Bronkopneumonia adalah infeksi paru yang menyebabkan peradangan dan konsolidasi pada bronkiolus dan alveolus. Pneumonia ini sering terjadi pada pasien yang memiliki gangguan refleks batuk, fungsi silia, atau makrofag alveolus akibat usia lanjut, obat-obatan, merokok, alkohol, atau penyakit lain. pneumonia:
•Banyak dari mereka tidak batuk dan membersihkan
paru-paru sebagai seharusnya karena obat-obatan,
usia lanjut, kelemahan fisik, fibrosis paru, penyakit
lain, atau apapun;
•Banyak dari mereka memiliki fungsi elevator
mukosilier yang buruk akibat merokok, infeksi
penyakit lain, atau apapun;
4. Mengapa orang sakit menderita
pneumonia:
•Banyak dari mereka memiliki fungsi makrofag alveolus
yang buruk karena merokok, terapi oksigen,
alkoholisme, atau apapun;
•Udem paru dari apapun penyebabnya adalah media
kultur yang besar
•Bahan setengah cair (glop) dalam paru-paru (cystic
fibrosis, sumbatan jalan napas, “bronkitis kronik”, dan
lain-lain) membantu membuat paru-paru terinfeksi.
5. •Bronkopneumonia biasa merupakan komplikasi dari
campak dan pertusis. Bronkopneumonia sekunder
yang disebabkan oleh stafilokokus merupakan
pembunuh utama pada pandemi influenza tahun
1918.

• Bronkopneumonia ini lebih jelas,
tetapi ada daerah dengan
konsolidasi yang lebih terang. Hilus
terlihat di kanan bawah dengan
arteri pulmonalis dan bronki.
Banyak bronkopneumonia
mengikuti pneumonia virus,
khususnya pada orang yang lebih
tua pada musim dingin saat
influenza sering terjadi.
18. •Meskipun sering, bronkopneumonia adalah infeksi
oportunistik, dan pasien sudah sakit dengan sesuatu
yang memungkinkan bakteri untuk meraih pijakan
•Banyak orang dengan bronkopneumonia memiliki
gangguan refleks batuk, akibat usia tua, anestesi,
obat-obatan, nyeri, penyakit yang membuat pasien
menjadi kurus, paralisis.

virus), sulit untuk membersihkan bakteri.

kemampuan makrofag alveolus untuk membunuh
bakteri, dan bahkan terapi oksigen diperkirakan dapat
melakukan hal ini.
20. •Bila sekresi berkumpul di dada (bronkitis kronik yang

menyumbang, atau hanya tidak dapat batuk secara
baik), bakteri akan memiliki tempat yang baik untuk

•Stafilokokus (sesudah influenza), Hemofilus influenza,
pseudomonas, dan yang lain adalah penyebab
pneumonia lobaris yang terkenal pada orang dengan
kerusakan daya tahan paru.
37. Empat tahap pneumonia lobaris:
•HIPEREMIA DAN UDEM: kuman membelah dengan
cepat, dan pembuluh darah berdilatasi dan cairan
tumpah sebagai respon terhadap cedera
•HEPATISASI MERAH: peradangan berlangsung, dan
pembuluh darah sekarang mengeluarkan fibrinogen,
yang membentuk jaringan fibrin dalam alveoli.
Sebagian sel darah merah dikeluarkan oleh kerusakan
pada pembuluh darah, dan neutrofil datang untuk
memerangi bakteri. Eksudat alveolus menjadi
“sputum berkarat”.
38. •HEPATISASI ABU-ABU: fibrin mendominasi gambaran,
saat sel neutrofil dan sel darah merah (“abu-abu”
karena perdarahan tidak lagi terjadi dan sel darah
merah sudah hancur). Fibrin mungkin membentuk
bola dalam alveoli sebagai “ikatan fibrin”.
•RESOLUSI: plasmin membersihkan fibrin, dan paru-

39. •KOMPLIKASI: permukaan pleura yang terkena infeksi
hampir selalu terlibat, bertanggung jawab terhadap
nyeri dari pneumonia lobaris. Akan ada adesi
fibrinosa, yang mungkin sembuh atau berubah

