We would like to thank you in advance for reviewing this request for information (RFI) and assisting the State
of Illinois (State) in this effort. The State very much appreciates your efforts and values your input.
This RFI is for informational purposes only and will not result in an award of a contract.
The Illinois Health Information Exchange Authority (ILHIEA) is issuing this RFI to obtain information from
responders regarding third-party software designed to extend the functionality of the Illinois Health Information
Exchange (ILHIE). This third party software would be known as ILHIE Apps (ILHIE Apps). ILHIE Apps will
be similar to applications that run on smartphones or personal computers.
The response to this RFI will help ILHIE create and/or share with other states an organized system for
developing ILHIE Apps and distributing them to ILHIE users or clients. This will involve a set of application
programming interfaces (APIs) for the use of ILHIE App developers, a fair and impartial certification process to
approve proposed ILHIE Apps, and a set of templates for agreements or contracts relating to ILHIE Apps.
The information provided in response to this RFI is considered the property of the State.
Do not provide
information that you believe is confidential or proprietary to your entity as it will not be kept confidential, even
if so marked. All information received in response to this RFI will be available for public review.
This RFI is not a commitment by the State to approve any responder as a developer or user of ILHIE Apps, to
certify specific ILHIE Apps, or to follow any other courses of action. The State does not promise or guarantee a
reply or other further contact between the State and responders to this RFI. Also, please note that the State
cannot reimburse responders for any expenses associated with responding to this RFI.

Overview
1) The State of Illinois has developed the Illinois Health Information Exchange (ILHIE), a statewide
electronic network to securely share clinical and administrative data according to national standards.
Under state law, it is charged with facilitating secure data exchange among health care providers, health
care payers, government agencies, human services agencies and providers, individual patients, and other
authorized ILHIE users in order to improve health outcomes, reduce medical errors and improve public
health reporting.
2) As part of this project, the State plans to use the ILHIE as the platform for the ILHIE Apps. ILHIE
Apps will be created by independent third parties to expand the functionality and positive impact of the
ILHIE infrastructure. In this way ILHIE is creating a dynamic marketplace that can serve its clients,
improve health outcomes for Illinois residents and evolve as ILHIE participant needs and technology
advance.
3) ILHIE services that will be available through the platform include the items listed below. Only
authorized users that have been properly vetted, positively identified and are legally contracted with
ILHIE as authorized 3rd
party vendors will be qualified to offer applications using the ILHIE platform.
a. Authorization services where an application can verify the credentials of a user of its software.
b. Access by authorized users to the ILHIE statewide Master Patient Index (MPI) for appropriate
uses, containing demographic and other cross-reference information.
c. IHE and/or ONC compliant exchanges via web services or SMTP (or other protocols as they
become standards in electronic health exchanges).
d. Audit functionality where ILHIE will store and maintain audits of all transactions
e. Clinical repositories that are connected to ILHIE directly or through other secure connections
that follow national standards and protocols such as offered by Healtheway.
f. Repositories of relevant human services data.
g. Other infrastructure services as they become available.
4) Third-party products would be able to securely interface with ILHIE APIs in order to provide authorized
ILHIE clients with an expanded group of services.

5) It is anticipated that a State-designated certifying body will evaluate and approve proposed ILHIE Apps
that meet specified requirements including security, privacy, reliability, compatibility, and compliance
with all applicable laws.
6) It is anticipated that once an ILHIE App is certified, its owner may enter into a contract with the State of
Illinois for the right to sell or otherwise distribute its certified ILHIE App to ILHIE clients.
7) It is anticipated that each ILHIE client would decide which ILHIE Apps to acquire for its own use, and
an online marketplace would allow different ILHIE Apps to compete for customers.

