic Dissection
While IRAD reported a painless aortic dissection rate of about 5%, a more recent study out
of Japan reported that 17% of aortic dissection patients had no pain. These patients
presented more frequently with a persistent disturbance of consciousness, syncope or a
focal neurologic deficit. Cardiac tamponade was more frequent in the painless group as
well.
B) THE CONCEPT OF CP +1 AND 1+ CP IN
AORTIC DISSECTION
The intimal tear in the aorta can devascularize any organ from head to toe including the brain,
heart and kidneys, and so 5% of dissections present as strokes, and these certainly are not the
kind of stroke patients who should be receiving tPA! An objective focal neurologic deficit in the
setting of acute unexplained chest pain has +LR = 33 for aortic dissection. Some of the CP +1
phenomena to think about include:
CP + CVA
CP +paralysis
CP +hoursness
CP + limb schemia
 In addition to thinking of CP +1 it may help to think backwards in time and ask the patient who
presents with end organ damage if they had torso pain prior to their end organ damage
symptoms. For example, for patients who present with stroke symptoms, ask them if they had chest
pain, back pain or abdominal pain before the stroke symptoms.
C) PHYSICAL EXAM PEARLS IN AORTIC
DISSECTION: LOOK, LISTEN & FEEL
Anyone under the age of 40 years who presents to the ED with unexplained torso pain
should be asked if they have Marphan’s Syndrome.
Look. The patient doesn’t always know they have Marphan’s so you need to look for the signs:
1) Arachnodactyly (Elongated fingers): if they look long try to elicit the wrist sign
2) Pectus excavatum : sternal excavation (see image)
3) Lanky limbs
Listen. A new aortic regurgitation murmur has a surprisingly high +LR = 5.
Feel. Feel for a pulse deficit which has a +LR = 2.7.
BP Pitfall: Do not assume that the patient with a normal or low BP does not have an aortic
dissection. We know from the IRAD data that only about half of patients are hypertensive
at initial presentation. Dissection that progress into the pericardium and end up with
tamponade are often hypotensive.
BP Pearl: Patients with dissection who have a wide pulse pressure should be considered
pre-terminal, and usually require immediate surgery.
WHAT ABOUT BILATERAL BP DIFFERENCES IN
AORTIC DISSECTION?
It’s important to know that 19 % of the general population have a BP difference between
arms > 20 and 53 % > 10. This is important to take into account when coming up with a
pretest probability.
D) INITIAL TESTS: CXR, TROPONIN, D-DIMER &
POCUS
It’s not only about wide mediastinum in CXR interpretation of aortic dissection
1/3 of CXRs in aortic dissection are normal to untrained eye, and a common pitfall is to
assume that if the CXR is normal that the patient does not have an aortic dissection.
Tips on reading CXR for aortic dissection:
1st compare to an old CXR to see if there’s been an interval change.
2nd look for more than just a wide mediastinum.
There are about a dozen x-ray findings, but two of them are especially important: Loss of
the aortic knob/aortic-pulmonary window and the calcium sign.
1.Mediastinal widening
2. double-density appearance of the aorta
3. irregular aortic contour
4. inward displacement of atherosclerotic calcification (>1 cm from the aortic margin)
5. deviation of mediastinal structures
6. apical capping, particularly on the left
7. pleural effusion
TROPONIN
About 1/4 of Type A dissections will have a positive troponin, so if you suspect the
diagnosis based on other clinical findings, don’t assume isolated ACS when the troponin
comes back positive.
Remember that < 1/100 of of patients with a dissection will have associated coronary
ischemia in any coronary distribution (most commonly inferior).
Another pitfall is working up a disproportionate number of your STEMI patients for
aortic dissection causing delays to STEMI treatment.
Workup for aortic dissection in STEMI patients should be reserved for those patients
with a significant pretest probability of dissection.
D-DIMER
While D-dimer seems like it might be appealing to help rule out the diagnosis in low risk
patients, for such a rare diagnosis and poor test characteristics of D-dimer for dissection,
guidelines do not recommend the use of D-dimer in the workup of aortic dissection.
POCUS FOR AORTIC DISSECTION
US is used widely by radiologists, cardiologists (echocardiography) and obstetricians. In
recent years its use has become widespread in the field of critical care (emergency
medicine and intensive care ).
Pocus is different from US by radiologist , Diagnostic ultrasound (US) is a safe, rapid
imaging technique just use in combination to medical examination for more specific
evaluation of patients’ chief compliment. It is non-invasive and painless, requires no contrast
medium and no special patient preparation for routine studies.
While the sensitivity of POCUS by ED physicians to detect an intimal flap is only 67%,
the specificity has been shown to be 99-100%.
ULTRASOUND IN ED.
AORTIC DISSECTION DETECTION RISK SCORE (ADD-RS)+ D-DIMER
Low risk : ADD<1 and Neg D-Dimer : Sens. 100 , NPV 100 , Spec. 30.4
Intermediate risk: ADD=1 and negative D-Dimer : Sens. 98.7 , NPV 99.2 , Spec. 35.7
high risk : ADD >1 : do not use D-Dimer for R/o AoD
E) THE IMPORTANCE OF THE CORRECT ORDER
& AGGRESSIVE USE OF IV MEDICATIONS IN
AORTIC DISSECTION MANAGEMENT
First, call the CV surgeon for all aortic dissections (even for Type B dissections), as many
Type B dissection require surgery.
The indications for surgery in Type B dissection are: Malperfusion, ongoing
progression, inability to control BP, and perforation.
NEXT, IN THE FOLLOWING ORDER…
1st: Pain control to counter sympathetic contribution to elevated heart rate and blood
pressure
Fentanyl 25-50 mcg boluses
2ND: HR CONTROL TO A GOAL OF 60 BPM
3RD: BP CONTROL TO A GOAL OF SBP = 110
BP AND HR CONTROL
TAKE HOME PEARLS IN DISSECTION :
1. Do not let concerns about kidney injury delay or prevent necessary imaging with contrast
2. Respect 5 big pain pearls in taking history (quality , intensity at onset , radiation , colicky
torso pain need opioud , migration pain , Start and Stop pain , the worst chest pain in life 0
3. Look for Marphan’s syndrome
4. Think not only about CP+1 like chest pain plus CVA but also 1+CP , like in CVA backward
5. Do not assume that if the CXR is normal that the patient does not have an aortic dissection
and try to know cxr finding
6. Use POCUS to augment your clinical skill and physical exam.
7. Do not be misled by a positive troponin , ¼ of dissection is positive.
8. Negative D-Dimer is just only 100% in very low risk patient with ADD<1
9. When it come to management please fallow treatment of pain then HR then BP.

