   discharge plan that is being used and

   Services are monitored against predetermined
    service benchmarks at least once per quarter.

Our community uses a universal process and/or tool
       at intake that helps us assess the most
  appropriate resources for the consumer's needs.

 In my opinion, the intake process is used the same

Our community uses a universal process and/or tool at intake that helps us assess the most appropriate resources for the consumer's needs.