ILHIE Apps Project Vision
The ILHIE’s clients include hospitals, clinics, private medical practices, universities, community organizations,
insurance companies, government agencies, other payors, and other HIEs as well as individual patients. These
clients have widely varying needs, and the number of potential services from which they could benefit is large.
To better meet the needs of ILHIE clients and to make a positive impact to improve patient care, the State plans
to allow third parties to create ILHIE Apps for the ILHIE platform. IHLIE Apps could provide services beyond
what the State itself is able to provide. An online marketplace would allow ILHIE clients to purchase or
otherwise acquire ILHIE Apps for their own use. This platform strategy would maximize the value of the ILHIE
by vastly increasing the number of ILHIE services available.
There is no limit on the number or variety of ILHIE Apps that can be created. In fact, the ILHIE is expected to
become more valuable to clients when more ILHIE Apps are available.
In order to maximize vendor participation, ILHIE wants to ensure this project is consistent with other similar
efforts such as those being undertaken at the New York eHealth Collaborative (NYeHC). As a result, ILHIE
has asked and is asking other states to participate in this process. It is critical that all states that wish to
collaborate in this process enable the same APIs. Each state may implement these APIs differently, but from
the point of view of API users, each state’s system will appear to be identical. In particular, the NYeHC has
agreed to partner with ILHIE. The NYeHC is already pursuing this third-party program.
This RFI will conclude with a webinar where ILHIE will present the results of the survey. In addition the
results will be posted on the ILHIE website. In addition, if ILHIE feels the response is strong enough, ILHIE
will announce the formation of a process to create a multi-state governance process and will make some initial
suggestions on what this process should look like and who the members should be and how they should join.
Proposed Elements of ILHIE Apps
! App Center
o An online ILHIE App Center would provide a centralized application store or marketplace,
allowing clients to select and acquire ILHIE Apps.

o Different ILHIE Apps could compete for customers in this marketplace based on price, quality,
and overall value to clients.
o The App Center would also serve as a resource providing information on ILHIE Apps to
developers, clients, and the general public.
! Impartial certification process
o Any ILHIE App offered to clients must first be approved by the State using a rigorous and
ongoing certification process.
o This process would require each ILHIE App to meet specified conditions including criteria for
compatibility, reliability, security, and legal compliance as well as promised functionality.
o The State would designate an independent entity to conduct the certification.
o The State and the certification body would be neutral on all certified ILHIE Apps and would not
make recommendations.
o Each ILHIE App would be certified based on whether it meets the specified objective criteria,
and would then compete for customers on a level playing field.
! Licensing and Finance
o Once an ILHIE App is certified, its developer must enter into a licensing agreement with the
State of Illinois before its ILHIE App would be placed into the ILHIE App Center.
o The State would license the use of the ILHIE and related technologies (and in some cases,
certain State-owned data) to the developer for purposes of creating, running, and distributing
ILHIE Apps.
o The developer may pay the State a licensing fee or ILHIE may decide to not charge the
developer, depending on how this marketplace evolves. At this point it is difficult to predict
whether or not such a fee will ever become necessary. Since ILHIE is a non-profit, the fee, if
any, expected to be small and should not be a barrier to participation in this program. This issue
will be resolved in the future by the ILHIEA board as well as participating vendors and other
stakeholders.
o Different ILHIE Apps (and the clients who use those ILHIE Apps) may use different amounts of
the IHLIE’s computing or network resources, and licensing fees may need to be adjusted
accordingly.
! Data Use and Access by authorized users.
o ILHIE Apps would be able to access, use, and transmit certain data via the ILHIE, subject to
privacy, security, legal limitations and other specified protections.
o ILHIE Apps would not enable clients to access data that they are not authorized to access by
other means.

o For some purposes, such as medical or scientific research, an ILHIE App may be permitted to
analyze confidential data via the ILHIE and return an anonymized or de-identified result of the
analysis, without revealing the actual confidential data.

Technical Information and Requirements
The ILHIE is an information hub and router dedicated to the exchange of clinical and administrative health data.
In November 2011, after successfully conducting a Request For Proposals to select a Software as a Service
(SaaS) technology vendor, the State of Illinois executed a contract with InterSystems Corporation (ISC) for the
implementation of the ILHIE.
The ILHIE includes the following high-level services:
1. Master Patient Index
2. Provider Index
3. Record Locator Service
4. Identity services, including probabilistic matching service
5. Privacy and security services
a. Login
b. Access auditing
c. SAML pass through
d. Encrypted transmission
e. Encrypted storage (Upcoming)
6. Secure email or messaging (a.k.a. ILHIE Direct)
7. Public Health Node access
8. C32/CCDA aggregation, with other query and aggregation services in the future
9. Access to other State of Illinois health care data as it becomes available.
In addition, the ILHIE’s layered architecture allows the “Business Services Layer” to invoke ISC or third-party
applications that use the lower “Repository Layer” and “Privacy, Security and Consent Layer.” This in turn
enables the creation of third-party applications that can provide additional types of ILHIE services.