40. •KOMPLIKASI: Pada pneumonia lobaris Klebsiella,
stafilokokus, atau pseudomonas, seringkali terbentuk

mereka menjadi serbuk kaseosa (semua sel di daerah
tersebut menjalani apoptosis dan menjadi berkeping-
keping), yang mana diharapkan basilus bisa
dibatukkan keluar.
66. •Ide bahwa tubuh merespon berlebihan terhadap
organisme yang dibenci telah dikonfirmasi dengan
penemuan mikobakterium tidak virulen yang
dimodifikasi secara genetik masih menginfeksi, tetapi
tidak merekrut sel-sel lain, dan menyebabkan sedikit
bahaya
67. •TUBERKULOSIS PRIMER terjadi bila basilus TB pertama

cenderung menjadi lesi fokal.
Terlihat disini dalam sebuah kelenjar
limfe hilus adalah sebuah
granuloma. Granuloma karena agen
infeksius seperti mikobakterium
seringkali digambarkan sebagai
“kaseosa” bila mereka memiliki
nekrosis kaseosa yang jelas.
69. •Basil-basil menemukan jalan mereka ke kelenjar llimfe
regional, dan dalam beberapa minggu, granuloma
memiliki dinding basil dalam dua lokasi. (kombinasi
lesi di paru-paru dan kelenjar disebut KOMPLEKS
GHON). Basil yang dapat hidup tetap dalam fokus /
kompleks
Ghon seumur hidup.
•Banyak TB primer yang tidak menunjukkan gejala.
70. •TUBERKULOSIS SEKUNDER (“TB aktif”, tuberkulosis
post primer”, “tuberkulosis dewasa”, “tuberkulosis
infeksi ulang”, “tuberkulosis kavitasi”) terjadi bila
basil-basil lepas dari fokus Ghon asal atau lebih
banyak basil yang masuk ke dalam tubuh dari luar.
82. •Basil-basil mungkin dikeluarkan dari fokus Ghon oleh
kanker yang menyerang atau “oleh penurunan daya
tahan tubuh”.
•Infeksi biasanya muncul kembali pada apex dari satu
atau kedua paru-paru (“fokus-fokus Simon” – basil-
basil TB sebenarnya memasuki aliran darah, tetapi
tumbuh terbaik pada apex paru dimana oksigen paling
banyak.) Hal ini adalah satu penyebab dari
“pneumonia putih” (“pneumonia alba”, yang lain
adalah sifilis).
83. •Semakin baik imunitas cell-mediated, semakin klasik
granulomanya
•Neutrofil paling banyak saat caseosa mengalami erosi
ke saluran pernapasan besar (bakteri yang sering ada
dapat memasuki bahan mati). Juka ingat bahwa TB
cenderung mengalami pengapuran (mudah bagi
radiologis yang ingin mengatakan ini dari kanker)
91. •Sebaliknya, TUBERKULOSIS PARU PROGRESIF
menyebar ke seluruh paru dan dapat menghasilkan
“empiema tuberkulosis” dengan melibatkan rongga
pleura.
•Saat bagian besar dari paru menjalani nekrosis
kaseosa, perluasan ke saluran pernapasan besar
menyebabkan semua debris dibatukkan – demikianlah
yang “diinginkan” oleh basil TB, karena dengan cara
itulah dia ditularkan ke orang lain. Hasilnya adalah
sebuah LUBANG.
92. •TB juga rentan menginfeksi laring dan saluran
pernapasan yang lebih besar (“TB trakeobronkial”)
dan, karena basil-basil ditelan, mempengaruhi usus.
•TB MILIER terjadi bila banyak basil-basil TB masuk ke
aliran darah tetapi respon granulomatosa bagus.
•Bila TB ruptur ke arteri pulmonalis, ada keterlibatan
milier dari bagian paru-paru, sedangkan ruptur ke
vena menghasilkan keterlibatan milier dari seluruh
tubuh.
93. •Sebenarnya membuat diagnosis untuk kasus yang
dipertanyakan sulit meskipun di era kedokteran
molekuler. Memerlukan waktu beberapa minggu
untuk menumbuhkan mikroba. Saat ini teknik DNA
tidak terlalu sensitif; penelitian gen untuk mendeteksi
resistensi cukup baik untuk rifampin tetapi tidak untuk
isoniazid.
94. Tuberkulosi

proses granulomatosa

metastasis.
96. Tuberkulosis
• Pada inspeksi yang lebih dekat,
granuloma memiliki daerah nekrosis
kaseosa. Ini adalah penyakit
granulomatosa yang sangat luas. Pola dari
granuloma kaseosa multipel ini terutama
pada lobus atas adalah karakteristik
utama dari tuberkulosis sekunder
(reaktivasi). Meskipun demikian,

