Chronic heart failure (HF) is a prevalent world-wide. Angiotensin receptor blockers (ARBs) are
widely prescribed for chronic HF although their role is controversial.
OBJECTIVES
To assess the benefit and harm of ARBs compared with ACE inhibitors (ACEIs) or placebo on
mortality, morbidity and withdrawals due to adverse effects in patients with symptomatic HF and left
ventricular systolic dysfunction or preserved systolic function.
Twenty two studies evaluated the effects of ARBs in 17,900 patients with a LVEF ≤40% (mean 2.2
years). ARBs did not reduce total mortality (RR 0.87 [95% CI 0.76, 1.00]) or total morbidity as
measured by total hospitalisations (RR 0.94 [95% CI 0.88, 1.01]) compared with placebo.Total
mortality (RR 1.05 [95% CI 0.91, 1.22]), total hospitalisations (RR 1.00 [95% CI 0.92, 1.08]), MI (RR
1.00 [95% CI 0.62, 1.63]), and stroke (RR 1.63 [0.77, 3.44]) did not differ between ARBs and ACEIs
but withdrawals due to adverse effects were lower with ARBs (RR 0.63 [95% CI 0.52, 0.76]).
Combinations of ARBs plus ACEIs increased the risk of withdrawals due to adverse effects (RR 1.34
[95% CI 1.19, 1.51]) but did not reduce total mortality or total hospital admissions versus ACEI
alone.Two placebo-controlled studies evaluated ARBs in 7151 patients with a LVEF >40% (mean 3.7
years). ARBs did not reduce total mortality (RR 1.02 [95% CI 0.93, 1.12]) or total morbidity as
measured by total hospitalisations (RR 1.00 [95% CI 0.97, 1.05]) compared with placebo.
Withdrawals due to adverse effects were higher with ARBs versus placebo when all patients were
pooled irrespective of LVEF (RR 1.06 [95% CI 1.01, 1.12]).
AUTHORS' CONCLUSIONS:
In patients with symptomatic HF and systolic dysfunction or with preserved ejection fraction, ARBs
compared to placebo or ACEIs do not reduce total mortality or morbidity. ARBs are better
tolerated than ACEIs but do not appear to be as safe and well tolerated as placebo in terms of
withdrawals due to adverse effects. Adding an ARB in combination with an ACEI does not reduce
total mortality or total hospital admission but increases withdrawals due to adverse effects compared
with ACEI alone.

16. Tolerabilitat: balanç
• La TOS no és tan comú amb IECA com es pensa
(2%-10%): (3,2% ARA-2 vs 9% IECA)
- És notable en ICC sovint degut a la patologia subjacent
-No tota la tos requereix discontinuació de tractament.
- Característiques: NO desapareix antitusígens, apareix 1
setmana a 6-10 mesos després de l’inici, desapareix 1 a 4
dies i tornar aparèixer si introduïm un IECA.
-Abans de canviar de IECA a ARA II degut a tos
intractable, considerar si un fc SRA és la única opció ?
Ex. Calciantagonista o tiazidics en HTA no complicada ?

17. Quina és la proporció ESPERADA ?
Assaig Clínics 2 -10% IECA suspèn per tos (ONTARGET 3%).
S’assumeix que el 80% de pacients que comencen un IECA
continuaran amb aquest i un 20% s’haurà de canviar
a ARA II per intolerància a IECA (Opinió d’experts)

population-based longitudinal analysis
• 32 312 new users of either medication (mean age 76.1 years, median creatinine level
92 mol/L), 1750 (5.4%) received combination therapy.
• 1512 (86.4%) of the patients who were given combination therapy did not have trial-
established indications such as heart failure or proteinuria.
•Renal dysfunction was more common among patients given combination therapy
(5.2 [95% CI 3.4 to 7.9] events per 1000 patients per month) than among patients
given monotherapy (2.4 [95% CI 2.2 to 2.7] events per 1000 patients per month)
(adjusted hazard ratio [HR] 2.36, 95% CI 1.51 to 3.71).
•Hyperkalemia was also more common among patients given combination therapy
(2.5 [95% CI 1.4 to 4.3] events per 1000 patients per month) than among patients
given monotherapy (0.9 [95% CI 0.8 to 1.0] events per 1000 patients per month)
(adjusted HR 2.42, 95% CI 1.36 to 4.32).

of Benicar(olmesartan) and cardiovascular events
Safety Announcement
[06-11-2010] The U.S. Food and Drug Administration (FDA) is evaluating data
from two clinical trials in which patients with type 2 diabetes taking the blood
pressure medication, Benicar (olmesartan)* had a higher rate of death from a
cardiovascular cause compared to patients taking a placebo.
FDA's review is ongoing and the Agency has not concluded that Benicar
increases the risk of death. FDA currently believes that the benefits of Benicar
in patients with high blood pressure continue to outweigh its potential risks.

25. FDA Drug Safety Communication: Safety Review Update
of Benicar (olmesartan) and cardiovascular events

After reviewing the results of the ROADMAP and ORIENT trials,1-2 FDA
has determined that the benefits of Benicar continue to outweigh its potential
risks when used for the treatment of patients with high blood pressure
according to the drug label.
Benicar is not recommended as a treatment to delay or prevent protein in
the urine (microalbuminuria) in diabetic patients.
Daiichi Sankyo, the makers of Benicar, have agreed to work with the FDA
to perform additional studies, as well as conduct additional analyses of
completed clinical studies, to obtain more complete information about the
cardiovascular risks or benefits of Benicar in various clinical settings. FDA
will update the public when new information is available.

