the challenge in keeping Western-European office buildings
comfortable until the end of the 21st century … large decreases
in cooling costs (up to 30%) caused by the introduction of
(external) shading and increased night-time ventilation in
actively cooled buildings, and the improvements in working
conditions in free-running buildings caused by moving workers
to cooler locations and splitting workdays in morning and
evening shifts.

Future healthcare will tackle disease by moving away from a ‘one size fits all
approach’, by integrating data on the entire dynamic biological makeup of
each individual, as well as the lifestyle and environmental factors that
interact with this makeup to develop a complex and individual phenotype.
This integrated information will be used to identify the most appropriate
healthcare for individual citizens.

No solution [to the problem of poverty] is so effective as providing income to the poor. Whether in
the form of food, housing, health services, education or money, income is an excellent antidote for
deprivation.

…where 50 % of the total budget is spent on 5% of the population…
…and where less than 5% is spent on prevention…?!
What we should have…
• A system that saves and fosters Health
• Collectively
• Personally
• Predictive
• Preventive
• And that supplies , when saving health fails, Care for
the diseased,of the highest possible quality
What is “Quality of Care”
‘The degree to which health services for individuals and populations
increase the likelihood of desired health outcomes and are consistent with
current professional knowledge’ (IOM)
= a worldwide challenge
What is “Quality of Care” ?
“Quality” contains various elements
• Effective
• Efficient
• Continuous
• Safe
• Patientcentered
• Timely
• Accessible
• Integrated

Traditional Systems
• Improvement by decomposing
performance and management
into component elements, and
subsequently recomposing it by
integrating the designed
solution for each element
• Characteristics:
• Ability to de- and recompose
the elements of the system;
“designed”
• Linearity, predictability, “dead”
• Can be complicated, but not
“complex”
• Power with authority and
resources to (re)design the
system
• Works well in automotive,
manufacturing, retail,…
CAS
• Can NOT be addressed thru
hierarchical de/recomposition!
• Characteristics:
• Non-linear & dynamic : system
behaviour may appear to be
random/chaotic
• Composed by individual agents
who’s behaviour is based on
physical, psychological, or social
rules rather than on demands of
the system
• Agents are often conflicting and
intelligent : the overall system
behaviour learns and adapts
• Hence may lead to self-
organisation : patterns emerge
rather than being designed
• No single point of control : no one
is in charge
147
148
Real challenges…
• Do we accept that “Healthcare” is ( or at least behaves
like) a CAS (Complex Adaptive System ) with it’s own
governing laws ?
• And, therefore, is in need for a longterm purpose
(“Ikigaï”) and framework design, and should allow for the
role of non-linear thinking? E.g., allowing for Serendipity
• If Integrated Care is that purpose, should the obvious
first step not be...(re)integration..?
• (Hence, “networking” is (only) a means, not a goal)
• Do we accept than, that : “collaboration”, “networking”,…
and therefore Healthcare, is foremost dependent on
human interaction (sometimes two are sufficient…)

Traditional Systems • Improvement by decomposing performance and management into component elements, and subsequently recomposing it by integrating the designed solution for each element • Characteristics: • Ability to de- and recompose the elements of the system; “designed” • Linearity, predictability, “dead” • Can be complicated, but not “complex” • Power with authority and resources to (re)design the system • Works well in automotive, manufacturing, retail,… CAS • Can NOT be addressed thru hierarchical de/recomposition! • Characteristics: • Non-linear & dynamic : system behaviour may appear to be random/chaotic • Composed by individual agents who’s behaviour is based on physical, psychological, or social rules rather than on demands of the system • Agents are often conflicting and intelligent : the overall system behaviour learns and adapts • Hence may lead to self- organisation : patterns emerge rather than being designed • No single point of control : no one is in charge

  • 144. Real challenges… • Do we accept that “Healthcare” is ( or at least behaves like) a CAS (Complex Adaptive System ) with it’s own governing laws ? • And, therefore, is in need for a longterm purpose (“Ikigaï”) and framework design, and should allow for the role of non-linear thinking? E.g., allowing for Serendipity • If Integrated Care is that purpose, should the obvious first step not be...(re)integration..? • (Hence, “networking” is (only) a means, not a goal) • Do we accept than, that : “collaboration”, “networking”,… and therefore Healthcare, is foremost dependent on human interaction (sometimes two are sufficient…)

  1. big Pharma companies to partner with multiple other companies (start ups) to lead and/or execute leading projects
Land use Water management Air management Land management Remote sensing