97. Bagaimana kavitas
terbentuk
• Saat ada kaseosa luas dan granuloma
melibatkan bronkus yang lebih besar,
hal ini memungkinkan bagi pusat
nekrosis yang lunak untuk mengalir
keluar dan meninggalkan sebuah
kavitas (lubang). Kavitasi adalah khas
untuk granuloma besar dengan
tuberkulosis. Kavitasi lebih sering di
lobus atas.
99. Kompleks Ghon
• Ada granuloma sub pleura di lapang paru
tengah di kanan. Dalam hilus ada
granuloma kuning kecil dalam kelenjar
limfe hilus dekat bronkus. Ini adalah
“kompleks Ghon” yang merupakan
karakteristik kasat mata dari tuberkulosis
primer. Pada sebagian besar orang,
penyakit granulomatosa tidak akan
berkembang. Dari waktu ke waktu ukuran
granuloma akan berkurang dan dapat
mengapur, meninggalkan titik
pengapuran pada radiografi dada yang
menunjukkan penyakit granulomatosa.
105. TB milier
• Bila respon imun buruk atau dikalahkan
oleh infeksi yang luas, maka
memungkinkan untuk melihat pola kasat
mata dari penyakit granulomatosa yang
terlihat disini. Ini adalah pola
granulomatosa “milier” karen ada banyak
granuloma kecil, berukuran sekitar 2-4
mm, tersebar diseluruh parenkim paru.
Pola milier mendapatkan namanya dari
kemiripan dari granuloma dengan biji
millet.
107. •Pengingat: Pada kebanyak penyakit peradangan
kronik, clubbing dari kuku (“perubahan Hippocratic”)
seringkali muncul karena embolisasi megakariosit
melalui saluran pembuluh darah yang terbentuk di
paru-paru.

