G o a l  # 1   H e a lt h  a n d W e ll n ess I will learn how to keep fit and eat right at school. This goal is important because I would like to be healthy and fit so I don’t feel so tired. I will work on this goal by eating well at lunch time at school and I will work to be fit.  Who can help me myself and my mom and dad and some other people.
G o a l # 2   S c ho o l w o r k I will try to work independently with out an S E A when I know I can do it .  This goal is important because I have to be independent for the future. I will work on this goal by challenging myself to not have an S E A.  Who can help me: Me , teachers and S E A.
G o a l  #3   T r a n s it i ons P l a n n i n g I will get my grad photos taken. This goal is important to me because I would like to get them done so I can have some when I leave the school. I will work on this goal by going to get them done. Who can help me: Me
G o a l  #4   T r a n s it i ons P l a n n i n g i will look into college programs  (early childhood education)  Kwantlen college – Douglas college – Surrey/Delta continuing education.
G oa l  #5   S e lf - D e te r mi n ati o n I will learn about my learning  needs by reviewing my IEP  and to explain my disability. This goal is important because it is apart of my life and what I go throw. I will work on this goal by doing stuff I can do  Who can help me: Me , My mom and dad
G o a l #6  C o m m u nica t i o n  s k i l l s I will speak with a clear voice and use active listing skills  This goal is important because I talk all the time and I’m just so outgoing I will work on this by listing to myself  Who can help me: Me, My friends, Teachers, SEAS, Mom and Dad
G o a l #7  S ki l ls f o r  c o m m u ni t y L i v i ng I will cook simple meals independently gets a drivers license volunteer in the community. This goal is important because I would like to do those things.  I will work on this goal by doing it. Who can help me: My mom, dad, teachers, S.E.A.S
