Este documento discute hiponatremia, definindo-a como um distúrbio do metabolismo da água e não do sódio. Apresenta suas principais causas, incluindo uso de diuréticos tiazídicos, hipovolemia, síndrome de secreção inapropriada de hormônio antidiurético (SIHAD) e mutações no receptor V2. Explica os mecanismos fisiológicos envolvidos na regulação da osmolaridade plasmática e na geração e excreção renal de água livre.

Este estudo clássico de Robertson e colaboradores demonstra como o pico de vasopressina coincide com o desenvolvimento de náusea após injeção de apomorfina. Como vocês podem perceber, não houve alteração da PA ou da osmolaridade que justificassem esse pico de HAD. Gostaria de chamar a atenção para a magnitude deste pico; desidratação e hiperosmolaridade geralmente elevam o HAD para cerca de 5 a 10 pg /ml enquanto que aqui a náusea elevou o HAD para cerca de 400 pg /ml!!
Severe hyponatremia developed after elective surgery in 15 previously healthy women who subsequently either died or had permanent brain damage. The mean age was 41 years (range, 22 to 66), and the preoperative serum sodium level was 138 mmol per liter. All the patients recovered from anesthesia, but about 49 hours after surgery, when the average plasma sodium level was 108 mmol per liter, grand mal seizures, followed by respiratory arrest requiring intubation, developed in all 15. At that time, the urinary sodium level and the osmolality averaged 68 mmol per liter and 501 mOsm per kilogram, suggesting inappropriate secretion of antidiuretic hormone. In 10 of 15 patients, an acute cerebral vascular disorder was suspected, leading to a delay in treatment and multiple diagnostic studies, including CT scanning, cerebral angiography, and open-brain biopsies. The net postoperative fluid retention was 7.5 liters, and when correction of the serum sodium level was initiated, the rate of correction was less than 0.7 mmol per liter per hour. Histologic studies of the brain in five patients were not diagnostic, and no patient had any evidence of central pontine myelinolysis on the basis of autopsy, brain biopsy, or CT scanning. Seven patients recovered from coma after the serum sodium level was increased to 131 mmol per liter, but coma recurred two to six days later and ended in either death or a persistent vegetative state. Overall, 27 percent of the patients died, 13 percent had limb paralysis, and 60 percent were left in a persistent vegetative state.

Por fim, ao tratamento crônico. Os princípios gerais são os mesmos: limitar a ingesta de água e tentar aumentar a excreção renal de água.
Vejamos como isto funciona. Um indivíduo com SIHAD e osmolaridade urinária fixa em torno de 600 mosm por litro que precisar excretar 600 mosm de soluto por dia irá fazê-lo em 1 litro de urina. No entanto, ao aumentarmos a ingesta e consequentemente a excreção de solutos para 900 mosm, a excreção obrigatória de água livre irá aumentar proporcionalmente. Assim, se dobrarmos a ingesta de solutos para 1200 mosm, dobraremos a excreção de água para 2 litros.
Em geral, estas duas medidas de restrição hídrica e dieta rica em solutos são suficientes para manter o paciente com SIHAD longe de problemas. No entanto, nos pacientes com SIHAD mais severa e osmolaridade urinária acima de 600, medidas adicionais como a administração de furosemida são essenciais.
Quando isto não resolve, podemos utilizar medicações que tem como efeito colateral antagonizar a ação do HAD e promover uma espécie de diabete insipidus nefrogênico, aumentando a excreção de água livre. O carbamato de lítio é uma opção pouco utilizada pois é bastante tóxico, inclusive para os rins. A demeclociclina é um derivado da tetraciclina que pode ser usado sem maiores problemas em adultos mas deve ser evitada em crianças por interferir com o desenvolvimento ósseo.
Avanços no conhecimento da biologia molecular do receptor de HAD,
… principalmente o conhecimento da estrutura tridimensional deste receptor através de radiografia cristalográfica, possibilitaram a criação de drogas não-peptídeas inteligentes, capazes de se ligar ao receptor e não ativá-lo, funcionando portanto como antagonistas.
Conivaptan is a combined V1aR and V2R antagonist, whereas the others are selective V2R antagonists. All agents of this class are inhibitors of the cytochrome P450 3A4 (CYP3A4) system, but conivaptan appears to be the most potent in this regard. Although the drug is orally active, to minimize the possibility of drug interactions, the FDA has restricted its distribution to a parenteral form for short-term (4-day) in-hospital use only. The remaining V2R antagonists appear to have more limited CYP3A4 interactions and are currently being developed for long-term oral use.
Schrier et al. conducted two identical trials to test the efficacy of oral tolvaptan in patients with euvolemic and hypervolemic hyponatremia. Suitably named SALT-1 and SALT-2 for Study of Ascending Levels of Tolvaptan in Hyponatremia, an American and international cohort of 448 patients was randomly assigned to tolvaptan or placebo. In SALT-1, sodium rose from 128.5 +/- 4.5 mmol/L at baseline to 133.9 +/- 4.8 mmol/L on day 4 and 135.7 +/- 5.0 mmol/L on day 30 in patients who received active drug but only from 128.7 +/- 4.1 to 129.7 +/- 4.9 and 131.0 +/- 6.2 mmol/L, respectively, in the placebo group. The rise in sodium exceeded 12 mmol/L per 24 h in only four of the 225 patients who received tolvaptan. Dry mouth and increased thirst and urination were more frequent in the treatment group, and the remainder of adverse effects was similar in both groups. Serious adverse effects potentially a result of tolvaptan treatment occurred in eight patients: Hypotension, dizziness, syncope, acute renal failure, and hypernatremia. This study clearly demonstrated that orally administered tolvaptan effectively and safely treats euvolemic and hypervolemic hyponatremia.