KARSINOMA BRONKOGENIK

Bronchopneumonia affects one or more lobes, being frequently bilateral and basal. Macroscopically, one can identify multiple foci of condensation (1 - 3 cm diameter), white-yellowish, imprecisely circumscribed, centered by bronchiole, separated by normal lung parenchyma. In children, it has a tendency to confluence, resulting in large condensation area (pseudolobar pneumonia)
Microscopy : foci of inflammatory condensation centered by a bronchiole with acute bronchiolitis (suppurative exudate rich in neutrophils in the lumen, foci of ulceration of the epithelium and parietal inflammation). The alveolar lumens surrounding the bronchia are filled with neutrophils ("leukocytic alveolitis"). Capillaries in the alveolar walls show congestion. Inflammatory foci are separated by normal, aerated parenchyma.
The cut surface of this lung demonstrates the typical appearance of a bronchopneumonia with areas of tan-yellow consolidation. Remaining lung is dark red because of marked pulmonary congestion. Bronchopneumonia (lobular pneumonia) is characterized by patchy areas of pulmonary consolidation. These areas become almost confluent in the left lower lobe on the bottom left of the photograph.The areas of consolidation are firmer than the surrounding lung.This radiograph demonstrates patchy infiltrates consistent with a bronchopneumonia from a bacterial infection. Typical organisms include Streptococcus pneumoniae, Staphylococcus aureus, Pseudomonas aeruginosa, Hemophilus influenzae, Klebsiella pneumoniae, among others.
At the left the alveoli are filled with a neutrophilic exudate that corresponds to the areas of consolidation seen grossly with the bronchopneumonia. This contrasts with the aerated lung on the right of this photomicrograph.
At higher magnification can be seen a patchy area of alveoli that are filled with inflammatory cells. The alveolar structure is still maintained, which is why a pneumonia often resolves with minimal residual destruction or damage to the lung.
At high magnification, the alveolar exudate of mainly neutrophils is seen. The surrounding alveolar walls have capillaries that are dilated and filled with RBC's. Such an exudative process is typical for bacterial infection. This exudate gives rise to the productive cough of purulent yellow sputum seen with bacterial pneumonias.
More virulent bacteria and/or more severe pneumonias can be associated with destruction of lung tissue and hemorrhage. Here, alveolar walls are no longer visible because there is early abscess formation. There is also hemorrhage.
At higher magnification, early abscessing pneumonia is shown. Alveolar walls are not clearly seen, only sheets of neutrophils.
This more focal abscess containing a neutrophilic exudate as well as dark blue bacterial colonies suggests aspiration or hematogenous spread of infection to the lung. Aspirated material from the oral-pharyngeal region contains bacterial flora. Hematogenous spread of infection to lungs could occur from septicemia or from infective endocarditis involving the right side of the heart.
Case 66. Aspiration pneumonia (69 y-o F) Pneumonia seen in a dysphagic patient suffering from esophageal cancer E. coli antigens (lipopolysaccharides) are demonstrated in the cytoplasm of phagocytic cells (immunostaining). Opportunistic infection of Gram-negative enteric bacteria is indicated.
Case 66. Reference case 66A Foreign body granulomatous reaction around the respiratory bronchiole (HE). In this case, bronchopneumonia is associated with foreign body reaction against aspirated food material. Pathogens are commonly anerobic oral flora, includingStreptococcusviridans group
Aspiration bronchopneumonia appears when early respiratory movements during labor, facilitate filling of alveolar ducts and alveolar lumens with elements of amniotic fluid : amniotic cells, squames and squamous cells from fetal skin, lanugo,meconium. Reduced inflammatory infiltrate (neutrophils) and capillary congestion is present. (H&E, ob. x20)
There is a localized foreign body giant cell response to the aspirated material seen here at high magnification. Aspirated material may also produce inflammation from chemical irritation, as with gastric contents.
This foreign body is a fragment of pulse. Pulses are seeds of leguminous plants--beans, peas, lentils, peanuts. When these particles are found in the lung, they are diagnostic of aspiration. The diagnosis is aspiration pneumonia.
Case 67. Pseudomonas pneumonia (39 y-o M) Autopsied lung in a leukemia patient after chemotherapy, accompanyingPseudomonas septicemia Methylene blue stain identifies numerous rods clustered along the alveolar septa.
This is an abscessing bronchopneumonia in which several abscesseswith irregular, rough-surfaced walls are seen within areas of tan consolidation. Lung abscesses, if large enough, will contain liquefied necrotic material and purulent exudate that often results in an air-fluid level by chest radiograph in the abscess.
Seen here are two lung abscesses, one in the upper lobe and one in the lower lobe of this left lung. There are adjacent areas of tan consolidation with bronchopneumonia. An abscess is a complication of severe pneumonia, most typically from virulent organisms such asStaphylococcusaureus. Abscesses are often complications of aspiration, where they appear more frequently in the right posterior lung.
Seen here are lung abscesses grossly in which the purulent exudate has drained following sectioning to reveal the abscess cavities. Abscesses can be a source for septicemia and are difficult to treat.
The pleural surface left demonstrates thick yellow-tan purulent exudateand the pleural cavity is filled with purulent exudate. This is anempyema. Pneumonia may be complicated by a pleuritis. Initially, there may just be an effusion into the pleural space. There may also be a fibrinouspleuritis. However, bacterial infections of lung can spread to the pleura to produce a purulent pleuritis. A collection of pus in the pleural space is known as empyema.
This is a lobar pneumonia in which consolidation of the entire left upper lobe has occurred. This pattern is much less common than the bronchopneumonia pattern. In part, this is due to the fact that most lobar pneumonias are due to Streptococcus pneumoniae (pneumococcus) and for decades, these have responded well to penicillin therapy so that advanced, severe cases are not seen as frequently. However, pneumoccoci, like most other bacteria, are developing more resistance to antibiotics. Severe pneumococcal pneumonia still occurs, even in young to middle aged persons (not just the very young and the very old) and has a mortality rate of 20%!
A closer view of the lobar pneumonia demonstrates the distinct difference between the upper lobe and the consolidated lower lobe. Radiographically, areas of consolidation appear as infiltrates.