30. Aliskiren-containing Medications: Drug Safety Comunication - New
Warning and Contraindication

AUDIENCE: Internal Medicine, Pharmacy, Endocrinology
RECOMMENDATION:
• Concomitant use of aliskiren with ARBs or ACEIs in patients with
diabetes is contraindicated because of the risk of renal impairment,
hypotension, and hyperkalemia.
• Avoid use of aliskiren with ARBs or ACEIs in patients with renal
impairment where GFR < 60 mL/min.

31. •Alerta de l’Agència Britànica del Medicament: risc d’angioedema en
pacients tractats amb aliskirèn (efecte advers rar, però greu).
En cas d’aparició de símptomes d’angioedema, retirar el fàrmac de
manera immediata.
•També s’han notificat casos d’insuficiència renal aguda en pacients
amb FR de disfunció renal (hipovolèmia o malaltia cardíaca, hepàtica o
renal). En aquests pacients, així com en els que presenten estenosi
unilateral o bilateral de l’artèria renal, es recomana una precaució
extrema si s’administra aquest fàrmac.
•AINE poden reduir l’efecte antihipertensiu de l’aliskirèn. Així mateix, els
pacients d’edat avançada o amb alteració renal poden tenir més risc de
deteriorament de la funció renal quan s’administren de manera
concomitant un AINE i l’aliskirèn. Precaució en pacients tractats amb
AINE.

46. Controvèrsies NICE (resum):
• Evidència d’efectivitat alta diürètics.
• Basa
– models farmacoeconòmics de cost-efectivitat realitzat al Regne
Unit que s'haurien de validar en el nostre entorn. (comparava amlodipí
amb bendroflumetiazida NO comercialitzada).
– NO resultats SALUT: no diferències morbi-mortalitat.
• Considera AC vs D:
– - desenvolupar diabetes mellitus,
– + desenvolupar insuficiència cardíaca.
Qüestiona que la diabetis de nova aparició que s’atribueix als
diürètics tiazídics doni lloc a un major risc d’esdeveniments
cardiovasculars i augmenti la mortalitat.
El model farmacoeconòmic assumeix que qualsevol diagnòstic de
la diabetis s'associa amb el doble risc de mortalitat i altres
esdeveniments de malaltia cardiovascular comparació amb
l'absència de diabetis.

47. Controvèrsies NICE (resum):
• Recomanació clortalidona (12,5–25 mg/24h) i indapamida
(1,5 mg/24h retard o 2,5 mg/24h) (RE HC només comercialitzada
en associació):
La reducció de la pressió arterial és similar en tots
aquests diürètics tiazídics.
• Clortalidona 50 mg, que és entre 2 i 4 vegades la dosi
recomanada. A més, la situació real a Espanya és que la
indústria farmacèutica ha comercialitzat preferentment la
hidroclorotiazida, que es troba en nombroses
presentacions en combinació amb un IECA i que ofereix
comoditat als pacients.

ser-ho?????

50. • En la subanàlisi de l’ALLHAT (Diabetis Care 2008;31:353-60):
– diabetis de nou diagnòstic
• Sense síndrome metabòlica aguda: significatiu (diferència 0,16 mmol/L).
• Síndrome metabòlica va ser menor amb lisinopril S amlodipi NS.
– Clortalidona millor en variable combinada que incloïa l’ictus, en
prevenció d’IC i en mortalitat (vs amlodipina i vs lisinopril).
– Anàlisi post hoc, cal interpretar els resultats amb cautela.
Conclusió:
malgrat un perfil metabòlic menys favorable, els diürètics tiazida-like
en el tractament inicial de la HTA ofereixen un efecte similar pel què
fa a outocmes cardiovasculars (i de vegades superior) en comparació
amb IECAs i BCC.
• Wright et al van tornar a avaluar aquestes dades de l’ALLHAT juntament amb
altres anàlisis posteriors i els resultats d’altres assaigs i metanàlisis (Arch
Intern Med 2009;169:832-42).
– Objectiu: avaluar IC i diabetis de nou diagnòstic, així com les seves
conseqüències cardiovasculars.
Conclusió:
Ni els alfa-bloquejadors, ni els IECA ni els BCC sobrepassen els
diürètics tipus tiazides, a les dosis adequades, en el tractament inicial
quant a reducció de risc cardiovascular o renal.
Les tiazides s’associen amb menys insuficiència cardíaca, i la diabetis
de nou diagnòstic amb què s’associen no augmenta els outcomes
cardiovasculars.

No extrapolable a Espanya.
Basat només estudis farmacoeconòmics.
No diferències de morbi-mortalitat.
Els diürètics tiazídics a dosis baixes segueixen sent
fàrmacs de primera elecció en l'HTA no complicada
en joves i gent gran i en la hipertensió sistòlica
aïllada, Guia Ministeri també HTA estadis 1 i 2 associada 1
factor addicional.
