•   What is the Army MWR Services Survey?
•   The Army MWR Services Survey is an approved Army survey about Family and Morale, Welfare and Recreation (MWR) activities and
    experiences you have had living and working within the Army community and the support services and programs you would find most
    helpful to you. We are interested in your opinions about MWR programs so that MWR can provide quality services and programs.

•   Who will be selected to participate in the survey?
•   Approximately 70,000 Active Duty members, 49,000 spouses of Active Duty Soldiers, 73,000 DoD Civilians and 31,000 Retirees at 75
    Army installations worldwide have been randomly selected to participate in the survey.

•   When will the survey be administered?
•   All members selected in the sample will be notified by mail and via e-mail, wherever applicable, in late February 2012 that they have
    been selected to participate in the Army MWR Services Survey which will be active 27 Feb 2012. All members in the sample will be
    mailed their survey packages in Feb/Mar 2012.

•   Is there a web option to take the survey?
•   Yes. Every survey package will contain a url link to the web survey and a respondent ID that will be unique to each respondent.
    Respondents shall be able to login to the web survey using their unique respondent ID. The survey url link is ArmyMWRSurvey.com.

•   How long will it take to complete the survey?
•   It should take participants approximately 25-30 minutes to complete the survey.

•   Whom can I contact if I have questions about the survey?
•   Please contact ICF toll-free at 1-877-471-9785 or email us at MWRSurvey@icfi.com if you have questions about the survey.

•   How can I be sure that my responses will remain confidential?
•   There are no questions on the survey that allow individual identification. Responses will be analyzed and reported as a group (e.g.,
    spouses, active duty).

•   How will the survey results be used?
•   The survey results will be used to the benefit of respondents and those they represent to improve their installation's MWR services. The
    results from this survey can make a difference in the quality of life for Army families and the entire Army community!