Case 76. Reference case 76A Caseous granuloma at the periphery of the lung (gross findings). Within the central caseation, pre-existing structures such as vessels and bronchioles can be traced, indicating coagulation necrosis.
On closer inspection, caseous tan necrotic tissue is seen to constitute the granulomas in this gross appearance of a Ghon complex. Most patients with primary tuberculosis are asymptomatic, and the granulomas resolve.
Extensive cavitation of multiple granulomas of lung are typical for secondary tuberculosis from reactivation of primary infection or reinfection as an adult. Such lesions have a predilection for appearance in the upper lobes of the lung (left)The caseous necrosis is extensive, and cavitation is prominent. Such patients can be highly infectious (right)

An acid fast stain is used to diagnose the presence of mycobacteria in tissue and cytologic preparations. Note the thin red rod-like organisms.
More sensitive than an acid fast stain, this auramine stain requires fluorescence microscopy. Note the yellow-orange rod-like mycobacteria. Of course, the most sensitive method for detection of mycobacteria is microbiologic culture.
This spleen shows a miliary pattern of granulomatous inflammation, with numerous small tan granulomas. This suggests a poor immune response. This patient had AIDS. The infection turned out to beMycobacterium avium-intracellulare (MAI), also known asMycobacterium avium-complex (MAC).
With a poor immune response to the agents producing granulomatous inflammation, there can be extensive spread of infection with the production of a "miliary" pattern of granulomas, as seen here in the lung of a patient with miliary tuberculosis. The 1 to 2 mm granulomas are scattered around like millet seeds (millet is a type of cereal grain).
Here is another more focal abscess in the lung. The alveoli in that area have been destroyed.
CT appearance: CT shows multiple, small (1-3 mm) nodules that are randomly positioned in the parenchyma, and along the pleura and fissures. They are especially well seen in the right upper lobe. This random pattern is the CT equivalent of a miliary pattern and is distinguished from a pure centrilobular pattern by nodules seen at the pleural surfaces and at the ends of vessels. The nodules appear randomly, rather than uniformly, spaced, as in centrilobular nodules
Active stages: This picture of a multilobular consolidation shows central necrosis (between arrows). After liquifaction, this material could be discharged into an airway. Note also the small, white, satellite, centrilobular nodules, which represent past bronchial and bronchiolar spread.The faint lines (single arrow) represent interlobular septa thought to be widened by edema. A centrilobular nodule lies just to the left of the arrow. Note that the septal lines surround the nodule to form a polygon that defines the lobule.
The oldest lesion is an irregular, mass-like consolidation with central cavitation anteriorly on the right. Several other focal consolidations, one cavitary and two bordering the fissure, are also present. Architectural distortion about the large cavity, and focal retraction of the major fissure adjacent to a focal consolidation are consistent with fibrosis. These consolidations likely developed from coalescence of centrilobular nodules.More recent endobronchial spread of tuberculosis is seen as the smaller, branching opacities, which represent diseased bronchi/ioles, and the small centrilobular nodules (2-4 mm), which represent bronchiolar/acinar disease. As these become confluent, larger macronodules may form, as seen as a solitary nodule in the left upper lobe.
The arrow marks a centrilobular nodule that is coalescing with two similar nodules below it to form a macronodule. The dark blue center represents lumenal exudate.The macronodules enlarge and merge to form the larger consolidations with irregular margins, which represent continuing endobronchial spread of disease.
Tuberculosis is caused by Mycobacterium tuberculosis. This disease is on the rise and the causative agent is gaining resistance to current medications. The characteristic lesion of TB is a caseating (cheesy) granuloma. Caseous material is not seen in this particular slide. In general, a granuloma is characterized by an aggregation of modified macrophages, called epithelioid cells because of their resemblance to squamous epithelial cells. Epithelioid cells may fuse to form a giant cell (Langhan's cell). A Langhan's cell is seen in the bottom left hand corner of this slide-the large pink cell with about 20 nuclei arranged in a horseshoe shape. Granulomas are found in many diseases-sarcoidosis for example. However, caseating granulomas are found in relatively few diseases e.g. TB and some of the fungal infections.
Well-defined granulomas are seen here. They have rounded outlines. The one toward the center of the photograph contains several Langhans giant cells. Granulomas are composed of transformed macrophages called epithelioid cells along with lymphocytes, occasional PMN's, plasma cells, and fibroblasts. The localized, small appearance of these granulomas suggests that the immune response is fairly good.
At low magnification, this photomicrograph reveals multiple granulomas. Granulomatous disease by chest radiograph can appear as reticulonodular densities.
The edge of a granuloma is shown here at high magnification. At the upper right is amorphous pink caseous material composed of the necrotic elements of the granuloma as well as the infectious organisms. This area is ringed by the inflammatory component with epithelioid cells, lymphocytes, and fibroblasts.
At high magnification, the granuloma demonstrates that the epithelioid macrophages are elongated with long, pale nuclei and pink cytoplasm. The macrophages organize into committees called giant cells. The typical giant cell for infectious granulomas is called a Langhans giant cell and has the nuclei lined up along one edge of the cell. The process of granulomatous inflammation takes place over months to years. (Did you ever hear of a committee action that was completed in a short time? Think of a granuloma as a "task force".)
In order to find the mycobacteria in a tissue section, a stain for acid fast bacilli is done (AFB stain). The mycobacteria stain as red rods, as seen here at high magnification.
At closer range, the miliary pattern is seen throughout the lung. Dissemination of the infectious agent (M. tuberculosis, fungi) may produce a similar pattern in other organs.
This is another sqamous cell carcinoma that extends from hilum to pleura. The black areas represent anthracotic pigment trapped in the tumor.
The specimen consists of the right lung, which has been bisected to display a tumour which arises from the main bronchus, approximately 1 cm below the origin of the upper lobe bronchus, and extends behind the bronchus into the adjacent lymph nodes, forming a tumour mass some 6 cm in diameter. The lower and middle lobes have collapsed secondary to the bronchial obstruction, and the cut surface shows fine, yellow flecking, while the bronchi contain yellowish, thick pus.
Nests of tumor have small cells at the edge and larger, more differentiated cells with more cytoplasm centrally. Intercellular bridges (desmosomes) or keratin pearls (arrow) must be present for diagnosis.
Intercellular bridges are present between tumor cells (area below arrow).
This is the microscopic appearance of squamous cell carcinoma with nests of polygonal cells with pink cytoplasm and distinct cell borders. The nuclei are hyperchromatic and angular.
In this squamous cell carcinoma at the upper left is a "squamous eddy" with a keratin pearl. At the right, the tumor is less differentiated and several dark mitotic figures are seen.
Squamous cell carcinoma involving a subsegmental bronchus with distal chronic obstructive pneumonia. The tumor is seen as a rounded nodule, approximately 2 cm in diameter, proximal to a more irregular focus of chronic obstructive pneumonia with fibrosis.

