pleasure to offer our services for Consultancy and Maintenance of various records under following
attached Annexure–A.
The detailed job profile for providing consultancy services and undertaking documentation
correspondence & consultation in respect of above enactment is annexed herewith.
All the services agreed as per Annexure – A will be rendered in respect of your establishment as a
principal employer and also in respect of vendor engaged by you.
Our professional fees for the services as per annexure under above enactment shall be Rs.
Per month for undertaking documentation, correspondence, record maintenance & consultation
as per annexure –A.

1. The Employee Provident Fund & Miscellaneous Provisions Act, 1952
 Calculation of PF liability & preparation of Monthly PF Statement.
 Preparation of PF Elecrtonic Challan cum Return.
 Obtaining Form.No-2 from new entrants joined during the month.
 Submission of PF Nomination forms for every new employee on monthly basis
 Maintaining Form.No-11.
 Processing of Form 13 and submission to PF Department.
 Processing of Form 20,10D and Form 5(IF) in respect of Death claims.
 Submission of P.F. Withdrawal forms (for left employees). Filling up and submitting Form 10D
& Form 10C (for pension) with required documents
 Preparation & Submission of Form-9
 Correspondence with PF Authorities whenever required
 Processing of FomNo.31, if any.
 Follow-up/obtaining Form.No-23 (Annual account slips) from the PF Department.
2. The Employee’s State Insurance Act, 1948
 Obtaining Form No. 1 from new entrants and entering the data of the same in the ESIC
website and generating TIC .
 Preparation of monthly ESI remittance statement .
 Filing monthly contribution online.
 Preparation of ESIC SBI challan(online).
 Modification of SBI Challan if required.
 Updating ESIC unrealized challan details online.
 Online updating of the IP.
 Collecting ESI TIC /Permanent Cards (Pahchan card) from ESI Local Office.
 Submission of Accident report in Form No.16 to ESI Dispensaries/ESI Local Office.
 Maintenance of Accident Register.
 Maintenance of Inspection Book.
 Processing Form No. 1A/1B for addition /deletion of members of family. ( Online )
 Processing Form No. 105 availing ESI facilities at outstation
 Processing Form No.86 to avail ESI facilities before getting the ESI TIC/Permanent card.
 Processing Form No.37 to confirm the Re-employment / continuing employment.
 Submission of Annual return in Form.No.01A before 31st of January.

 Maintenance of Salary Register.

4. The Payment of Gratuity Act, 1972
 Submission of Form No.1 to Deputy Labour Commissioner.
 Maintenance of Form F Nomination
 Arranging for Formation of Trust
 Arranging for obtaining the Scheme from Insurance Company.
 Processing Gratuity Application Form.
 Display of Abstract on Notice Board.
5. The Payment of Bonus Act, 1965
 Preparation & Maintenance of Form A, Form B and Form C Register.
 Submission of Annual Return in Form D before 30th November
6. The Employment Exchanges (Compulsory Notification of Vacancies) Act, 1959.
 Notification of Vacancies to be submitted to the Employment Exchange.
 Quarterly Return in ER-1 submission on or before 30th April , 30th July,30th October and 30th
January to the Employment Exchange.
 Submission of the Biennial Returns in ER2 to the department.
7. The Contract Labour ( Regulation & Abolition) Act, 1971
 Obtaining License from Assistant Commissioner of Labour.
 Submission of Annual Return in Form XXV before 15th of February.
 Maintenance of Contractor registers in Form XIII.
 Follow up with contractors to maintain records/registers as per the Act.
8. The Maternity Benefit Act 1961.
 Submission of Form-U before 31st January.
 Maintenance of Register with pertaining to Maternity Benefit Act.
9. The Factories Act 1948.
 Renewal of Factory license before 30th October every year or as the case may be.
 Display of Abstracts.
 Submission of Form-21 before 15th July ( Half Yearly Returns)
 Submission of Form-20 before 15th February (Annual Returns).
 Maintenance of Inspection, Accident, Adult & Census Register.
 Submission of Accident report to Inspector of Factories.
 Intimation of Annual Holidays to Inspector of Factories before 31st of December.
10. Professional Tax.
 Preparation of PT remittance statement
 Preparation of Form No.5-A
 Submission of monthly returns in Form 5-A to professional Tax office before 15th of
every month.
 Submission of Annual Returns in Form No.5 before 30th April.
11. The Gujarat Labour Welfare Fund Act
 Recovery of Rs.3/- to be made from the employee on rolls of the company as on 30th
June & 31st December every year.
 Employer contribution at the rate Rs.6/- per employee along with employees
contribution of Rs.3/- to be remitted to “Welfare Commissioner, Gujarat Labour Welfare
Board” before 15th of July / January