Pearls and pitfall in aortic disection

  • 3. CASE PRESENTATION o A 66-year-old woman, who was brought in by his son to ED. (7:00 a.m.) oPatient was referred to GP in Tohid Hospital as a Simple flank pain and as the pain was not responded to conventional out.pt therapy including : Lasix 40 mg (IM) , Morphine Sulfate(IM), Apotel (IV), fluid therapy she was referred back to the triage for admission (7:50 a.m.)

ONSET, DURATION , LOCATION , FREQUENCY oShe complained of right flank pain starting over 4 hours ago. osignificant pain (8/10), right flank and radiated to the back. no related to respiration or movement. It was worsen at about1 hr. before arriving to ED. oMinimal chest discomfort from 20 minutes before admission in acute ward of the ED and the pain not responded to analgesic medications. so our GP decided to referee her for admission in Acute area for visiting Emergency medicine Specialist. oAssociated symptoms : Mild Sweating and Nausea. No hematuria ,
  • 5. The patient was alert and she seemed neither ill nor toxic. The pulmonary examination was NL. Heart Examination was NL. All the pulses was detected and the left upper limb’s pulse was slightly declined compared the right side . JVP not distended.  Dialysis fistula was at left upper limb . Abdomen: no distended , soft and slight tenderness over right side of Abdomen  Neurological examination : NL.
  • 6. PAST HISTORY Previous disease :Hypertension , ESRD , History of occasional chest discomfort and heart failure based on her family’s report. DH: Unknown
  • 7. INFORMATION BASED ON TRIAGE NOTE o flank pain , non response to out.pt therapy. ostage 2 of Triage (ESI). oV/s: BP 220/110, PR: 110 , RR: 24 , SO2: 92 % , BS : 145.

No intra-Abd fluid Small kidney and Suspected right mild hydronephrosis Flap in Abdominal Aorta

INITIAL MANAGEMENT 8:15 A.M : o Follow the Airway, Breathing, Circulation (ABC) approach Iv- line & lab test : CBC , RFT, BS, Troponin,VBG, coagulation profile Cardiac monitoring , pulse oximetry. O2 supplement if So2 less than 93-95 % Pain control Blood pressure control : goal ? Heart rate control : goal ? Ct Angiography , was not available in Tohid hospital. Specialist consultant : Vascular surgeon , cardiac surgeon

A) THE FIVE PAIN PEARLS OF AORTIC DISSECTION Pain Pearl #1: Ask the following 3 questions for all patients with torso pain: Quality of pain (most commonly “sharp” but highest LR for “tearing”) Pain intensity at onset Radiation of pain (back and/or belly) A 1998 study that reviewed a series of aortic dissection cases showed that for the 42% of physicians who asked these 3 questions, the diagnosis was suspected in 91%. When less than 3 questions were asked dissection was suspected in only 49%.

PAIN PEARL #2: THINK OF AORTIC DISSECTION AS THE SUBARACHNOID HEMORRHAGE OF THE TORSO Just as the patient who presents with headache and is suspected of suffering from a subarachnoid hemorrhage, if the patient describes truly abrupt onset of severe torso pain with maximal intensity at onset, think aortic dissection.
  • 25. PAIN PEARL #3: SEVERE COLICKY CHEST PAIN + OPIOIDS = HEIGHTENED SUSPICION If you find yourself treating your chest pain patient with IV opioids to control their severe colicky pain, think about the possibility of aortic dissection.
  • 26. PAIN PEARL #4: MIGRATING PAIN HAS A +LR = 7.6 In addition to the old adage “pain above and below the diaphragm should heighten your suspicion for aortic dissection”, severe pain that progresses and moves in the same vector as the aorta, increases the likelihood of aortic dissection.
  • 27. PAIN PEARL #5: THE PAIN CAN BE INTERMITTENT AS DISSECTION OF THE AORTIC INTIMA STOPS AND STARTS The combination of severe migrating and intermittent pain should raise the suspicion for aortic dissection. Painless Aortic Dissection While IRAD reported a painless aortic dissection rate of about 5%, a more recent study out of Japan reported that 17% of aortic dissection patients had no pain. These patients presented more frequently with a persistent disturbance of consciousness, syncope or a focal neurologic deficit. Cardiac tamponade was more frequent in the painless group as well.