The tumor is an adenocarcinoma because it lines spaces, forms glands, and produces mucin. The lack of cohesion of the cells is a postmortem artefact.

LOWER RESPIRATORY TRACT: SOLID ADENOCARCINOMA WITH MUCIN PRODUCTIONThis poorly differentiated (solid) adenocarcinoma shows many cells with mucin vacuoles stained with digested PAS. The vacuoles were difficult to see on routine sections, and the case had originally been classified as large cell carcinoma.
Microscopically, the adenocarcinoma-in-situ (formerly bronchioloalveolar carcinoma) is composed of columnar cells that proliferate along the framework of alveolar septae, a so-caled "lepidic" growth pattern. The neoplastic cells are well-differentiated. These neoplasms, a form of adenocarcinoma, in general have a better prognosis than most other primary lung cancers, but they may not be detected at a low stage.
The lungs were heavy (R 2020 g, L 1950 g; normal 250 g apiece). This slice of the left lung shows multiple, grey consolidations of various size and shape scattered throughout both lobes. The bronchi appear patent but contained clear mucus. The parenchyma also has a glistening, mucoid appearance that is evident to the left of the main airway and elsewhere (highlights). Between the consolidations, lung parenchyma is somewhat firmer than normal. There is no evidence of fibrosis. Vessels are patent.
LOWER RESPIRATORY TRACT: BRONCHIOLOALVEOLAR CARCINOMA, NONMUCINOUS TYPEThis gross photograph is from the lungs of a patient who died of widespread bilateral BAC. A cut surface of the formalin-inflated lungs shows multiple, small, gray-white nodules with absence of necrosis, hemorrhage, or architectural distortion. The bronchi course normally through the lung tissue, and the presence of air bronchogramsradiographically in such cases is not surprising.
LOWER RESPIRATORY TRACT: MUCIN STAINING IN BRONCHIOLOALVEOLAR CARCINOMADespite the fact that this BAC does not have obvious goblet cell features and the cells are eosinophilic, there is abundant luminal mucus secretion, in addition to occasional cells with intracytoplasmicmucin production as seen with a digested PAS stain.A histologic section shows a proliferation of atypical cells along the alveolar walls (A).

