Medical abortion pdf
1. Medical Abortion
As the name suggests, medical abortions do not involve surgery or other invasive methods but
rely on medications to end pregnancy. A medical abortion involves taking the drug mifepristone;
often called 'the abortion pill,' its generic name is RU-486 and its brand name is Mifeprex
Mifepristone is not available over the counter and must be provided by a health care
professional. A woman seeking a medical abortion can obtain one through a doctor's office or
clinic and should expect two or more visits to complete the process, as another drug, misoprostol,
must be taken to terminate the pregnancy.
Mifepristone is prescribed in the first trimester and is FDA-approved for use up to 49 days (7
weeks) after a woman's last period. Although considered off label (not FDA-approved), some
providers may opt to use it up to 63 days (9 weeks) after the first day of a woman's last period,
although its effectiveness is diminished after 7 weeks. In 2005, medical abortions made up 13%
of all abortions and 22% of abortions that took place within the first 8 weeks of pregnancy.
. RU-486 (Mifepristone): within 4-7 weeks of the last menstrual period. Also known as "the
abortion pill."
1.This drug interferes with levels of progesterone, a hormone that keeps the fetus implanted in
the wall of the uterus. The woman is prescribed progesterone and then returns to the clinic two
days later to receive a prostaglandin drug that induces labor and expels the dead fetus. A third
visit may be required if the baby is not expelled, at which time a woman has a 5-8% likelihood of
needing a surgical abortion to complete the process. RU-486 is documented to be unsafe for
women.
2. Methotrexate and Misoprostol Methotrexate is used for treatment of cancer, and Misoprostol
is used for ulcer treatment. In a chemical abortion, these two drugs are used in combination.
Methotrexate causes cells in the placenta (the organ that nourishes the fetus) to die. Misoprostol
empties the fetus from the uterus by causing the uterus to contract and push the fetus out.
Methotrexate is a drug used in chemotherapy and has the potential for serious liver toxicity.
3. "Morning After" Pill: sometimes used in rape cases Up to 72 hours after intercourse, a woman
is administered large doses of birth control pills (or levonorgestrel, also known as Plan B) to
prevent the embryo from implanting in the uterus wall. Twelve hours after the first dose, a
second dose is given. Large doses of birth control pills work to prevent ovulation and hinder
sperm motility.

2. HOW THE PILL WORKS
Mifepristone blocks the effects of progesterone, a hormone necessary to sustain pregnancy.
Without progesterone, the fertilized egg detaches from the uterine wall, the cervix softens and
opens and the uterine lining begins to break down. Two days after taking it, women are given the
drug misoprostol, an anti-ulcer drug that is also known by the brand name Cytotec. In small
doses, misoprostol causes contractions in the uterus, helping to expel the embryo. Women
typically remain in a doctor's office or clinic office up to four hours after taking misoprostol.
Most are then released to go home.
Medical Abortion is a safe and effective way to end an early pregnancy by using medicine. You
must be at least 18 years of age to receive a medical abortion.
You'll meet with the doctor and then take the first pill (Mifeprex) in the office. Within 2-3 hours
you will use the second pills (Misoprostol) at home. The combination of the two medicines will
cause miscarriage and in most cases end the pregnancy within a few hours of using the second
medicine at home.
You'll return in 3 weeks to make sure everything worked properly. We provide free womb
cleaning pills and free deliveries of the medicine to those who can't come in. It's FDA approved.
The medical abortion is a way to end pregnancy without using instruments or surgery. It's done
with medicine up to 20 weeks from the first day of the last period.
Why do some women prefer the medical abortion?
• No anesthesia or surgery required
• It can be done very early in the pregnancy
• They feel more in control
• They may feel it is less invasive
• More "natural"
• They may feel it's more like a miscarriage
• It's more private
• They have the abortion at home
Who can choose medical abortion?
You must be at least 18 years old. You may choose the
abortion if you are less than 20 weeks pregnant from the
first day of your last period.
3. How does the medical abortion work?
There are two steps: Our doctor gives you a Mifeprex tablet to take in the office and 3 to 4 hours
later you will use a second medicine, Misoprostol, at home as a suppository. In combination the
drugs cause a miscarriage.
Step One: Mifeprex blocks the hormone progesterone. Without progesterone the lining of the
uterus breaks down ending the pregnancy.
Step Two: Misoprostol makes the uterus contract and empty causing a miscarriage.
How long does it take?
Some women (2-5%) may begin to bleed after using the Mifeprex in the office. Even so you
must use Misoprostol as instructed. Most women will bleed and pass the pregnancy within
several hours of inserting the Misoprostol at home.
How effective is it?
Medical abortion is approximately 95% effective. Mifeprex and Misoprostol will cause serious
birth defects to this pregnancy. Approximately three weeks later: You must return to the office to
confirm that the abortion is complete.
How does it feel?
You may experience some or all of the following:
• some cramps
• nausea or vomiting
• diarrhea
Call the Physician if you:
• Soak more than two sanitary pads per hour for 2 consecutive hours
• Bleed heavily for more than 12 hours in a row
• Pass clots larger than a size of a lemon
• Run a temperature over 100.4
• Have severe pain not relieved by pain medication
• Have an allergic reaction to any medication (itching, rash or hives)
• Develop a foul vaginal odor
4. Will I need a follow-up?
Yes. Your Physician needs to make sure the abortion is complete.
What if I don't have a complete abortion after taking the
medicine?
Your Physician will decide what option is best for you. You may:
• Take more misoprostol
When will I get my period?
You should expect your next menstrual cycle four to eight weeks after your miscarriage. If you
start birth control pills you will know exactly when your period will begin.
When can I have sex again?
You should not have vaginal intercourse until you are cleared by the Physician at your follow-up
visit. You could get pregnant right away after the miscarriage.
