events
•5% of inpatients suffer an adverse incident with 20%
being due to medicines
•In Northern Ireland this equates to a health-care resource
loss of over £60 million in terms of bed days.

patients over 65 years of age
•2005 - present Pharmaceutical Clinical Effectiveness
•2006 - Safer Patient Initiative (International Healthcare
Institute (IHI) )
•2010 - present Innovation Programme (DHSSPS)
•2013 – EIP 3 star status (DHSSPS)
•2015 – Establishment of MOIC

5. • Reduced length of stay by 2 days
• Reduced readmission rate
• Reduced length of stay on readmission 5.8 days
• Reduction in 4.2 errors per admission kardex
• Reduced medicines administration error rate
• Reduced discharge error rate from 22% to less than 2%