LOWER RESPIRATORY TRACT: LARGE CELL CARCINOMA This large cell carcinoma at autopsy shows a large multilobulated tumor adjacent to the hilum. A metastatically involved lymph node is present next to the bronchus.

This is the microscopic pattern of a small cell anaplastic (oat cell) carcinoma in which small dark blue cells with minimal cytoplasm are packed together in sheets.
Azzopardi phenomenon: In necrotic areas of tumor, dark blue DNA liberated from dead cells sometimes coats vessel walls (arrow). The tumor cells surrounding the vessels show coagulative necrosis. The pink area at the lower right shows karyolysis of tumor nuclei.
Vascular invasion of tumor:The rounded nests of tumor with central necrosis (arrow) lie in a distended vein. Such tumor may be disseminated systemically.
Spindled cells: Here, tumor cells show elongation or spindling of nuclei, a common feature of all types of neuroendocrine tumor from tumorlet to small cell carcinoma. The small, dark nuclei represent dying tumor cells, mitotic figures, or lymphocytes.
Oat Cell CarcinomaThis is the same as small cell bronchogenic carcinoma. It is highly malignant and spreads very quickly.In this picture you can see how the cells mould one another, revealing their epithelial origin. You can find several cells with pyknotic or karyorrhecic nuclei. You can see some outlines of dead cells too, and some oftheir protein debris.If you have sharp eyes, you may even see a pigment-laden macrophage. The pigment may be iron from bleeding, or pigment from tobacco itself.Oat cell carcinomas never make much of a fibrous stroma.Of course there is no normal lung tissue left. This form of lung cancer occurs only in smokers. The rare exceptions have been in uranium / radium miners.Oat cell carcinoma usually responds well to chemotherapy and stays in remission for at least a few months.

Congenital and pediatric neoplasms are uncommon. One type that can occur is a teratoma. Shown here is a large nasopharyngeal teratoma that is protruding from the oral cavity.
CT scan and MR of the brain revealed a heterogeneous mass which filled the paranasal sinuses and nasal cavity, with erosion of the right orbital roof, posterior wall of the sphenoid sinus, posterior wall of the right frontal sinus, bilateral fovea ethmoidalis and cribriform plates. The mass extended into the right orbit, through an eroded lamina papyracea. In addition, MR of the skull base, paranasal sinuses and brain revealed an obstructed concha bullosa on the left. The findings of the CT and MR studies were interpreted as most likely representing polyposis of the paranasal sinuses and nasal cavities.
Structures of the bronchovascular bundle are marked: B = bronchiole, A = pulmonary artery, V = vein, L = lymphatic. Some small bronchial veins that are filled with blood lie to the left of the airway.Note that the veins have thin walls. The artery is narrow and has a thick muscular wall. The endothelial lining of the lymphatics is not visible.Anomali: Presence of large veins in the bronchovascular bundle. Normally, large veins do not accompany the pulmonary artery; they are situated in the interlobular septa. In this case, large veins were present in most bronchovascular bundles of small bronchi and bronchioles. In some cases of this entity, the anomalous veins may not be present in every bundle.
Metaplasia of laryngeal respiratory epithelium has occurred here in a smoker. The chronic irritation has led to an exchanging of one type of epithelium (the normal respiratory epithelium at the right) for another (the more resilient squamous epithelium at the left). Metaplasia is not a normal physiologic process and may be the first step toward neoplasia.
