Sulla base dei dati disponibili, gli eventi che si susseguono dal momento dell’infezione alla fase acuta di malattia possono essere schematizzati come segue. 1. Sebbene siano stati pubblicati risultati contrastanti sull’effetto antivirale delle risposte innate intracellulari, i dati derivati da studi nell’infezione naturale umana indicano che il pathway degli interferoni di tipo I risulta scarsamente attivato da HBV 2. Anche sulla cinetica delle risposte NK i dati non sono univoci, ma suggeriscono comunque che le cellule NK si attivino prima o in concomitanza del picco viremico, contribuendo comunque al controllo iniziale dellinfezione 3. Le risposte adattative si innalzano invece tempestivamente non appena il virus inizia a replicare attivamente 4. Gran parte del virus viene inizialmente eliminato dall’organismo attraverso meccanismi non citopatici citochino-mediati, a cui contribuiscono sicuramente i linfociti T, ma probabilmente anche le cellule NK 5. L’eliminazione citolitica delle rimanenti cellule infettate, ad opera dell’attività citotossica dei linfociti CD8, è sicuramente essenziale per il conseguimento del controllo finale e persistente dell’infezione
Nei casi di infezioni autolimitate, le risposte immunitarie adattative umorali e cellulo-mediate si susseguono in modo efficiente e coordinato, per cui le risposte T linfocitarie di fase acuta risultano solitamente intense (rispetto alle risposte misurabili nelle fasi croniche dell’infezione), multispecifiche (cioè dirette simultaneamente verso varie sequenze [epitopi] del virus) e orientate verso una prevalente produzione di citochine a effetto antivirale. Queste risposte di fase acuta esitano nella maturazione di risposte di memoria T linfocitarie di lunghissima durata, le cui caratteristiche suggeriscono che esse siano mantenute dal continuo stimolo operato da piccole quantità di antigene. Queste osservazioni sono in accordo con l’evidenza secondo la quale piccole quantità di virus persistono nell’organismo anche dopo la completa risoluzione di un’epatite acuta autolimitata, configurando il quadro dell’infezione occulta. Il concetto che ne deriva è che la guarigione da un’epatite acuta non significa completa eradicazione del virus, ma piuttosto capacità del sistema immunitario di tenere sotto stretto controllo piccole quantità di virus che persistono indefinitamente nell’organismo infettato.