B) THE CONCEPT OF CP +1 AND 1+ CP IN AORTIC DISSECTION The intimal tear in the aorta can devascularize any organ from head to toe including the brain, heart and kidneys, and so 5% of dissections present as strokes, and these certainly are not the kind of stroke patients who should be receiving tPA! An objective focal neurologic deficit in the setting of acute unexplained chest pain has +LR = 33 for aortic dissection. Some of the CP +1 phenomena to think about include: CP + CVA CP +paralysis CP +hoursness CP + limb schemia  In addition to thinking of CP +1 it may help to think backwards in time and ask the patient who presents with end organ damage if they had torso pain prior to their end organ damage symptoms. For example, for patients who present with stroke symptoms, ask them if they had chest pain, back pain or abdominal pain before the stroke symptoms.

C) PHYSICAL EXAM PEARLS IN AORTIC DISSECTION: LOOK, LISTEN & FEEL Anyone under the age of 40 years who presents to the ED with unexplained torso pain should be asked if they have Marphan’s Syndrome. Look. The patient doesn’t always know they have Marphan’s so you need to look for the signs: 1) Arachnodactyly (Elongated fingers): if they look long try to elicit the wrist sign 2) Pectus excavatum : sternal excavation (see image) 3) Lanky limbs Listen. A new aortic regurgitation murmur has a surprisingly high +LR = 5. Feel. Feel for a pulse deficit which has a +LR = 2.7.

  • 31. BP Pitfall: Do not assume that the patient with a normal or low BP does not have an aortic dissection. We know from the IRAD data that only about half of patients are hypertensive at initial presentation. Dissection that progress into the pericardium and end up with tamponade are often hypotensive. BP Pearl: Patients with dissection who have a wide pulse pressure should be considered pre-terminal, and usually require immediate surgery.
  • 32. WHAT ABOUT BILATERAL BP DIFFERENCES IN AORTIC DISSECTION? It’s important to know that 19 % of the general population have a BP difference between arms > 20 and 53 % > 10. This is important to take into account when coming up with a pretest probability.
  • 33. D) INITIAL TESTS: CXR, TROPONIN, D-DIMER & POCUS It’s not only about wide mediastinum in CXR interpretation of aortic dissection
  • 34. 1/3 of CXRs in aortic dissection are normal to untrained eye, and a common pitfall is to assume that if the CXR is normal that the patient does not have an aortic dissection. Tips on reading CXR for aortic dissection: 1st compare to an old CXR to see if there’s been an interval change. 2nd look for more than just a wide mediastinum. There are about a dozen x-ray findings, but two of them are especially important: Loss of the aortic knob/aortic-pulmonary window and the calcium sign.

TROPONIN About 1/4 of Type A dissections will have a positive troponin, so if you suspect the diagnosis based on other clinical findings, don’t assume isolated ACS when the troponin comes back positive. Remember that < 1/100 of of patients with a dissection will have associated coronary ischemia in any coronary distribution (most commonly inferior). Another pitfall is working up a disproportionate number of your STEMI patients for aortic dissection causing delays to STEMI treatment. Workup for aortic dissection in STEMI patients should be reserved for those patients with a significant pretest probability of dissection.

In recent years its use has become widespread in the field of critical care (emergency medicine and intensive care ). Pocus is different from US by radiologist , Diagnostic ultrasound (US) is a safe, rapid imaging technique just use in combination to medical examination for more specific evaluation of patients’ chief compliment. It is non-invasive and painless, requires no contrast medium and no special patient preparation for routine studies. While the sensitivity of POCUS by ED physicians to detect an intimal flap is only 67%, the specificity has been shown to be 99-100%.

The indications for surgery in Type B dissection are: Malperfusion, ongoing progression, inability to control BP, and perforation.

Respect 5 big pain pearls in taking history (quality , intensity at onset , radiation , colicky torso pain need opioud , migration pain , Start and Stop pain , the worst chest pain in life 0 3. Look for Marphan’s syndrome 4. Think not only about CP+1 like chest pain plus CVA but also 1+CP , like in CVA backward 5. Do not assume that if the CXR is normal that the patient does not have an aortic dissection and try to know cxr finding 6. Use POCUS to augment your clinical skill and physical exam. 7. Do not be misled by a positive troponin , ¼ of dissection is positive. 8. Negative D-Dimer is just only 100% in very low risk patient with ADD<1 9. When it come to management please fallow treatment of pain then HR then BP.
